Summary
Our EHR system lists four providers, but we have successfully acquired only one of
them—Dr. Benjamin Zertuche, MD. The total number of patients recorded in the EHR
stands at 10,445. Based on an estimation approximately 1% of these patients (or around
105 individuals) are likely to be receiving home healthcare services.

However, we currently have only six patients registered in our SST, this highlights a
potential gap. This tells us there may be an alternative paper documentation is still in use.

To address this, we should investigate whether these providers continue to rely on paper
documents for patient records and workflows. By identifying and understanding their
existing processes, we can approach them with the benefits of transitioning to our Doctor
Alliance (DA) platform.

5. Billing Code Analysis

● 5.1 Billing Compliance

We ensure strict compliance for our clients by carefully managing the billing process. Each
month, we gather patient care and demographic information, verify start-of-care dates, and
assign appropriate billing codes, such as G0179, G0180, G0181, and G0182. We perform
audits to ensure all billing complies with Medicare and client standards. Once the review is
complete, the billing sheets are sent to Physician Groups for approval, ensuring that all billable
services are accurately recorded, preventing any revenue loss. Our meticulous approach
facilitates a steady cash flow for Physician Groups and helps Home Health Agencies maximize
their reimbursements.

● 5.2 Billing Efficiency

● We ensure that billing sheets are prepared and sent to clients on time. We ensure we
meet their deadlines consistently. This punctuality helps prevent any delays in
reimbursement, ensuring the PGs don’t lose revenue. By delivering accurate billing
sheets on time, we support their cash flow and enable them to maximize their earnings
without interruptions. This demonstrates our ability to create value for PGs and HHAHs
consistently.

To streamline and expedite the billing sheet preparation process, an automated system can be
implemented with the following enhancements:

1. Automated Previous Month Evaluation: Develop a system that analyzes last month’s
billing data to identify patients with ongoing care needs. This system will flag patients
whose certification (Cert) or recertification (ReCert) dates are outdated but still require
continued billing under G0181.
2. Document Parsing Automation: Automate the extraction of critical data from patient
documents stored on the back office portal. This includes demographic details, Home
Health Agency (HHAH) information, the first six ICD codes, start of care (SOC) dates,
and episode periods for current billing.
3. Automated Billing Code Assignment: Assign billing codes automatically based on the
updated SOC and episode information. For flagged ongoing patients, assign G0181 for
continuity of care. For new patients or those requiring different codes, assign billing
codes according to Cert and ReCert timelines.
4. Efficient Final Billing Sheet Generation: Create a consolidated billing sheet for each
Physician Group that seamlessly includes both ongoing episodes and new patients for
the current month, ensuring accuracy and efficiency in the billing process.

This automated approach minimizes manual effort, reduces errors, and accelerates billing
operations while maintaining compliance and maximizing reimbursements.

1. BSZ Medical PA

Based on the provided data for July, August, September, and October, we observe the following
trends:

1. Cert/Recert Billing Codes (G0179 & G0180):

Month G0179 G0180

July 4 0
August 1 1
September 0 0
October 3 0

The trend in G0179 indicates that the PG constantly monitors the existing patients, and the

San Antonio Billing Code Analysis - Overall Summary

Cert/Recert Billing Codes (G0179 & G0180)

1. G0179:

○There is a mixed trend in G0179 usage across all accounts. While some
accounts, like Centric Physicians Group, show a steady increase, others, like
BSZ Medical PA and Northeast ProHealth, reflect a decline or inconsistency. This
suggests variability in recertifications across physician groups.
○ Minimal use in accounts such as WoundCentrics, LLC (8 instances) indicates
limited certifications or care plan updates.
2. G0180:
○ G0180 consistently shows lower volumes compared to G0181, with most
accounts experiencing sharp declines (e.g., Doctor at Your Service dropped to 1
in September, and Responsive Infectious Diseases Solutions dropped
significantly from July to August).
○ However, some accounts, like Northeast ProHealth, show a partial recovery in
October, suggesting a resurgence in new patient onboarding efforts.

Ongoing Care Management (G0181)

● G0181 is the dominant billing code across all accounts, reflecting its critical role in
patient care management.
○ WoundCentrics, LLC: 85% of billing attributed to G0181, with 243 instances.
○ Centric Physicians Group: A significant rise from August (15) to October (34)
highlights the growing emphasis on ongoing care.
○ Doctor at Your Service: A decline from July (47) to September (36) indicates
fewer patients requiring ongoing care.
○ Responsive Infectious Diseases Solutions: Recovery in G0181 from August
(13) to September (26) suggests efforts to stabilize ongoing care services.
This dominance of G0181 emphasizes the primary focus of the San Antonio
market on sustained care management over certifications or new patient
onboarding.

Overall Market Trends

1. G0179 & G0180:

○ Limited use, highlighting a lack of focus on new patient acquisition and

recertifications. This indicates a need for more aggressive patient acquisition
strategies and better engagement to retain and certify patients.
2. G0181:

○ Strong presence, though some accounts show a gradual decline (e.g., Doctor at
Your Service), suggesting a need for better retention strategies and consistent
care delivery.
3. Total Charges and Patients:

○ WoundCentrics, LLC had the highest total charges ($29,879.00) and 286
patients, with an average charge per patient of $104.47. This reflects its strong
operational performance and effective use of G0181.

