COMPUTER DATA No. 65-2025

COMES NOW, the undersigned applicant from the office of the


Regional Anti Cybercrime Unit 1 RACU 1), Camp Diego Silang, Carlatan,
San Fernando City, La Union most respectfully return unto this Honorable
Court the above described WDCD issued on March 11, 2025 (attached
hereto is the Warrant to Disclosed Computer Data) with the following
information:

On March 11, 2025 at about 04:58 PM, the undersigned personally


implemented the Warrant to Disclosed Computer Data under docket
WDCD No. 65-2025 pursuant to Section 14 of RA 10175 at Bank of the
Philippines Island Quezon Avenue, San Fernando City, La Union,
Philippines duly acknowledge received by Allen Grace Medina for the
Disclosure of Computer Data particularly BPI Account Number:
4016038995.

On March 12, 2025, the undersigned submitted Initial


Compliance/Return.
Further on March 17, 2025 received thru Gmail the result of Warrant
to Disclosed Computer Data (WDCD) no. 65-2025 from BPI signed by Noly
B Bolaňos, Digital Operation Unit Head.

PRAYER

WHEREFORE, it is respectfully prayed to that Honorable Court that


the Final Compliance/Return with attached result from BPI of the WDCD
No. 65-2025 be accepted.

Other reliefs and remedies, just and equitable under the premises are
likewise prayed for.
