MANAGEMENT VALUE & COMMITEMENT INTRODUCTION

This HSE Plan manual demonstrates and documents Hydroserv’s commitment to maintain high-
level of Occupational Health & Safety and maintain the effective working environment, focusing
on customer requirements, safe working practices and fostering on continual improvement.
Conformance and Compliance Standards
The HSE Plan manual is intended to demonstrate conformance to the ISO 45001:2018
International Standard: Occupational Health & Safety Management Systems – Requirements.
Reference to these standards also implies reference to all guidance standards contained therein.

1.1 HSE Policy


• The HYDROSERV HSE Policy is displayed on Workplace Health and Safety has been
issued by the Managing Director.

• It is the responsibility of all Workers to understand and implement the Health Safety and
Environment Policy at the workplace.

• It is essential that a copy of this HSE Policy be displayed on appropriate notice boards,
PSC Cabin and offices.

• All employees shall be given the opportunity to understand the Hydroserv HSE Policy.

1.2 Management Values


• The following safety management values will be adopted by Hydrosev:
• No objective is so important that it will be pursued at the sacrifice of safety.
• Each of us has a personal responsibility for our safety and the safety of others both on and
off the job.
• Safe conduct is an obligation of employment.
• A job is well done only if it is done safely.
• We will have an injury free workplace.
• All accidents are preventable.

1.3 Commitment
The following Safety Performance Targets have been established for Hydroserv to apply for all job
sites and all sub-contractors:

• Proactive Hazard Management


• Zero Lost Time Injuries
• Meet all commitments and agreed measures contained within the Safety Leadership
Matrix
• Completion of a formal, documented workplace inspection of entire sites every two
months for the duration of work activities.

2 MANAGEMENT RESPONSIBILITY AND AUTHORITY


Hydroserv employee and sub-contractor have a personal duty of care with respect to safe working
environment.
The responsibilities of Hydroserv personnel and other workers on Hydroserv job sites with respect
to a Safe Working Environment at the work place are as follows.

2. 1 All Individuals
In addition to the specific responsibilities outlined below, each individual working on a Hydroserv
Job site is expected to fulfill the following responsibilities:

• Personally, adopt the Hydroserv Safety Values.


• Perform their duties in a manner, which does not create any situation where they can
cause damage to themselves, to others or to equipment.
• Co-operate with others to ensure that they can fulfil their own safety responsibilities.
• Abide by all the Hydroserv Safety Rules outlined in this Health and Safety Management
Plan.
• Intervene immediately to correct any sub-standard acts and conditions that they observe.

2.2 Engineering Director


The Engineering Director is responsible for achieving the following in relation to safety.
• To be familiar with all legal responsibilities placed upon that position by the Qatar Health
and Safety Act 1999, the Workplace Health and Safety Act 1995, and other acts
applicable.
• To advise the Managing Director in formulating the Hydroserv Health and Safety policy.
• To overview of the management structure so that Hydroserv has a hierarchy of capable
people who are adequately instructed on their responsibilities and authorities to allow
them to carry out their work on the basis of improving safety.
• To ensure that all Hydroserv activities comply with the requirements of the Hydroserv
Business Procedures Manual, the specific site Health and Safety Management Plans and
the relevant statutory and contractual requirements.
• To conduct Safety Meetings and Audits with relevant senior management at least once a
year of Corporate office and site locations.
• Attend all Pre-Start Safe Production meetings when at project sites at the commencement
of a shift.
• Attend at least once a month a pre-start meeting at Hydroserv locations other than project
sites.
• Review one procedure or SOP on each site when inspecting or visiting by observing
compliance by workers and record the findings.
• Give feedback to the relevant Line Manager immediately on completion of an inspection.
• Ensure a system is in place to be able to monitor all Hazard Reports, Incident Reports
and Correction Action Requests.
• Ensure that Safety is the first Agenda Item on all Line Management meetings.
• Present a report of findings and actions from Inspections and Audits that are performed
at Hydroserv job sites and other Hydroserv workplaces, relevant to their performance
against the area specific Health and Safety Management Plan, to the Board of Directors
and Owners or Clients if required.
• Ensure a system is in place to record and monitor the completion of all Hazard controls
and Incident recommendations within the agreed times and that a subsequent review of
the controls is initiated to ensure that the risks have been minimised.

• Ensure Inspection and Audit systems are in place.


• Ensure a system is in place to monitor work behaviour, improvements and any reward
recognition program aligned with positive outcomes.
• Ensure a system is in place for all workers to create feedback, improve and add value to
the Hydroserv Safety system.
• Ensure a system is in place to give assistance to workers who may be adversely affected
by outside influences which may directly or indirectly affect their performance in the
workplace creating damage and losses.
• Meet all agreed commitments in the Hydroserv Safety Leadership Matrix

2. 3 Operations Manager
The Operations Manager is responsible for achieving the following aspects in relation to
safety.
• Reporting to the Engineering Director on all activities which have a positive or negative
effect on the company involving Safety
• To be familiar with all legal responsibilities placed upon that position by the Qatar Health
and Safety Act 1999 and the Workplace Health and Safety Act 1995.
• Coordination of the activities of the Construction Managers with respect to the Health
and Safety of the workplace.
• Ensuring that all contract site activities comply with the requirements of this Health and
Safety Management Plan and the relevant statutory and contractual requirements.
• Supporting Construction Managers in the fulfillment of their responsibilities and
ensuring that they have the necessary skills and resources.
• Instilling and demonstrating the required level of commitment to safety management at
all Hydroserv workplaces.
• Participating in safety audits and inspections at least once at each site.
• Review and auditing of the function and effectiveness of the site Pre-start, Safety
Committee, Safety forums and Tool Box Talk meetings.
• Attend all Pre-Start Safe Production meetings when at project sites at the commencement
of a shift.
• Attend at least once a month a pre-start meeting at a Hydroserv location other than Job
sites.
• Review one procedure or SOP on each site when inspecting or visiting by conducting a
Job Observation and observing the degree of compliance by workers and record the
findings.
• Give feedback to the Line Manager immediately on the completion of the Job
Observation.
• Ensure that line management reporting to your role follows the system in place to monitor
all Hazard Reports Incident Reports and Correction Action Requests.
• Ensure that Safety is the first Agenda Item at all Line Management meetings.

• Present a report of findings and actions from Inspections and Audits that are performed
at Hydroserv Job sites and other Hydroserv workplaces, relevant to their performance
against the area specific Health and Safety Management Plan, to the Managing Director.
• Ensure that a system is in place to record and monitor the completion of all Hazard
controls and recommendations from Incidents within the agreed times and a subsequent
review of the controls is initiated by Engineering Director to minimise risk
• Ensure that Construction Managers put in place Inspection and Audit systems and they
report all findings of the Inspection and Audits within 24 hours.
• Meet all agreed commitments in the Hydroserv Safety Leadership Matrix

3.2 Applicable Legislation


In addition to the standards described in this document, this site Health & Safety Management Plan

3.3 Definitions
3.3.1 Audit
 A critical examination of all or part of a total Health and Safety Management System
which is directed by the Business Procedures Manual.

3.3.2 Accredited Auditors


 Personnel accredited to audit the implementation of the Safety Management Systems at
Golding workplaces against the Golding Safety protocols.

3.3.3 Control Measures


 Actions to reduce the chance of a hazard occurring or to reduce the effect of the hazard
if it does occur.

3.3.4 Dangerous Goods


 As defined under Federal/State legislation

3.3.5 Employee
 An individual who works under a contract of employment, apprenticeship or
traineeship. The term ‘employee’ includes: direct employees of all levels, including
management.

3.3.6 Employee Representative


 A worker who has been elected by other workers on the site to represent them on health
and safety matters.

3.3.7 Hazard
 Any actual or potential occurrence or condition that may result in harm, (including
death and injury to people, damage to property or interruption to an activity or process).

3.3.8 Hazardous Goods


 As defined under Federal / State legislation.

3.3.9 Hazard Identification


 Identification of all hazards at the site.

3.3.10 Hazard Register


 A document or process detailing hazards associated with all aspects of the work. The
register may be ranked according to the potential severity and/or probability of the
hazards.

3.3.11 Incident
 Any unplanned event where control is lost and/or there is or could be harm to workers,
plant or equipment and an interruption to an activity or process.

3.3.12 Independent Audit


 An audit where at least one member of the audit team is a person who does not
normally work on the site and is not normally associated with its functions.

3.3.13 Line Management


 An Individual with direct responsibilities for the project for Golding and who reports to
another individual on performance outcomes

3.3.14 Lost Time Injuries (LTI)


 Are the sum of lost day injuries and restricted work duty injuries;
 A lost day injury (LDI) is an occupational injury that results in one or more days away
from work. A fatal injury is counted as a lost day injury;
 A restricted work duty injury (RWDI) is an occupational injury where, as a result:
the employee was assigned to another job on a temporary basis
the employee worked at a permanent job less than full time, or
the employee worked at his or her permanently assigned job but could not
perform all the duties normally connected with it.
 Restricted work activity occurs when the employee, because of the job-related injury, is
physically or mentally unable to perform all or any part of his or her normal assignment
during all or any part of the normal workday or shift.