Recommendations

1. Boost New Patient Acquisition:

○ Focus on increasing G0180 to onboard new patients and strengthen long-term
care engagement.
2. Retain and Engage Existing Patients:

○ Address declining G0181 trends in some accounts by improving patient retention

strategies and offering additional care management services.
3. Consistent Recertification Efforts:

○ Enhance G0179 usage to ensure existing patients stay within the care cycle,
particularly for accounts showing declining trends.
4. Data-Driven Improvements:

○ Use patient and billing code trends to identify underperforming accounts and
focus resources on areas with significant declines.
5. Collaboration Across Accounts:

○ Leverage successful strategies from high-performing accounts (e.g., Centric

Physicians Group’s increase in G0181) to improve overall outcomes in San
Antonio.

This overall analysis underscores the need for balanced efforts across new patient acquisition,
recertifications, and ongoing care management to maximize patient engagement and revenue in
the San Antonio market.

6. Value and Rapport

● 6.1 Value Fulfillment
○ 6.1.1 Proactive outcomes
○ 6.1.2 Reactive Outcomes
■ Inflow of tickets

reflecting a diverse range of issues impacting client operations.

High-Priority Tickets (6):- These tickets were primarily related to urgent

system access issues and workflow disruptions that demanded
immediate attention to avoid operational delays. For example, San
Antonio Wellness requested a time-sensitive account setup for a private
duty nurse.

Medium-Priority Tickets (12):- These tickets covered recurring

challenges like failed invite links and fax submission errors, impacting
workflow efficiency.
Low-Priority Tickets (11):- Non-critical matters like account creation or
access adjustments were addressed for agencies including Family First
Home Health.

■ Analyzing the most common types of tickets received in Q4

1. Account Management:

New Account Setups: There’s a significant volume of tickets related to

requests for creating new user accounts, primarily driven by seasonal
onboarding or system updates.

Access Adjustments: A considerable number of tickets involve changes

to user access levels, which could include adding permissions, updating
roles, or adjusting credentials.

Unlocking Accounts: There’s also a recurring pattern of tickets

requesting assistance with unlocking accounts, often due to users
forgetting passwords or security verification failures.

2. Document Handling:

Document Access Issues: A large portion of tickets pertains to users

experiencing issues with accessing documents, whether due to
permission errors, broken links, or file corruption.

Document Queuing Delays: Several tickets highlight delays in document

processing or queuing, often caused by system overloads or high-volume
periods.

Document Signing: There are frequent inquiries related to issues with

document signing, such as difficulties in electronically signing or
submitting forms.

● 6.2 Value Communication

1. Data-Driven Communication

● Proactive Outcomes Reporting: Emphasize outcomes such as decreased

administrative burdens, improved compliance rates, and optimized billing processes in
MBRs and QBRs.
● Custom Metrics: Tailor metrics specifically for each PG, showing before and after
scenarios that quantify the direct benefits of your services on their operations.
2. Strategic Reviews

● MBR/QBR Focus: Use these platforms to discuss strategic growth plans, upcoming
changes in healthcare regulations, and how your services align with their evolving
needs. This keeps the PGs informed and strategically engaged.
● Goal Alignment: Regularly align the services provided with the PG's long-term goals,
demonstrating a clear connection between your offerings and their desired outcomes.

3. Enhanced Case Studies and Success Stories

● Highlight Successes: Develop detailed case studies that outline specific instances
where your services have led to significant improvements for PGs. Discuss these case
studies in MBRs and QBRs to provide concrete examples of potential benefits.
● Peer Comparisons: Offer benchmarking insights comparing their performance with
similar PGs in the region, which can help motivate and guide improvements.

4. Feedback Integration

● Feedback Loops: Create structured opportunities during MBRs and QBRs for PGs to
provide feedback on the services received and suggest areas for enhancement.
● Responsive Adjustments: Quickly implement changes based on PG feedback to
demonstrate your commitment to meeting their needs and improving service delivery
continuously.

● 6.3 Rapport
● Visiting Practitioners (Mona and Hetal Vyas): Strengthen connections through routine
updates and scheduled MBRs/QBRs, using these sessions to communicate DA’s value
effectively.
● Wound Centrics SAN ANTONIO: Our POC, Ben Simpson, is over there, along with
Adam and Erica (from the billing department).
However, due to their unavailability, it was established that Ben would help us get new
POCs from the billing department to ensure that our weekly cadences for Thursdays go
well.
● UT Health-Geriatrics & Supportive Care Clinic SAN ANTONIO: We have our POC
Prinny, with whom we are in constant touch via email. Whenever they have any
questions or concerns, they are addressed on time and on a priority basis as it is a big
corporate PG. However, since it is a non-turnkey account at the moment, our plan is to
convert it into a turnkey account and bring it down the PG funnel.
