We, CWTS Group 4: Team SS, are writing to kindly request your approval for a sponsor
initiative and assistance that aims to support our project entitled, Safe Choices, Bright Futures:
Strengthening Youth Through Comprehensive Sex Education as our chosen project is in your
field. We are needing professional help and support as we tend to test HIV, STD’s, and AIDS.
This contribution will play a crucial role in making a meaningful impact by providing
much-needed resources and support. We believe that with your endorsement, we can move
forward confidently and begin making a difference where it matters most. Thank you for

Project Title: Safe Choices, Bright Futures: Strengthening Youth Through Comprehensive Sex
Education
Project Description:
This project proposal addresses the critical need for comprehensive sex education (CSE)
in the Philippines, to youths especially adolescents in Brgy. San Agustin, San Fernando,
Pampanga. The Philippines faces alarming rates of teenage pregnancy, sexually transmitted
infections (STIs), and gender-based violence, contributing significantly to health, social, and
economic burdens. This community-based initiative, "Safe Choices, Bright Futures," aims to
mitigate these challenges by providing age-appropriate, culturally sensitive CSE to adolescents
aged 13-18.
The primary goal is to reduce the incidence of teenage pregnancy, STI transmission, and
gender-based violence while fostering self-esteem, critical thinking, and respectful relationships.
Specific objectives include delivering CSE to at least several adolescents within 12 months,
demonstrably increasing their knowledge of human development, consent, healthy relationships,
contraception, STI prevention, and gender equality. The program will also enhance adolescents'
decision-making, communication, and negotiation skills related to sexual health, creating a
supportive and safe learning environment that reduces stigma. A key component will be
establishing sustainable collaborations among educators, health professionals, parents, and youth
organizations.
To complement the educational components, the initiative will also feature a series of
community-based health activities such as free and confidential STI, STD, HIV and AIDS
testing and counseling, in partnership with the Department of Health (DOH), Bahay Lingad, and
local health units. These testing services will be offered during designated outreach events and
will be paired with awareness campaigns, and peer-led discussions to reduce stigma, encourage
early detection, and promote overall sexual and reproductive well-being. This holistic approach
ensures that adolescents not only receive accurate information but also have access to essential
health services and support systems within their community.
Specific Objectives:
This sex education awareness program and testing aims to:
● equip youths with accurate, age-appropriate information about human development,
reproductive health, consent, contraception, sexually transmitted infections (STIs), and
healthy relationships.
● empower young people to make informed, respectful, and responsible choices regarding
their sexual and reproductive health.
● decrease the incidence of unintended pregnancies, STIs, and unsafe sexual practices
among youth through evidence-based education.
● educate youths on the importance of family planning, including understanding various
contraceptive methods and their correct usage.
● reduce the stigma and misinformation surrounding sex, contraception, and reproductive
health topics through open, judgment-free discussions.
● empower young people to say no to peer pressure and risky sexual behavior by boosting
self-esteem and decision-making skills.
Adopted Community:
Barangay San Agustin, San Fernando, Pampanga
Target Participants:
Adolescents aged 13–18 years old residing in Barangay San Agustin.
Persons Involved:
● Educators – responsible for delivering the Comprehensive Sex Education (CSE)
curriculum.
● Health Professionals – involved in STI, STD, HIV/AIDS testing, counseling, and health
education.
● Parents/Guardians – engaged in awareness and support activities to promote open
dialogue.
● Youth Organizations – help in organizing facilitated discussions and community outreach.
● Department of Health (DOH) – provides medical support and testing services.
● Bahay Lingad (Jose B. Lingad Memorial General Hospital) – partner institution for
health services.
● Local Health Units (LHUs) – assist with outreach, testing, and counseling services.
● Community Volunteers or Peer Educators – help facilitate sessions and reduce stigma
through youth engagement.

Materials and Logistics Needed:
For the successful implementation of “Safe Choices, Bright Future: A Comprehensive Sex
Education Initiative for Adolescent Health in Pampanga,” the following resources and logistical
support will be necessary:
To be prepared by the organizing team:
● Medical supplies, including basic first aid items, testing kits and bandages, for use during
interactive sessions
● Educational handouts and printed materials related to adolescent health and sex education
● Tools for documentation, such as equipment and services for capturing photos and editing
videos
● Printed certificates to be awarded to participants and speakers
● Projector for use during presentations and visual demonstrations
To be facilitated by partner institutions:
● Chairs for participants (to be provided by the host venue)
● Audio equipment, including speaker and microphone (to be provided by the Barangay)
● Light refreshments and snacks for attendees (to be arranged by the Sangguniang
Kabataan Council)

Source of Funding:
Major operational expenses are subsidized by the DHVSU NSTP-CWTS Group 4 SS Team.
Other miscellaneous expenses will be subsidized by partner organizations and donors.
Time Frame:
April 25, 2025: Finalize project proposal and objectives.
April 28, 2025: Submission of proposal letter to the school or organization head.
April 30, 2025: Submission of invitation letter to Bahay Lingad (A DOH-designated HIV and
STI treatment hub that offers treatment, care, and support to people living with HIV and STI.)
May 2, 2025: Submission of request letter and proposal letter for expert speaker from DOH
(Bahay Lingad).
May 5, 2025: Coordinate with Barangay and Sk officials for support and implementing donation
drives.
May 9, 2025: Schedule meeting with collaborators for the project (DOH, Bahay Lingad,
proponents, organization head.
May 15, 2025: Confirmation of venue, materials, and testing setup that will be approved by
DOH.
May 18, 2025: Approval of the place, time, date, materials, and set up by the collaborators.
May 19-23, 2025: Release promotion materials/ inform participants.
May 24, 2025: Final coordination meeting with all involved parties.
May 28, 2025: Project Implementation.
June 4, 2025: Follow-up checking for results
