Subject: Letter of Appointment

Dear Syed Muballagh Hussain Raza,

You are hereby appointed as an employee of Prime Human Resource Services (Private) Limited (PrimeHR) with
effect from Monday, 10 March, 2025, in connection with a Contract/Agreement of Prime Human Resource Services
(Private) Limited and its Client. Your working title shall be District Medical Officer in Health & Accidental
Insurance Department and you will be posted at Faisalabad. The following terms and conditions shall govern your
employment.

COMMENCEMENT & DURATION OF Employment:


1. Your Employment shall commence from the date you assume your assignment, which in any case should not
be later than Monday, 10 March, 2025.

2. The Employment assigned to you, essentially exists only for the duration of our agreement with our client,
unless otherwise extended / discontinued by the Client. On the expiry of Prime Human Resource Services
(Private) Limited (PrimeHR) contract with its Client, your Employment shall be terminated in accordance with
the Termination Clause of this appointment letter. No separate letter of termination is necessary on or after
expiry of PrimeHR agreement with its client.

COMPENSATION:
3. That your monthly salary, inclusive of all cost of living allowances and other statutory allowances, enhancement,
benefits, perquisites and mandatory increments shall be the sum of Rs.126,986/-

You shall also be entitled for perks; breakup is as under:

4. That the emolument shall be paid after any deductions required under any tax or applicable legislation(s). You
shall be personally responsible and liable for all personal income or other taxes associated with your
employment.

LEAVE.
5. That you shall be allowed leave in accordance with the Company’s rules in force from time to time.

GRATUITY:
6. You shall be entitled for gratuity as terminal benefit as per company’s policy payable after cessation of your
services; provided that the same is not due to termination or that there is no legal proceeding / inquiry in process
against you; as per rules of Primehr.

GROUP LIFE INSURANCE:


7. During the tenure of the employment you will be insured under Group Life Insurance covering death & total
disablement as per company policy.

LIABILITY OF CONTRACT:
8. You will be initially be on probation for 3 months from the date of joining and after completion of probation period
you shall be considered as a permanent employee, no separate letter of confirmation will be provided to you.

9. You shall be required to furnish two references of persons not related to you. Your confirmation shall be subject
to our receiving satisfactory antecedent verification.

10. You shall be required to perform duties as assigned by our Client, with whom we have an Agreement.

11. Your employment responsibilities or duties may be changed from time to time by the Company, as it may deem
necessary.

12. During the period of Employment your services are liable to be transferred anywhere in Pakistan as and when
required.

13. Any change in your residential address, phone number or in your civil status must be notified to us immediately.

TERMINATION OF CONTRACT:
14. During the period of initial three months, your employment may be terminated without any notice or any
payment in lieu thereof. After successful completion of the initial three months period, your appointment may be
terminated at any time subject to either of the following conditions.
a. If in the Company’s opinion, which shall be final and binding in this matter, you are found guilty (after
due diligence/investigation) of dishonesty, misconduct, disorderly behavior, negligence, indiscipline,
insubordination, absence from duty without permission, or having acted in a manner adversely
prejudicial to the interests of the Company; or of any conduct which violates one or more terms of
this letter, your services shall be liable to be terminated forthwith without any notice whatsoever or
any compensation in lieu of notice.
b. After initial three months period either you or the Company shall have the right to terminate the
employment under this clause by giving to the other not less than one month’s notice in writing at
any time, or pay a lump sum equivalent to one month’s salary as payment in lieu thereof.

OTHER RULES AND REGULATIONS:


15. That in the course of your employment you shall act in accordance with the literature and directions provided to
you by the Company / its Client from time to time in relation to any assignment for which you may be engaged.

16. That you shall execute all such documents as may be reasonably required by the Company in order to protect
the interest and confidential information of the Company and its Client(s).

17. That you shall not seek employment with any Client of the Company nor at any time represent yourself as an
employee or agent of any Client of the Company.

18. That you shall not act in any manner contrary to the literature and instructions provided to you. You shall / may:

a. Work on the basis of a six (6) day working week and shall observe the normal working hours fixed
by the Client from time to time;
b. Be required to work beyond the normal working hours and during weekends and public holidays;
c. Attain requisite qualification and training to perform the services required of you;
d. During the tenure of your employment, devote the time, attention and skill required for the proper
performance of service required by the Company;
e. Comply with all reasonable requests of the management of the Company and the authorized
persons/officer of the Client of the Company;
f. Not hold yourself out as having any authority to bind any Client of the Company or to incur any
liability on behalf of such Client, and shall at all times act in accordance with the literature provided
by such Client in relation to all your dealings with any third parties;
g. Comply with all applicable laws, rules and regulations and the customs of Pakistan;
h. Perform all services, and always act in course of your employment, with all reasonable skill and
care;
i. Prepare and submit such reports in such format and at such frequency as any Client of the
Company may reasonably require, giving details of all aspects of the duties carried out by you;
j. Promptly notify the Company and the Client of the Company of any matter coming to your
knowledge which could have a material effect on the business or affairs of such Client of the
Company;
k. Comply with any regulations provided to you by the Company from time to time;
l. Not divulge, either directly or indirectly, to any person or body any knowledge or information which
you may acquire concerning the affairs, property, enterprise and under taking of our Clients and its
associate companies or subsidiaries and shall maintain the usual secrecy about the business and
other affairs of our Client.
m. Be terminated or dismissed with immediate effect if you have made any alteration, misrepresentation
or omission on any document requested by the company

19. Furthermore, you shall not take or remove from the premises of the Company and its Client where your services
are utilized without the consent of the management/Client any data, table calculations, letters and other
documents or items of property or confidential information pertaining to the company’s/Client’s business or
affairs, in any form (paper, diskette, tape, CD, optical or magnetic etc).

20. You shall be responsible for the safe custody and return in good condition and order, of all Company’s property,
which may be in your use, custody, care or charge. The Company shall have the right to deduct the money
value of all such thing from your dues and take such other actions, as it deems proper in the event of your
failure to account for such property to the company’s satisfaction.