3.3.15 Medically Treated Injury (MTI)


 An injury that requires treatment by a medical practitioner and/or is beyond the scope
of normal first aid.

3.3.16 Minor Injury (MI)


 An injury that requires first aid treatment. First aid treatment involves minor scratches,
cuts, burns, splinters, etc. and the worker would typically return immediately to normal
duties.
 Such treatment is considered first aid even if a medical practitioner administers it.

3.3.17 Monitoring
 A Line Management function of checking for acceptable performance as tasks are
completed.

3.3.18 Performance Review


 Evaluation of how well Golding achieves its goals as specified in this Health and Safety
Management Plan.

3.3.19 Risk
 The risk associated with an undesired event is described as a combination of the
likelihood of the event occurring and the resultant consequences should it occur.

3.3.20 Performance Standards


 Standards established by Golding that indicate who is responsible for carrying out an
activity, what must be done, when an activity must be performed and what outcomes
are to be expected.

3.3.21 Preventative Action


 Action taken to identify and eliminate the causes of a potential non-conformity, defect
or other undesirable situation to prevent occurrence.

3.3.22 Procedure
 A procedure is a formalized or structured statement as to how an activity, or a group of
related activities, is or are performed giving reasons, methods, responsibilities and
outcomes.

3.3.23 Risk Management


 The ongoing management process of identifying risks, evaluating the consequences and
probabilities of these risks, and then reducing the risk levels to help ensure the target of
Zero accidents.

events and the means to prevent or minimize their occurrence or consequences to help
ensure the target of zero accidents.

4 HYDROSERV HEALTH & SAFETY MANAGEMENT SYSTEM


4.1 General
A comprehensive integrated system of managing Health and Safety at the which sets out:
The Health & Safety objectives.
The specific responsibilities for achieving these objectives and meeting these standards.
The means for determining the effectiveness of the system (auditing and measurement).
This Safety and Health Management Plan contains the details on how the HHSMS will be
implemented on this site.

4.2 Purpose and Scope


To establish guidelines for the implementation and administration of a Health & Safety
Management Plan and to assign specific responsibilities for the execution of the program.

To provide a Project Health & Safety Management System that ensures Hydroserv and others can
recognize, evaluate and control hazardous activities and/or conditions within each of their
respective areas of responsibility.

This system applies to everyone associated with this contract work including Hydroserv
employees, sub-contractors, vendors, Client Representatives, Government Representatives and any
other visitors to the sites.

Requests to distribute this Plan to another organization, regulatory or governmental agency shall
be submitted to Hydroserv in writing for consideration and approval.

Hydroserv reserve the right to modify this Plan at any time during any phase of work of the site as
new regulations or standards are disseminated, distributed and circulated.

Controlled revisions of this document will be provided in electronic systems as well as in hard
copy to the relevant parties in accordance with the document distribution schedule.

5 HYDROSERV HEALTH & SAFETY MANAGEMENT PLAN


5.1 General
This Hydroserv Health & Safety Management Plan is set out in sections. These sections detail the
specific Safety rules and regulations for the site.

Where work activity detail is called for, the site standards are laid down in the project Standard
Operating Procedures (SOP). The SOP’s are living documents that may require additions and
alterations from time to time due to changing conditions.

Everyone working on the site will be required to follow these procedures.

A listing of the procedures is filed in the Safety Officer’s Office on site.

5.2 Administration
Hydroserv
Hydroserv is responsible for an effective Health & Safety Management System.
This Health and Safety Management Plan stipulates the minimum standards for all workers on
site.
Hydroserv Sub-Contractors
If a sub-contractor is seeking to have their own system approved, they shall prepare and submit
its project Health and Safety Management Plan to Hydroserv for review and approval., The
Health and Safety Management Plan shall fully describe their commitment for meeting
Safety and Health obligations.
On approval by Hydroserv, the sub-contractor shall implement their Health and Safety
Management Plan in conjunction with this Hydroserv Health & Safety Management Plan.
As a minimum, all Hydroserv sub-contractors will be contractually obligated to meet the
requirements of the Hydroserv Health and Safety Management Plan.
Elements.
Hydroserv is responsible for ensuring that Hydroserv sub-contractors are aware and fully comply
with the requirements set forth herein and all subsequent requirements of Hydroserv.

6 DUTIES AND RESPONSIBILITIES


6.1 Hydroserv Construction Manager
The Hydroserv Construction Manager shall be responsible for:
• Implementing and evaluating the provisions of this plan and promoting teamwork with all
personnel in its implementation.
• The Health & Safety Policy being foremost in the minds of all workers on site and that the
requirements are communicated to everybody engaged to work on the site.
• Ensuring that all members of the team, including Hydroserv sub–contractors, Client’s
Representatives etc. working on sites, are informed of hazards associated with work
assignments and that there is compliance with all Health & Safety requirements.
• Ensuring that a Safety interface is established between separate Hydroserv sub-contractors
that will enable them to individually function in a safe, productive and harmonious manner.
• Ensuring that Hydroserv minimises any adverse impacts on other Hydroserv sub contractor’s
activities.
• Coordinating and standardising the Health and Safety Management Plan activities of Line
Management.
• Ensuring that all levels of management of Hydroserv sub-contractors are aware of their
responsibilities in enforcing the Health and Safety Management Plan.
• Attending all Pre-Start Safe Operation meetings held on site. If more than one workgroup is
to be attended, then they shall be attended alternative days.
• Review one procedure or SOP on site each week by conducting a Job Observation and
observing the degree of compliance by workers and record the findings.
• Ensure a system is in place to monitor all Hazard Reports, Incident Reports and Correction
Action Requests and it is followed by Line Management reporting to your role.
• Ensure the system is in place to record and monitor completion of all Hazard Controls and
Incident Recommendations within the agreed times and that a subsequent review of the
controls is initiated by Supervisors and Foreman’s to minimise risk.
• Ensure that Supervisors and Foreman’s put in place Inspection and Audit systems and they
report all findings of the inspection and audits within 24 hours.
• Give feedback to Supervisors and Foreman’s immediately on completion of inspection.
• Ensure that Safety is the first Agenda Item on all Line Management meetings.
• Review and auditing of the function and effectiveness of the site Pre-Start Safe Operations,
Safety Committee, Safety Forums and Tool Box Meetings.
• Present a report of the results of Inspections and Audits of this project’s performance to this
Health and Safety Management Plan to the site Supervisors.
• Meet all agreed commitments in the Hydroserv Safety Leadership Matrix

6.2 Safety Manager


The Safety Manager shall be responsible for:
• The ongoing development and implementation of the Health and Safety Management Plan
and the Program Elements.
• In conjunction with the Project Managers and Superintendents, review, advise and approve
the project Health & Safety Management Plan, ensuring that it complies with the Hydroserv
Health & Safety Management System.
• Developing and implementing a Health & Safety Compliance (Audit) Program for
monitoring and reporting the Hydroserv site and sub contractor’s Health & Safety
performance.
• Coordinating and conducting Health & Safety inspections with relevant Line Managers and
Hydroserv personnel.
• Conducting and coordinating Safety and Health orientation and training sessions.
• Providing daily monitoring of work activities and practices on the site.
• Assisting with the establishment of work practices that are to be implemented before work
can be commenced.
• Conducting, participating and reviewing the conduct of site management Pre-Start, Safety
Committee, Safety Forums and Tool Box meetings:
• Ensuring compliance with legislated Health and Safety regulations and keeping Line
Management from HYdroserv and our sub-contractors informed of major changes and
revised requirements.
• Posting Health & Safety notices, bulletins and emergency phone numbers in designated
areas.
• Coordinating and assisting Hydroserv personnel with Accident Investigations and document
findings, utilising the Hydroserv or Clients Incident/Hazard Reporting and Investigation
Formats and Standards.
• Attending and assisting Hydroserv Line Management with meetings as required.
• Providing information to Hydroserv regarding emergency response responsibilities.

