Lockout PURPOSE: This procedure establishes the minimum requirements for the lockout of energy isolating devices whenever

maintenance or servicing is done on machines or equipment. It shall be used to ensure that the machine or equipment is stopped,
isolated from all potentially hazardous energy sources and locked out before employees perform any servicing or maintenance
where the unexpected energization or start-up of the machine or equipment, or release of stored energy could cause injury.
COMPLIANCE WITH THIS PROGRAM: The authorized employees are required to perform the lockout in accordance with
this procedure. Failure to follow LOTO Standard Work and Procedures will result in disciplinary action up to and including
termination. All employees, upon observing a machine or piece of equipment which is locked out to perform servicing or
maintenance shall not attempt to start, energize, or use that machine or equipment.

Lockout Application Process


1) NOTIFY: Affected employees what equipment is to be locked out. 2) SHUTDOWN: Equipment with normal shutdown procedure or by the applicable Lockout
Statement listed below. 3) LOCK: Disconnect and Lockout ALL energy sources and Release any stored energy if possible. A) NOTE: All personnel involved
MUST have individual lock on equipment. B) NOTE: Locks will be swapped out during shift change. 4) TAG: Ensure tag is clearly marked with required
information. 5) TRY: Verify all energy sources are isolated by TRYING equipment. 6) RETURN: Controls to the off or neutral position.

Lockout Removal Process


Final Lock-Out Removal Procedure
1. INSPECT - work area: remove all nonessential items and replace all guards.
2. VERIFY - employee safety: verify that all affected employees in the vicinity of the machine are in a safe area/position.
3. NOTIFY - affected employees that Lock-Out is going to be removed.
4. REMOVAL - each Lock/Tag must be removed by the individual who applied it.
5. NOTIFICATION - of employees: notify all authorized and affected employees that the lock-out/tag-out devices have been removed.
