Dear Parents,
Greetings!
We are pleased to inform you that swimming selections for School Team will be conducted from
Monday, April 21st, 2025[ Boys] and Wednesday, April 23rd, 2025[Girls] at our School Swimming
Pool during school hours.
Kindly send this Consent Form, clearly mentioning the fitness level of your child, and hand it
over to your child’s PE Teacher, Ms. Soosy Jona or Mr. Vishnu, on or before Sunday, April 20th,
2025
. (just before the selection.)
The consent form submission confirms that your ward had no health concerns or fitness issues of
any nature to undertake swimming.
Every enrolled student shall strictly follow the rules & regulations listed below:
1. The swimming kit that your child should have in possession for the swimming selection shall
consist of;
i. A neat, clean and dry towel
ii. A dark-coloured swimming costume – as shown in the picture
iii. A rubber swimming cap
iv. A pair of swimming goggles
v. A bottle of water
2. Proper decorum is to be mentioned in and outside the pool and no activity or behavior which
causes disturbance to fellow swimmer will be allowed. This may lead to severe behavior
penalties.
3. On the scheduled day of selection -swimming, the students are required to take a shower before
and after swimming and change into school uniforms in the washroom by the poolside itself for
hygienic reasons.
4. Besides maintaining discipline in and around the pool, all safety instructions are to be strictly
followed.
5. Jewellery, sharp materials, and objects made of glass should not be brought to the pool.
6. Without the permission of the swimming coach / lifeguard, no student should enter or leave the
pool.
7. For girls, lady swimming coach will be ensured and for boys, gents instructors will be assigned.

I hereby declare that:
My child is fit for swimming and he / she has no health issues.
a. He / she is not aqua phobic / asthmatic / epileptic;
b. has no food / chlorine allergy;
c. has not suffered any post-surgery effects;
d. has not undergone any major medical treatments; and
e. has no skin / ear / lung infection or has no fungal infection in any part of the
body.
I have read and explained the content of the procedures and rules of the swimming
activity to my ward and assure that he/she will abide by them.
He/she will be using the swimming pool in DMIS under the supervision and guidance
of a trained swimming coach and, as a parent, I hereby indemnify the school authority
and management for any injury or fatal accident to my ward.
All the particulars mentioned in this declaration form are correct to the best of my
knowledge.