6.3 Safety Officer


Hydroserv Safety Officer or on-site representative, is responsible for:
• Developing a Health & Safety Management program that upholds the standards of this Plan
as a minimum.
• Providing Health and Safety training for its employees. This training will include as a
minimum:
 Potentially hazardous conditions present in the work area.
 Identification of required Personal Protective Equipment (PPE) for the work
operation.
 Instructions to each employee on the proper procedure for reporting sub-standard
Acts and Conditions they may encounter.
 Instructions regarding the immediate reporting of work-related injuries or illness.
 Instructions regarding Hazard Communication and the location of Material Safety
Data Sheets.
• Ensuring that all levels of Hydroserv Supervision receive all necessary training in order to
properly implement this Health and Safety Management Plan.
• Implementing immediate action to correct reported or observed substandard Safety and
Health conditions.
• Monitoring employee compliance with fire prevention and protection requirements.
• Implementing all aspects of this Health and Safety Management Plan including its applicable
standards and monitoring and enforcing compliance.
• Conducting inspections of its work areas on a daily or more frequent basis and taking
necessary corrective actions to eliminate substandard practices and/or conditions.
• Conducting Toolbox Health & Safety meetings and keeping records for audit purposes.
• Notifying the Operations Manager immediately where possible or as soon as practicable
thereafter of any accidents or incidents on site.
• Investigating accidents and incidents and preparing the required reports.
• Attending Pre-Start Safe Operations meetings.
• Providing information to employees regarding their emergency response responsibilities and
assisting in its implementation
• Administering Workers Compensation requirements and responsibilities.
• Evaluating the effectiveness of the Health & Safety Plan and reporting findings to the
Hydroserv Operations Manager.
• Ensuring that Standard Operating Procedures are developed for all major work activities, and
the generation of method statements for all lifts or activities identified as hazardous.
• Ensure that a simple hazard analysis or JHA is conducted prior to the commencement of any
work activity and that a Pre-Start Safe Operations meeting is conducted with all those
involved, to best minimise risks.
• Ensuring, prior to allocating Employees to the work place, that the work area is not congested
and is safe to start work.

• Ensuring the correct PPE, tools and equipment are provided for the job. Check prior to
issuing tools/equipment that they are free of defects and have current certification if required.
• Promoting and rewarding Safe Behaviour.

6.4 Supervisors
Each Supervisor is responsible for ensuring:

• That they meet all agreed commitments in the Hydroserv Safety Leadership Matrix
• The active support of the Hydroserv target to achieve zero incidents.
• The promotion of Health & Safety first with every contract activity.
• They carry out at least once a week as a minimum a Safe Behaviour Observation
• Regular monitoring and inspections of the various contract areas
• The initiation corrective actions for substandard performance.
• The review and approval of risk assessments (Hazard Analysis, SOP) prepared by the
Hydroserv team.
• The implementation of immediate action to correct reported or observed substandard Safety
and Health conditions.
• They understand that as the roles are direct representatives of Hydroserv and consequently
share each of those accountabilities described above for Hydroserv Line Management.
• That all workers (employees and sub-contractors) are one team and adhere to one safety
standard

• They will be held responsible for each and every one of those accountabilities.

• Supervisors are the group that will have the greatest impact on Health & Safety on site and
consequently.
• Supervisors are required to lead by example and set the standard for Safety & Health in
every activity.

6.5 Workers
Workers shall be responsible for:
• Complying with the instructions given for workplace Safety and Health at the workplace.
• Working in a safe manner at all time.
• Learning and abiding by those Safety and Health rules and procedures which are applicable
to their work tasks and for reporting substandard practices or conditions to their Supervisor.
• Participating in the elimination of hazards.
• Promptly reporting incidents and injuries to their Supervisor and First Aid provider. All
personnel must know where the medical facilities and other emergency equipment or services
are located.
• Behaving in a pro-active manner to prevent and correct sub-standard acts and conditions.

• Knowing and accepting that any employee who jeopardizes their own Safety and Health
and/or the Safety and Health of others may be subject to disciplinary action.
• Presenting themselves in a fit state for work.

7 STANDARDS AND REQUIREMENTS


7.1 General
Each of the following standards specifies who undertakes an activity, the completion time and the
frequency rate of the task. Where a standard does not give any time frame or frequency for an
activity, it means that the responsible person must comply with SOP’s or guidelines before starting
any work that would create a hazard addressed by the standard.

Procedures, guidelines and SOP’s shall, in most cases, be produced before site work starts
and will be continually updated when the work method, client requirements or design of the
work changes.

7.2 Purpose and Scope


To clearly establish the minimum standards required of Hydroserv and its workers, Owner/Client
Representatives, and other organizations and individuals associated with the contract works and
visitors.

The standards will be clear in requirement and responsibility and compliance will be measurable.

7.3 Management
Hydroserv will control loss by the development of this Health & Safety Management Plan. It will
be consistent with the Health & Safety Management System.
The activities to be considered in this Plan will include, but are not limited to:

1. Personal Protective Equipment.


2. Employee training.
3. Planned inspections.
4. Risk Analysis procedures and method statements.
5. Emergency preparedness.
6. Fire prevention control plan:
7. Health control.
8. Accident and incident investigations.
9. Legislative requirements.
10. Compliance audit program.
11. Rehabilitation policy and procedure
12. Specific safety rules for their particular work activities.

7.4 Leadership
7.4.1 Standard
Every manager and supervisor working for Hydroserv will ensure that their own example of
safe behavior, sets the best possible standard.

7.4.2 Requirement
Hydroserv managers and supervisors and sub-contractor management and supervision will be
required to make incident prevention inspections as follows:

• Lead by example and “Walk the Talk”.

• Hydroserv Site Line Manager and sub- contractor or Owner / Client representative will make
a broad inspection of the contract site area every two months, as a minimum, and keep
records of the inspections.
• Each Hydroserv Supervisor will inspect their area every week, as a minimum, and keep
records of the inspections.

7.5 Plans & Targets


7.5.1 Standard
Hydroserv will aim and plan for zero accidents.
7.5.2 Requirement
Hydroserv line managers will develop their Health and Safety Positive Performance Outcome
Targets for the site work duration and these will be documented in the Hydroserv Safety
Leadership Matrix.

7.6 Personnel Placement


7.6.1 Standard
Hydroserv management and supervision will clearly define the skills and capabilities required
for each job done under their contract.
7.6.2 Requirement
Personnel skills and capabilities will meet any legal requirements of the position and nature of
work to be undertaken. The managers and supervisors will ensure that the appointed personnel
meet the required standards before personnel commence work under their contract.

7.7 Emergency Procedures


7.7.1 Standard
The Emergency Procedures will provide a simple and functional plan that can be followed by
each person on the site to protect personnel, equipment and materials in the event of an
emergency.
7.7.2 Requirement
The Emergency Procedures and Infrastructure prepared and supplied by Hydroserv will be
tailored to suit the requirements of all Projects site. Short, single page prompt sheets will list
the steps individuals will need to know to respond to an unplanned event.
• All Employees will be given a quick reference information card at the site induction for their
accountability should an emergency occur.
• In the event of an emergency all personnel will be familiar with and will respond in
accordance with the Site Procedure -Emergency.
• A mock drill on a situation that would trigger the Emergency Procedure will be carried out
at least once in every 6 months.
• All line management will complete one Critical Incident response during a year.

7.8 Pre-Employment Medical


7.8.1 Standard
Hydroserv must ensure a system is in place to identify that all workers are medically fit for
duty prior to their commencement of work on this contract.
7.8.2 Requirement
Hydroserv will ensure that all workers on the Hydroserv contract works have successfully
completed a Medical prior to commencing work on site.

7.9 Orientation (Safety and Health Induction) Workers


7.9.1 Standard
Hydroserv will establish a uniform method of providing Safety and Health awareness training
to all workers assigned to the site.

Hydroserv are responsible for ensuring that all workers are competent and adequately trained
for any work activity in which they are engaged.
7.9.2 Requirement
As a minimum, the following safety and health training shall be conducted as appropriate:

1. Each employee will hold a current Induction Safety Helmet Induction Sticker, prior to
commencement of work.

2. Each employee shall attend a site Safety Induction course prior to commencement of work.
This shall be a maximum of four hours duration during normal working hours depending
on client Safety Induction Program.

This Induction may include mandatory Blood Pressure, Blood sugar and physical
examination, . These tests will be carried out professionally and confidentially by trained
Staff. Persons failing to pass these tests will be escorted from Site and their Employer will
be notified.

Prospective employees will be told of this policy when applying for positions on the site.

3. Each employee shall attend the Hydroserv Orientation including the Hydroserv Specific
Safety and Health Induction.
This Orientation will clearly:
• Outline the scope of the work.
• Describe the commitment of Hydroserv to place safety first
• Motivate people to strive for world class safety performance and zero accidents
• Detail the site rosters and answer questions relating to “Fitness for Work”
• Detail our environmental obligations
• Detail our Quality Assurance obligations
• Train everyone in the Emergency Procedures and specify the first aid and medical
services
• Address all aspects of this Health & Safety Management Plan.

1. No workers are to work on site until they have successfully completed all of the
above.
2. Any deviation from this procedure must be approved by the Hydroserv Safety
Manager and Engineering Director.
3. Special interim inductions will be provided at times for consultants, technical
advisors, short term workers or workers who are yet to attend the full
Orientation. A host will escort such workers at all times.
4. Hydroserv will be required to give specific safety and health inductions for
their workers to supplement the Generic Induction.

