Records show that you have not been reporting for work/absent without approved leave from

Please be informed that we have sent you the following AWOL Notices / Letter on: (Date of 1st
AWOL Notice)_________________and (Date of 2nd AWOL Notice)___________________ .
The said letter was sent to you and yet you have not responded. This is also an insubordination
or non-compliance on a valid order of your superior.

Accordingly, you are reminded that as per HCDC Code of Discipline:

Article VI, Sec. 4: “Unauthorized and/or unexcused absence (AWOL) from work
for five (5) consecutive working days or more.”

In view of this, the company has no other recourse but to terminate your services effective
(Date)____________________.

Please return all company documents, tools, materials and equipment issued to you during your
employment to your supervisor or to the HR Department.

Ayon sa record ng kumpanya, ikaw ay hindi pumapasok o lumiban sa iyong trabaho na-walang-
pahintulot- o leave simula (Petsa)_____________ hanggang ngayon na may kabubuang (X
bilang na) _____ araw.

Ipinababatid naming sa iyo na ikaw ay pinadalhan ng dalawang (2) AWOL Notices o sulat ng
hindi-pagpasok noong: (date of 1st AWOL Notice)______________________ at (2nd AWOL
Notice)___________________. Ipinadala sa iyo ang mga nasabing sulat sa pamamagitan ng
registered mail ngunit hindi ka pa rin nagreport. Ang hindi mo pagpasok ay nangangahulugan ng
“insubordination” o hindi pagsunod sa valid na utos ng iyong superior.

Kaugnay nito, ipinapaala naming ang HCDC Code of Discipline:

Class D, Sec. 1: “Unauthorized and/or unexcused absence (AWOL) from work for
five (5) consecutive working days or more.” Penalty: Termination/Dismissal. (Hindi
awtorisado at/o walang sapat na dahilan sa pagliban sa trabaho mula lima (5) o higit
pang araw. Parusa: Termination/Dismissal)

Dahil dito, wala nang magagawa ang kumpanya kundi ang tapusin o i-terminate ang iyong pagtatrabaho
epektibo sa (Date) __________________.

Mangyaring isauli mo ang lahat ng dokumento, gamit, materyales at equipment na inisyu sa iyo habang
ikaw ay nagtatrabaho. Pakisauli ito sa iyong superbisor o sa HR Department.

Para sa iyong kaalaman at mahigpit na pagsunod.
