Login to iZone
Use your Student ID and Password to login to your iZone account with the following URL:
https://izone.sunway.edu.my/login
The latest version of iZone is incorporated with responsive web design (RWD) to improve browsing
experience from mobile devices.
From the iZone Home page main menu, go to Services > eForms.
In the eForms page, click on Student Status Modification Form to launch the module.
To submit a new SSMF case, click on the Submit NEW Application button on the top.
Each submitted case will go through a series of approval and administrative processing.
To view the processing status of submitted cases, click on the Ref. No. of each case under the YOUR
SUBMITTED APPICATIONS section.
If submitted cases are not available, click on the BLUE refresh button to reload the module.
A SEARCH feature is available to make browsing much easier when the number of cases increases.
The search box accepts Ref. No., Student ID, Programme and Form Type.
Cases completed processing will be relocated to the COMPLETED section for easy reference.
1) COMPLETION
2) WITHDRAWAL
3) TRANSFER
4) DEFERMENT
Select the relevant application and click on the CONFIRM button to proceed.
This is the Completion Application eform. To submit other applications, click on the CHANGE
button.
The Donate Payment Refund to Jeffrey Cheah Foundation is optional. If Donate All is selected,
refund particulars are not required.
For Crossed Cheque payment option, local students will only have the option to receive the cheque
via Post by mail.
Open Cheque payment option is only available for international students and they are only allowed
to collect the cheque personally or by the payee.
If By Telegraphic Transfer is selected as payment option, the following details are required:
1) Currency
2) Swift Code
3) IBAN No./BSB Code/IFSC Code
4) Account No
5) Bank (Please state name, branch, country)
If By Local Interbank GIRO (IBG) is selected as payment option, the following details are required:
1) Account No.
2) Bank (Please state name, branch, country)
Selected Refund Payee option other that self will require the Payee Name and NRIC/Passport No.
Upload required documents by dragging and dropping the files into the DOCUMENTS upload box.
Only Excel, Word, PDF, JPG, PNG and GIF file types accepted with each file size not exceeding 5MB.
Scroll down and read through the IMPORTANT NOTES and TERMS AND CONDITIONS carefully.
Tick the checkboxes for the MANDATORY DECLARATION and click the SUBMIT COMPLETION
APPLICATION button to proceed.
This is the Withdrawal Application eform. To submit other applications, click on the CHANGE
button.

If you are unable to return the student card, please upload an image of the card that has been
snipped at the bottom right corner by dragging and dropping the image into the DOCUMENTS
upload box. This will prevent the penalty fee charge.

Scroll down and read through the IMPORTANT NOTES and TERMS AND CONDITIONS carefully.
Tick the checkboxes for the MANDATORY DECLARATION and click the SUBMIT WITHDRAWAL
APPLICATION button to proceed.
This is the Transfer Application eform. To submit other applications, click on the CHANGE button.
1) Transfer Programme
2) Transfer Intake
3) Transfer Study Mode
4) Transfer Premises
(Transfer Premises only applies for college students transferring between SUNCOL and SUNVEL.)
Upload required documents by dragging and dropping the files into the DOCUMENTS upload box.
Only Excel, Word, PDF, JPG, PNG and GIF file types accepted with each file size not exceeding 5MB.
Scroll down and read through the IMPORTANT NOTES and TERMS AND CONDITIONS carefully.
Tick the checkboxes for the MANDATORY DECLARATION and click the SUBMIT TRANSFER
APPLICATION button to proceed.
Transfer application submitted by students below 18 years of age will require to declare their
Parent/Guardian Name and NRIC/Passport Number. Please read the terms and conditions carefully
before submitting the application.
This is the Deferment Application eform. To submit other applications, click on the CHANGE button.
Select the Period to Defer and the Reason for Deferment accordingly.
Upload required documents by dragging and dropping the files into the DOCUMENTS upload box.
Only Excel, Word, PDF, JPG, PNG and GIF file types accepted with each file size not exceeding 5MB.
Scroll down and read through the IMPORTANT NOTES and TERMS AND CONDITIONS carefully.
Tick the checkboxes for the MANDATORY DECLARATION and click the SUBMIT DEFERMENT
APPLICATION button to proceed.
Application Complete
After the SSMF Application processing has been completed, you will receive the following
notification email:
Please note that the official letter will NOT be issued to students for the followings:
1) COMPLETION
2) Transfer Premises
3) WITHDRAWAL - Not Enrol
Launch the SSMF module and search for the relevant case Ref. No. under the COMPLETED section.
You will be able to view the full processing history of your application.
To print the application, click the blue printer button on the top right corner of the page.