7.10 Orientation for Visitors


7.10.1 Standard
Visitors must be approved for site access as per procedures specified by Hydroserv. Visitors
must remain under the control of the party they are visiting at all times while on site.
7.10.2 Requirement
All visitors must undergo a visitor’s induction. Hydroserv is responsible for the actions and
conduct of its visitors and shall ensure that visitors obey all Health & Safety rules of the site.

All visitors shall be accompanied at all times and issued with the required personal protective
equipment by the Hydroserv representative they are involved with. Under no circumstances
shall a visitor undertake any physical work on site.

8 SAFETY SYSTEM REQUIREMENTS


8.1 Risk Management
8.1.1 Standard
Meet the requirements of the Qatar Construction Specifications 2014 Sec. 11
8.1.2 Requirements

8.1.2.1 Hazard Evaluations


Hazard Evaluations will be undertaken by Hydroserv and others (if required) for the various
construction activities of the work. An established format will be followed. The intent of the
Hazard Evaluations is to be pro-active in identifying, evaluating and controlling hazards that
may cause harm during the construction phase or later operations of work site.

A team will be developed representing Hydroserv, to review the constructability of various


structures or facilities. This will ensure fabrication, construction, and commissioning of the
critical elements are undertaken in the safest possible manner at all times.

These Hazard Evaluation Studies will be conducted at the job site.

8.1.2.2 Risk Assessments


Hydroserv and others will conduct risk assessments in the form of Hazard Analysis for all
other work activities where safety of the work is of concern.

Qualitative as well as Quantitively results must be recorded.

8.1.2.3 Risk Controls and Review


Risk controls are put in place where the risk of an activity has been identified as not acceptable
These Risk Controls must be implemented and a review of the controls undertaken to verify
that no new risks exist, and this must be recorded. On completion of Risk Assessments, a
Standard Operating Procedure (SOP) will be developed.
The SOP will contain the following:
• recognition of existing procedures,
• the relevant workers,
• PPE,
• risk controls that have been agreed upon,
• accountability (who),

• action time (when),


• Risk Assessment for the activity,
• signatures of all participants undertaking work under the Standard Operating Procedure.

8.2 Work Permit System


8.2.1 Standard
Work permit systems are used to protect people, property and the environment from damage
during complex work activities. Hydroserv will seek approval before the energising of any
system on site. Work Permit systems for some phases of the work are required. Work Permits
and procedures will be identified and implemented. It will be a site requirement for Hydroserv
and sub-contractors to comply with such systems when mobilizing, Installing and welding
activities of HDPE pipes
8.2.1.2 Requirement
Hydroserv will seek accurate details from the client in the SOP’s describing conditions for
work activities that require work permits.

The work permit system may include, Work at Heights, Confined Spaces, and Hot Work

8.3 Incident and Hazard Reporting and Investigation


8.3.1 Standard
Hydroserv workers will report all incidents and hazards to Hydroserv Safety Representatives.

Hydroserv will record all incidents on the Hydroserv Safety System Database and also use the
Site Incident/Accident reporting and investigating procedure.
8.3.2 Requirement
All incidents and hazards must be reported to Hydroserv immediately and documented by way
of a preliminary report.
• All workers shall be instructed to notify their Hydroserv supervisor of the existence of
any substandard conditions, activities or equipment in their work area.
• Hydroserv are to ensure those hazards are corrected immediately to maintain a safe work
environment for their workers or other workers of the site.
• After attending to an employee’s injury/illness, the Hydroserv first aider shall record the
necessary information in the Incident/Hazard Report form.
• Hydroserv and subcontractors will be required to submit a weekly statistical report on all
incidents and hazards to the appropriate person responsible.
• In the event of an accident resulting in one of the workers on the Hydroserv contract
being absent from the Site for one or more shifts, then:

On the day of the accident Hydroserv shall in writing provide the following information to
Client Representative:
 The name(s) of the injured employee(s)
 The cause of the accident
 The nature of the injuries
 The length of time the employee will be absent from the site or an estimate if not
known.
• Hydroserv shall report to the Safety Officer all incidents which result in:
 Referral of employee(s) to a Doctor
 Any accidental fire
 Any near miss or significant incident equipment damage
• These incidents shall be reported immediately after the occurrence.
• Hydroserv are required to develop a plan of action, following an investigation, to instigate
changes that result in improved safety for the work activity.
• At the completion of each month, (the closing dates of which will be nominated by HSE
Representative), Hydroserv shall submit in writing a report for the month detailing:
 Total number of lost time injuries
 Total number of days lost due to accidents or occupational illness
 Number of first aid cases
 Number of employees referred to a doctor
 Total number of hours worked by employees
 Details of any employee/s placed on restricted duties
• It cannot be too strongly emphasised the importance that Hydroserv places on incident
reporting, even where there is no injury involved. Hydroserv and workers are also reminded
that failure to report injuries may prejudice a worker’s claim for Worker’s Compensation.

8.4 Prohibited Activities


8.4.1 Standard
Indulgence in practical jokes, fighting, possession of firearms, offensive weapons, possession
or use of alcohol or unauthorized drugs while on work sites, is strictly prohibited and may
result in the immediate withdrawal of site access and or accommodation provisions.
8.4.2 Requirement
No personnel shall indulge in any of the afore mentioned activities.
The Managing Director will specify Health and Safety rules that apply to this contract their
area of responsibility. The rules must at least satisfy all the legal rules and regulations and the
Site Rules. The Managing Director will ensure these Health and Safety Rules are
communicated during the site induction and during the Hydroserv site-specific induction for
newly arrived workers on site. They will ensure that any special language or literacy needs of
personnel are taken into account when communicating the rules.

8.5 Fitness for Work


8.5.1 Standard
Every worker will present himself or herself to work in a state that they can perform their
duties in a safe manner.
8.5.2 Requirement
All personnel will be required to work within the site” Fitness for Work” procedure.

8.6 Yard Security


8.6.1 Standard
The Company has a zero-tolerance theft policy. As part of yard security measures, we will
randomly inspect vehicles entering and leaving our yard area.

8.6.2 Requirement
The following terms of entry apply to any worker or visitor to the Hydroserv contract works.
• It is a condition of entry by workers and visitors that vehicles belonging to Hydroserv or its
workers and visitors may be searched whilst the vehicles are on yard and upon entry to or
exit from the yard site;
• Hydroserv authorises and consents to vehicle searches in accordance with this requirement;
• Hydroserv will be responsible for advising all of its employees and agents of this
requirement;
• Company may take any lawful steps to remove or exclude any individuals who fail to comply
with this requirement at any time;
• If Hydroserv advises Security Personnel that an individual has been excluded, Security
Personnel will not permit that individual to carry out any further work at the Yard or for the
Job Site unless Hydroserv revokes the exclusion;
• Nothing that Security Personnel does in the application or enforcement of these procedures
will amount to a breach of any arrangement or contract entered between vendor and the
Hydroserv or will result in any liability of any kind.

8.7 Injury Management and Rehabilitation


Meet the obligations of Workers Compensation.

8.7.1 Claims Management

8.7.1.1 Standard
Hydroserv shall accurately and efficiently manage workers compensation claims.

8.7.1.2 Requirement
Hydroserv shall establish a process to handle workers compensation claims.

8.7.2 Rehabilitation
8.7.2.1 Standard
Rehabilitation is to commence as soon as medically approved following an injury or illness and
every effort will be made to assist workers in an early and safe return to meaningful and
productive work in consultation with treating practitioners.
8.7.2.2 Requirement
Hydroserv has a rehabilitation policy and procedure as part of this Health & Safety
Management Plan and shall nominate a rehabilitation coordinator for the site

Hydroserv will establish good communication with the injured worker as soon as possible and
shall maintain communication throughout the rehabilitation process

As part of the return to work process, Hydroserv shall provide alternative duties, training and
other vocational rehabilitation services where appropriate.

Meet the obligations of the Qatar Labor Law 2004, Article 109

8.8 Health, Safety & Environment Inspection & Frequency

The HSE inspection play a significant role in an organisation with respect to health & safety of
workers and helps to prevent from incidents, injuries and illnesses and helps to identify and record
the hazard & risk for corrective action. The inspection must examine who, what, where, when and
how.
The HSE inspection is a part of occupational and environmental health and safety systems for
checking that an organisation’s operating and working procedures are meeting the required legal
standards. An HSE inspection can identify occupational hazards and be the first step in introducing
measures to improve conditions. The HSE inspection can be formal, informal, recorded or
unrecorded, but the important aspect is that HSE inspections are carried out to a set standard at an
appropriate frequency.
HSE Inspection Planning
The HSE officer plan the schedule and visit the Project, Site and perform the inspection upon the
checklist “HSE Inspection Report” ref. no. HYDRO/MGT/FR-21 and identify the HSE personnel
and create a awareness program and make all site workers responsible to report on “Near Miss”
and prepare the “HSE Performance Report” ref. no. HYDRO/MGT/FR-20. The unplanned HSE
Inspection instruct by General Manager or Engineering Director upon the severity of HSE concern.
The following describes three other types of inspection reports:

The Supervisor and workers should inspect the HSE requirement prior execution of work and do
perform all necessary arrangement and ensure that work is completed safely. The ongoing
inspections is a part of the worker’s job responsibilities such inspections identify hazardous
conditions and either correct them immediately or report them for corrective action. The frequency
of these inspections varies with the amount and conditions of equipment use.
The Periodic inspections are regular, planned inspections of the conduct at regular interval as
below mentioned;

9 COMMUNICATIONS
9.1 Kick-off Meetings
9.1.1 Standard
Kick off meetings will be held to provide Hydroserv with all relevant site Safety Procedures
and Rules, and deal with all Safety issues relating to Hydroserv work
9.1.2 Requirement
Engineering Director will hold a Hydroserv kick-off meeting before Hydroserv starts any
work. The first item on the agenda shall be Health & Safety and Environmental issues.

9.2.1 Standard
Pre-Start Safe Operation Meeting or Safety Talk Analysis Risk Reduction Talk will identify
hazard and risk areas before activities are commenced.
9.2.2 Requirement
The Hydrosev procedure HYDRO/HSE/PR:11 “Pre-Start Safe Operation Meeting” shall be
followed.

9.3 Safety Walk Through


9.3.1 Standard
The Hydroserv Site Safety Officer may also form a team who will walk the site from
mobilization to demobilization, observing and monitoring dewatering safety to develop new
strategies to further improve site dewatering safety.
9.3.2 Requirement
The Hydroserv Site Safety Officer will regularly conduct site Safety forums for the
improvement of overall dewatering safety.
• The Hydroserv Site Safety Officer will conduct weekly or fortnightly safety meetings
with all Hydroserv project site safety personnel with the aim of striving for improved
safety.
• Each week the Safety Walk through will be conducted as a “Safety Coordination”
meeting and may include the operation manager, all Supervisor’s management.
• The agenda will vary but will always focus on Continuous Improvement for site safety

9.4 Safety Tool Box Talk Meetings


9.4.1 Requirement
Toolbox meetings shall be used to highlight specific Health and Safety issues relevant to the
work team.

The Hydroserv procedure HYDRO/HSE/PR:11 “Toolbox Meetings” shall be followed.

9.5 Safe Behaviour


9.5.1 Standard
A program that utilizes behavior modification techniques to improve Health & Safety culture
through peer recognition of sub-standard practices, shall be put in place.

9.5.2 Requirement
Hydroserv will support the program by encouraging Line Management and workers
participation.
• The HSE Officer will carry out regular observations of agreed Risk activities and identify
both safe and substandard behaviour against the behaviour that was identified in the Risk
management process as preventing damage
• At regular intervals the Supervisor, HSE Officer on site and observers will review the
observations.
• The Supervisors will provide feedback to their work team after the observation to verify
whether the work methodology reflected the written procedure.
• To create greater awareness for workers on site as well as Line Management on outcomes
and improvements.

9.6 Safety Recognition Program


9.6.1 Standard
The Safety recognition program is designed to maintain the workers interest in the site and to
encourage personnel to maintain a safe working environment for themselves and for their co-
workers.
9.6.2 Requirement
Hydroserv shall administer a recognition program that rewards Health & Safety performance at
the individual, team, and Hydroserv levels.

9.7 Regular Management Performance Reviews


9.7.1 Standard
Conduct a review process of Hydroserv safety performance.
9.7.2 Requirement
Hydroserv Management will review the progress of Hydroserv against their Health & Safety
Management Plans, at regular intervals. The Operation Manager and Safety Saftey Officer will
attend weekly safety meetings with Client and the Elected Safety Representative will also
attend a weekly safety meeting with Client.

10 TRAINING
10.1.1 Standard
Hydroserv will establish a uniform method of providing Health and Safety awareness training
to all workers assigned to the site.

Hydroserv are responsible for ensuring all workers are competent and adequately trained for
any work activity in which they are engaged.

10.1.2 Requirement
Hydroserv will provide adequate training for their workers for every work activity. Such
training must include the site Health and Safety requirements that might apply to the specific
work activity.

In order to ensure competency, a detailed Health and Safety Training Program Schedule shall
be prepared and maintained by Hydroserv. The statutory training package will be forwarded
onto the Operations for approval. All skills training showing evidence of training
competencies will be located in the computer based Incident and Training database.

The following training topics shall be included in the training schedule:


• Health & Safety induction
• Effective Tool box meetings (facilitators)
• Effective Pre-start Health & Safety Talks (facilitators)
• Hazard communication
• Emergency Preparedness
• Use of Personal Protective Equipment
• Additional topics that may be appropriate include:

Other specific topics appropriate to the work being undertaken.

The proposed training program shall include the type of course, the course duration, training
contents and name and qualifications of the training provider.

Workers who receive assignments that are not understood have an obligation to request
additional information and further clarification before they commence their work activities.

During periods when work is prevented from occurring (rain etc.), Hydroserv will have

11 HEALTH AND HYGIENE


11.1 First Aid & Medical Services
11.1.1 Standard
First Aid response will be immediately available to all workers following injury on site.
Furthermore, professional paramedical personnel will be provided for more advanced care.
11.1.2 Requirement
Hydroserv are required to provide a qualified first aid trained person. Such First Aider must
have a current certificate of competency to Senior First Aid level as a minimum.

Furthermore, Hydroserv is to provide a comprehensive first aid kit in the work area. Except for
those of a very minor nature, all injuries including every eye, back, and head injury are to be
treated by professional medical providers. Client has onsite a Paramedic Service and all
persons under the control of Hydroserv will report to the paramedic accompanied by a
Hydroserv person after giving notice to the Hydroserv First Aider.

It is a requirement that when any group works in isolation, there is a qualified first aider in the
group and a suitable first aid kit.

Meet the obligations of the Qatar Law No. 14 of the year 2004-The Labor Law

11.2 Manual Handling


11.2.1 Standard
Employers are required to provide for the general health and hygiene needs of their workers.
Managers and supervisors will ensure that jobs, workplaces, tools, and equipment fit the
capabilities of their people.
11.2.2 Requirement
Hydroserv will consult with their workers and others in regard to manual handling exposure
when undertaking Hazard Evaluations on site activities.
• Task observation activities will, amongst other things, consider physical or mental
fatigue (stress).
• Where unacceptable manual handling risks are identified, Hydroserv must take action to
remedy the situation. Efforts should be made to resolve all manual handling risks and
thus protect their workers.

11.3 Noise Control/Hearing Conservation


11.3.1 Standard
Legislation requires that suitable hearing protection be worn while working in areas or on plant
or equipment where the noise level is above 85 (dB)A for an 8hr work shift, or a reading of
140 lin
11.3.2 Requirement
Hydroserv will measure noise levels to identify high noise areas. Hearing protection must be
worn in designated areas, which will be clearly marked with appropriate signs.
• Hydroserv is responsible for designating and adequately sign posting areas under their
control, as hearing protection zones, where noise levels are regularly found to be above
85 (dB)A.
• Workers have an obligation to protect their hearing when exposed to noise levels above
85 (dB)A. Hydroserv must make a selection of suitable hearing protective equipment
available to their employees and visitors at all times
• Ear muffs and ear plugs used on site shall meet the OSHAS Standard and will provide a
minimum of 22 dB protection.

11.4 Biological Hazards

11.4.1 Standard
Hydroserv have a responsibility to protect their workers and other from hazards arising
from the environment.
11.4.2 Requirement
When biological hazards are identified on site, Hydroserv shall provide details in this
Health & Safety Management Plan on how they will control exposure to the biological
hazards. Appropriate information and training will be provided to workers through specific
SOP’s for the activities involved.

11.5 Smoke Free Environment


11.5.1 Standard
Smoking is prohibited in all buildings, crib rooms & lunch areas, confined spaces, PSC Cabins,
vehicles, fuel & lubricant storage areas, designated restricted areas and in any location where
passive smoke is a nuisance to other workers.

11.5.2 Requirement
Hydroserv will clearly mark smoking areas and ensure that their workers and others observe
the smoke free status of other areas.

11.6 Working in Extreme Temperature


11.6.1 Standard
Hydroserv will protect people who work in extreme temperatures.
11.6.2 Requirement
Hydroserv will identify extreme temperature sources and implement appropriate hazard control
measures.

Where an environment of extreme temperature exists, Hydroserv will use means of control that
include, but are not limited to:

1) Ensuring that people who have to work in such conditions are in good physical condition.

2) Providing shelter and cool drinking water.

3) Using power tools, lifting aids and/or other devices to reduce physical exertion.

4) Rescheduling work in areas so that strenuous work is done in the coolest part of the day.

11.7 Regional Health Issues


11.7.1 Standard
Regional Health issues will be identified and addressed.
11.7.2 Requirement
Hydroserv will monitor the generally available information on the existence of particular
Health issues in the region of the Qatar Project Site and will notify all workers and others of
any special precautions that need to be taken.

Hydroserv will implement any such precautions as required.

11.8 Amenities and Personal Hygiene


11.8.1 Standard
Hydroserv shall maintain a high standard of hygiene at all times in the facilities provided and
work practices. The standard will be such as to prevent an environment conducive to bacteria,
disease, and infection.
11.8.2 Requirements
Hydroserv are required to provide for the general health and hygiene needs of their workers
and others.

• Hydroserv shall provide an adequate supply of personal cleaning products, barrier creams
and sunscreen protection for their workers.
• Hydroserv shall provide adequate amenities for workers.
• Amenities include crib or lunchrooms, refrigerated drinking water, washing & toilet
facilities etc. Such amenities shall be maintained in a clean and tidy condition and with a
shared responsibility of all who use them.

12 PERSONAL PROTECTIVE EQUIPMENT


12.1 General
12.1.1 Standard
All workers and visitors will protect themselves by:
• Identifying the PPE needs for tasks done in their areas of responsibility by using risk analysis
and other hazard identification processes.
• Ensuring that there is appropriate PPE available in sufficient quantities for workers.
• Ensuring that PPE is kept in good working condition or replaced before it becomes unfit to
wear.
• Ensuring that everybody on site has a thorough understanding or is instructed and trained in
the proper use and maintenance of PPE.
• Ensuring that PPE is worn where it is mandatory.
• In instances where a question arises concerning appropriate Personal Protective Equipment,
the Hydroserv Safety Manager will be consulted for a determination as to the suitability and
conformance of specific equipment.
12.1.2 Requirement
Personal protective equipment shall be provided by Hydroserv and shall be worn by all
personnel on site, except when in car parks, offices, designated lunch rooms, while walking to
and from the job at start/end of shift, or when operating a vehicle or machine with a fully
enclosed cab.
The excepted areas are void when a hazard exists or manual work is being carried outing that
area.
Hydroserv has identified specific items of Personal Protective Equipment and clothing that will
be the approved lines for issue by Hydroserv.
Worn or damaged Personal Protective Equipment must be replaced once it no longer provides
the intended Safety protection.
It is the responsibility of individual workers to clean and maintain their personal protective
equipment and clothing.

12.2 Clothing Requirements


12.2.1 Standard
Workers working on the Job site must wear cotton long sleeved shirts and long trousers or full-
length overalls in accordance with the standards. High visibility shirts or vests are required on
the site. Unless nominated otherwise as a result of Risk Assessment, long sleeved shirts are to
be buttoned up at the sleeve
12.2.2 Requirement
Hydroserv are required to supply their workers with personal protective equipment and
clothing appropriate for the work being performed, and must maintain it in good order.
When working in an area where there is a hazard such as rough surfaces, hot metal sparks, hot
or corrosive vapors, liquids gases, or dusts, compressed air or explosive flash, the project will
determine the appropriate additional protective clothing to be worn in consultation with the
Hydroserv Safety Officer.

Clothing soaked with grease, paint, thinners, solvents or similar materials shall not continue to
be worn.

12.3 Eye Protection


12.3.1 Standards
All personnel on the Site shall wear safety glasses and eye protection complying with Qatar
Construction Standards.

HYdroserv shall provide a system for workers who require prescription Safety glasses.

Side shields are to be fitted to all Safety glasses which have no side covering.

All persons working on (or are near) jobs such as chipping, chiseling, grinding, wire brushing,
lathe work, paving breakers, rock or cement drilling, using heavy hammers or sledges,
handling glass, tinning, casting, welding or where there is a greater likelihood of an eye injury
shall wear well fitted mono-goggles and/or full-face shields.

12.4 Foot & Head Protection


12.4.1 Standards
Safety helmets and ankle length, lace-up safety boots with class 1 steel toe caps, shall be worn
by all personnel on the construction site. Thongs, sandals or sand shoes will NOT be permitted.
Riggers & roof plumbers may wear a site approved alternative type of footwear with a non-slip
sole (sand shoe) when engaged on work at height. Steel capped Safety footwear shall be worn
when rigger/s are not aloft. The use of wide rims over hard hats must be available to all
personnel and is recommended to be used, in addition to PPE.

12.5 Respiratory Protection


12.5.1 Standards
Respiratory protection is required whenever airborne contaminants exceed or are anticipated to
exceed the Qatar Construction Exposure Standards.
Cartridge type respirators are to be used except for nuisance dust, which is below the exposure
standard. These types of respirators are not to be used in oxygen deficient atmospheres. Only
airline units or self-contained breathing apparatus can be used in an oxygen deficient
atmosphere.

Those required to wear respirators will be clean-shaven to facilitate good seal on the face. The
Hydroserv reserves the right to require face fit tests for respirators on the site.

Workers working where there is a specific high-risk air contaminant must carry an appropriate
respirator, which provides protection from the airborne risk.

12.6 Hand Protection


12.6.1 Standards
Suitable gloves shall be worn when handling metal, wire, rough timber or any material where
the hands may be subject to abrasive action, acid, chemicals, or hazardous liquids or chemicals,
petrol or oil drums, hot metals, gases or flames.

12.7 Other Personal Protective Equipment


12.7.1 Standards
Other equipment may be required for unusual circumstances such as extreme temperature
work, handling corrosive liquids, etc. Requirements not specifically covered in this section
shall be reviewed with the Hydroserv Safety Officer.

Welfare, Safety and protection of workmen and Personal Protective Equipment/Attire

12.8 Housekeeping
12.8.1 Standard
Hydroserv shall ensure that their area of responsibility is kept in a neat and tidy manner and
that tools, equipment and materials are stored away when not in use.
12.8.2 Requirement
Housekeeping is the responsibility of every worker on the Project. The supervision of
housekeeping is the responsibility of the Hydroserv.

Work areas, passageways, stairways, and all other areas shall be kept free of debris and
materials and trip hazards.

Waste food, rubbish pipe and plastic generated by grinding the HDPE pipe shall be placed in
black plastic bag strategically across the job site and used for disposal of scrap materials and
other installation generated debris.

Storage areas shall be kept clean, and materials neatly stacked or placed. Walkways and other
areas where personnel travel shall be maintained free of trip hazards equipment, obstructions,
and other materials, which may cause an accident or injury. Construction materials shall be
stored or placed in an orderly manner.

Solvents, paints, oils, greases, and other such material and containers such as tins that have
contained chemicals shall be disposed of in an approved manner.

Crib rooms and eating areas shall be kept clean and free of all food scraps, wrappers, paper
cups, and other disposable items.

13 MOBILE PLANT
13.1 Standard
Hydroserv will ensure all activities involving the operation of mobile plant will commence with a
hazard analysis involving persons representative of the roles. Review HYdroserv or Clients
procedures and finalize the activity within a Standard Operating Procedure.

13.1.1 Requirements

Hydroserv will commence a hazard study on mobilization and set-up of mobile plant.

Hazard Studies will also be performed on activities associated with clearing, loading, hauling,
dumping, towing and Lifting. Previous studies and procedures must be revisited and reviewed.
The reviewed SOP is to be adopted.

13.2 Inspection of Plant on arrival

All plant will be cleaned to ensure Environmental protection requirements prior to arrival to site.
On arrival to site, a Machinery Safety inspection is to be conducted. The Hydroserv Plant
Inspection procedures are to be used.

Daily Inspection

All operators are to conduct inspections of plant they are to operate on each shift prior to
commencement of work. The Hydroserv Daily Operation Sheet completed by the Operator is to be
used, completed and returned to the Supervisor. The Supervisor in conjunction with the
Maintenance Supervisor is required to prioritize the faults identified and to ensure compliance and
no risk to operator or persons on site.
13.3 Maintenance Inspections

At each daily service and at the regular service intervals, maintenance personnel will inspect all
plant using the Hydroserv maintenance forms.

14 DRILLING AND INSTALLATION OF DEEP WELLS


14.1.1 Standard
Prior to drilling of deep well the locations of the proposed wells shall be set‐out by the main
contractor. Existing services on the areas shall be checked and verified that it shall not
conflict with the drilling works. Drilling is to be undertaken and maintained in a safe manner.

14.1.2 Requirement
Drilling and Installation of deep wells work cannot be commenced on site unless an Drilling
application has been submitted and then approved by Main contractor and Hydroserv.
• In compliance with Main Contractor Procedure - Drilling, installation of deep wells work
shall not commence on site unless an Drilling permit has been submitted and approved by
the Project Manager.
• For all drilling work a competent drilling supervisor will be appointed. The level of training
and experience required to determine competence will vary depending on the complexity of
the drilling. He will have previous experience of the type of drilling work to be undertaken
and formal Occupational Health and Safety training relating to the safety of drilling work.
• Once the design requirements are known a job hazard analysis of the drilling work will be
completed by the drilling supervisor with the assistance of Occupational Health and Safety
staff and engineers as required. The Job Hazard Analysis will clearly identify the job
specific risks and control measures for the drilling work.
• Prior to commencement of any drilling and installation of deep wells work all persons
involved will be given a pre-work briefing based on the Job Hazard Analysis relating to the

risks and required control measures for the drilling work. The appointed drilling supervisor
will be responsible for presenting and recording the briefing.
• Prior to commencement of work all materials, plant and equipment will be checked to
ensure that they are in good condition. In particular the condition of all mechanical drilling
rig will be checked to ensure safe operation.

15 WORKING AT HEIGHTS
15.1 General
15.1.1 Standard
Hydroserv are to ensure the safety of all personnel Working at Height.
15.1.2 Requirement
The preferred method of ensuring safety at heights is to provide a fully enclosed environment
in every instance. This can be achieved through physical barriers.
• Control measures to consider include working platforms, roof mesh, edge protection,
guard-railing, containment sheeting, penetration covers, working platforms, scaffolds,
fall arrest systems are the least preferred method of providing fall protection.
• Where the use of fall arrest equipment is necessary it shall be in compliance with
applicable regulatory standards.
• Safety harnesses with safety lanyards or of an inertia reel style fall arrest system, shall be
worn by personnel working 1.8 metres or more above the ground and there is a potential
to fall on assessment. Each fixed safety lanyard shall be no more than 1.5 metres or
arranged so that the wearer can fall no more than 600mm.
• Only approved, full body harnesses are acceptable. Harnesses are to be provided with
shock absorbers. Paired lanyards are to be provided for all applications where the worker
has to un-clip to move location while at height.
• In situations where a fall could result in impalement, fall protection equipment shall be
utilised regardless of the potential falling distance.
• Personal fall prevention equipment must be inspected daily prior to use by the user.
• Records of regular inspections must be maintained.
• A competent person designated by Hydroserv shall inspect safety harness assemblies,
lanyards, and other personal equipment monthly.
• Auxiliary fall protection equipment such as static lines and inertia reels shall be utilised
by personnel travelling from one location to another in elevated positions.
• No work is to be carried out on job site by people exposed to falls and who are not totally
protected.

15.2 Barricading
15.2.1 Standard
Hydroserv are to take great care with the barricading of work areas, delineating their sites and
protecting potential fall zones.
15.2.2 Requirement
Hydroserv will comply with the project Procedure “BARRICADING”
• Barricades on the job site are used to restrict entry and/or provide warning for areas that
involve construction activity, that contain safety hazards, abnormal conditions, or in
which unusual operations are being performed.
• Barricading should also be used to delineate work zones.
• Hydroserv are responsible for erecting and maintaining barricades that are required for
the protection of workers and personnel.
• Where a person could fall 1 metre or greater, then such barricading must also provide a
physical barrier such as a fence or guardrail. For the purposes of carrying out further
work or handling materials within the barricade, sufficient length of the barricading or
guardrail may be removed, provided that it is replaced immediately on completion of
such work.

15.3 Cranes and Material Handling


15.3.1 Standard
Hydroserv shall ensure the safe operation of cranes by complying with Qatar Construction
standards and ensuring that all operators and crews are competent and qualified.
15.3.2 Requirement
• Only licensed crane drivers are authorised to operate/drive any crane. Lorry Loader are
not permitted to work with a crane unless licensed by a competent authority.
• All cranes will be subject to a thorough safety machinery inspection prior to use on site.
NDT (non-destructive testing) of critical components may be requested. Recent
inspection certificates will be required.
• All lift boxes or materials boxes and elevated work platforms shall be used in accordance
with the site procedure.
• Proper barricades shall be placed around the swing radius of cranes and other lifting
equipment when appropriate.
• Cranes with a free-falling capacity will not be allowed to work on site unless it has a
padlock lockout control to prevent the free fall of the load.

• Cranes, elevated work platforms and other lifting equipment shall be inspected daily by
the operator and recorded in a Daily Inspection Log. All defects or repairs needed shall
be recorded on the log.
• Accidents with cranes and lifting loads are an insidious ongoing problem in work, which
we are determined to eliminate.
• Lifting loads of any type can be hazardous. It is imperative that someone takes charge of
the lifting activity and that the following factors are carefully considered:
• Know the weight of the load, plus the lifting gear, the hook and the rope to be used.
(These must be calculated into the load for compliance with the crane-lifting chart)
• Know the load radius.
• Know the boom length and parts of line required.
• Consider any operating area restrictions
• Consider and understand all other factors and limitations nominated for the specific
crane.
• Consider the site conditions and the weather.
• Consider how this lift impinges on other concurrent activities and personnel and make
sure they are kept informed.
• Hold a safety briefing with the crane operator, the Lorry Loader and others involved
before commencing the lift.
• A High Voltage Permit must be applied for when lifting is carried out within 15 meters
of overhead high voltage lines.
• With any complex lift a documented LIFT STUDY shall be prepared.
It is not just heavy loads that constitute complex lifts. Complex lifts might include but are not
limited to, lifts at great heights, lifts in confined areas, lifts out of the sight of the crane
operator and multiple crane lifts. Lift studies are to be submitted to Hydroserv HSE department
for approval.

15.4 Lifting Equipment


15.4.1 Standard
Hydroserv will ensure that all hoisting equipment and lifting mediums, has been thoroughly
inspected and certified as being in a safe condition before use on site.
15.4.2 Requirement
HYdroserv shall invoke the standards laid down in the Acts and regulations in relation to Cranes
& lifting appliances.
Rigging equipment that is in an unsafe condition shall not be used on site. Any defective
equipment shall be immediately reported to the supervisor and removed from service.

A competent person, nominated by Hydroserv, shall carry formal inspections of all


lifting/rigging equipment every 3 months.
Inspections must be carried out prior to every lift.
Documented evidence of all such inspections and maintenance shall be kept on site.
Hydroserv and sub-contractors, who intend utilising fabric slings on site, shall provide details of
the inspection regime that they utilise to ensure they are in serviceable condition Systems
shall be put in place for the care &proper storage of all lifting equipment

6 ELECTRICAL EQUIPMENT AND OTHER ACTIVITIES


16.1 General
16.1.1 Standard
Hydroserv are to ensure all electrical equipment is safe to use and that all electrical work is
undertaken in an approved safe manner.
16.1.2 Requirement
Hydroserv will comply with all Qatar Standards that address electrical safety.
• All portable, semi portable and transportable electrical power tools, equipment, leads,
earth leakage boxes etc. must be correctly inspected and certified as safe by some
qualified electrician and fitted with the appropriate compliance tag when they arrive on
site and then quarterly thereafter.
• A Hydroserv log is to be maintained on all electrical equipment.
• All portable power sources must be provided with Residual Current Protection. (RCD or
ELCB)
• Extension cords must not have taped joints or repairs. All leads must be positioned so
they are not liable to mechanical damage.
• Hydroserv are to supply appropriate electrical cable stands at their site, for the use of
their employees.
• Where this is impractical, additional mechanical protection must be provided for the
cables. particularly across pathways. Cable wires are not to be placed across roadways
under any circumstances. They may be elevated as aerials or be taken along with the
HDPE pipes. Electrical socket outlet adaptors, board adaptors, double adaptors, other
than power boards, are not to be used on the work site.
• Flexible cable shall only be allowed for hand lamps and hand-held tools and shall not
exceed 6 meters in length. Industrial type plugs and sockets shall be used. All electrical
installations shall oversee a competent person who shall accept full responsibility for its
use and any alterations or additions thereto. The name, designation and telephone number

of such person shall be prominently displayed close to the main switch or circuit breaker
of the installation, and in the Contractor’s site office.
• Electrical hand tools should be double insulated. Portable generators and welders with ac
outlets must be earthed to ground before use. This can be achieved with a heavy ground
lead attached to the frame and fitted with an earth stake, which should be driven into the
ground to 500 mm.
• Where the plant is operated in a partially constructed building or where an earth stake
cannot be used the unit should be earthed to the building foundations through column
bolts, reinforcing etc.

16.2 Welding (Butt Fusion and Electric)


16.2.1 Standard
Hydroserv are to ensure all butt fusion and electric welding activities are undertaken in a safe
manner, by competent personnel and in compliance with Qatar Construction Specifications.
16.2.2 Requirement
All welders engaged in electric and butt fusion welding shall wear the appropriate protective
shields and goggles in accordance with the Qatar Construction Specifications. Aprons,
gauntlets and welding spats shall be worn by welders and assistants where there is danger of
injury from hot material. All electric welders will wear insulated footwear.
• Welding leads must be inspected before use and checked for exposed parts. The welding
ground return shall be placed on the material to be welded closely adjacent to the weld
arc area. Any variations to this technique can only be carried out with the approval of the
Hydroserv Safety Officer.
• Welding and cutting shall not be performed on drums, tanks or other containers until it
has been ascertained they are free of flammable or combustible materials.
• Welding machines must be turned off whenever the job is complete and/or unattended.
Welding gear and electrical cable wire must be inspected before use and all connections
kept tight and secure.
• Equipment should be placed away from floor openings, edge of platforms etc. and all
hoses and power cable kept clear of walkways.
• In elevated areas where falling sparks could cause a fire or explosion, fireproof blankets
shall be used to isolate the work from the surrounding area. This applies to work such as
electrical welding, cutting and grinding operations.
• Where welding is to be carried out at ground level, welding screens are to be used to
minimise risk of fire and to screen nearby workers from welding flash

• A fire extinguisher will be accessibly located near the hot work with a backup
extinguisher within 20 metres. HDPE pipe grinder and heating plate must be stored in an
upright position and adequately secured. Water for burns irrigation and a first aid kit must
be available near welding activities, in case of accidental burns.
• Welding and cutting in some controlled areas may require a Hot Work Permit.
• Both electric welding cables and hydraulic hoses must be placed so as not to provide
tripping hazards and should be protected from mechanical damage. They shall not be laid
unprotected across roadways.
• Welding electrical cables with broken insulation will be taken out of service or repaired.
• Welding machine must be adequately secured when transported around site and must
only be used in an upright position.
• Whenever butt fusion or electric welding, cutting, burning, heating, or melting work is
done near combustible materials, either inside or outside of buildings, observe the
following precautions:
• Before heating or welding an HDPE pipe, check the blind side for combustible materials.
• Contain waste material or hot slag produced from overhead work by using fire blankets,
metal catch pans, or other appropriate means.
• Prevent flames, sparks, molten slag, and not metal from coming in contact with
combustible materials and debris.
• Move all combustible materials within the area to a safe distance. If, because of excessive
weight or bulk, the material cannot be moved, protect it with a fire-retardant material or
wet it down.
• Ensure that the work area is free of airborne combustible gases and flammable liquids.
• After completing the job or before leaving the area, check the immediate work area for
signs of fire.
• A suitable fire extinguisher shall be available for instant use in any location where
welding is performed. Screens, shields, or other safeguards shall be provided for the
protection of personnel or materials exposed to sparks, slag, falling objects, or the direct
rays of the arc.
• The welder shall wear approved eye and head protection adequate for the nature of work
being performed. Workers assisting the welder shall also wear similar protective
equipment
• Ventilation shall be provided in those areas where the possibility of accumulation of
fumes or vapours may occur.

16.3 Compressed Air


16.3.1 Standard
Hydroserv must ensure compressed air is always used in a safe manner.
16.3.2 Requirement
Use compressed air only for air-driven tools and such other uses as are safe. Do not discharge
compressed air against any part of the body of any person, or use it to blow dusts off clothes.
These practices can cause serious injury.

The practice of using compressed air for cleaning purposes, by blowing material is not
normally allowed. Blow cleaning can only be approved by the Hydroserv Safety Officer and
only following a thorough risk analysis.

Vacuum cleaning is the preferred cleaning method

Airline hoses for tools and other equipment shall be secured together to preclude uncontrolled
whipping in the event that couplings become separated while under pressure. Air supply lines
shall be protected from damage, inspected regularly, and maintained in good condition.

All pressure vessels shall comply with the State regulations for inspection and testing and
pressure vessel certificates are to be provided to the Hydroserv Safety, when such equipment is
brought onto site.

16.4 Hazardous Substances


16.4.1 Standard
Hydroserv are to ensure the safe management, storage, handling, end use, and disposal of all
hazardous substances used on site.
16.4.2 Requirement
Compliance with Project Procedure - Hazardous Substances as follows;

Hydroserv shall not be entitled to bring on to the job site any hazardous substance of any nature
or description whatsoever except as provided in this clause.

Subject to above Hydroserv shall, in writing, notify the Site Chemical Co-Ordinator appointed
by the main contractor:

• the hazardous substance which Hydroserv wishes to bring on to the job site setting out
the trade name, the chemical names of all constituents and the approximate proportions
of those constituents;
• the purpose for which Hydroserv intends to use the hazardous substance setting out the
reason why the hazardous substance is necessary for the carrying out by Hydroserv of
the services;
• the location in which Hydroserv intends to store and use the hazardous substance; and
• the proposed method of disposal of the hazardous substance.

The Site Chemical Co-Ordinator shall then advise Hydroserv whether Hydroserv shall be
permitted to bring the hazardous substance on to the job site.

If permission is granted by the Site Chemical Co-Ordinator to bring the hazardous substance on
to the mine site, then Hydroserv shall:
• store, use and dispose of the hazardous substance in accordance with its notification to
the Site Chemical Co-Ordinator; and
• after using or otherwise disposing of the hazardous substance, notify the Site Chemical
Co-Ordinator in writing of the date and use or method or disposal of the hazardous
substance and the name of the person responsible for the use or disposal of the hazardous
substance.
Copies of the Material Safety Data Sheets shall be maintained by the Hydroserv Safety Officer
and will be made readily accessible to employees engaged on the site. A copy must be supplied
to the main contractor HSE office and site clinic.

Meet the requirements of the Qatar Construction Specifications 2014, Section 01, Part
10.7.3 Occupational Safety and Health, Fire Precaution and Prevention for Hazardous
Substances.

16.5 Fire Prevention and Control


16.5.1 Standard
Hydroserv shall put in a system to identify and control flammable and combustible materials
and ignition sources and to provide adequate levels of fire protection.
16.5.2 Requirement
Hydroserv shall invoke the standards laid down in the regulations.
• The exposure on sites is for Class B fires, B (e) dry powder Extinguishers. (4.5 & 6kg units
are recommended) There should be an extinguisher in every PSC cabin, Generator, office,
store and workshop location, on every piece of mobile plant and other designated
equipment wherever combustibles or petroleum products are stored or decanted.
• Foam extinguishers may be required at fueling facilities. Where there is a possibility of
Class A fires. Hydroserv must ensure that fire-fighting water is available. For special

circumstances i.e. switch rooms or where there are computers carbon dioxide extinguishers
should be provided. Hydroserv is to provide instruction to their workers on safe working
practices in relation to fire prevention and protection.
• Fire teams will be established from Hydroserv workers and regularly trained at the site
during normal work hours.
• Work areas must be kept clean and free of combustible waste and scrap materials at all
times.
• Particular care must be taken when welding and cutting in locations where combustibles
are exposed. When such welding or cutting is done, the surrounding area must be
inspected. Combustible material shall be removed or protected with fire resistant blankets,
and an adequate number of approved fire extinguishers must be immediately available.
• All flammable and combustible materials shall be stored and handled with due regard to
their fire characteristics. Flammable liquids must be stored in an approved manner, and
dispensed only in acceptable safety containers.
• Oxygen and combustible gas cylinders must be stored at least 3 meters apart or separated
by a wall with a fire rating of 1 hour.
• Fire hydrants and stop valves adjacent to the work area shall be maintained clear and readily
accessible. No material or other obstruction shall be placed, parked or stored within five (5)
meters of any hydrant or stop valve. Access to fire extinguishers shall not be blocked.
• Inspection and maintenance of firefighting equipment and fire control measures shall be in
accordance with statutory regulations plus Hydroserv must inspect extinguishers at least
monthly.
• Each vehicle used by Hydroserv upon or about the Work, shall be fitted with at least one fire
extinguisher to a standard that is acceptable to main contractor’s Health and Safety Co-
ordinator.

10.7.1 Occupational Safety and Health, Fire Precaution and Prevention.

16.6 Machine Guarding


16.6.1 Standard
Hydroserv are to ensure that all machinery used on site has adequate safeguarding to all
moving parts.
16.6.2 Requirement
Hydroserv will ensure that employees are protected against accidental contact with moving
parts of fixed machinery, power tools and mobile plant.
Hydroserv are to regularly inspect and audit the safety status of their tools and equipment in
relation to machine guarding.

Unguarded equipment is to be tagged “Out of Service” immediately.

16.7 Lone and Isolated Workers


16.7.1 Standard
Hydroserv will as far as practical ensure that workers do not work in isolation.
16.7.2 Requirement
Where practicality requires a worker to work alone, the supervisor shall provide means of
communication and establish a means of contacting the worker once every thirty (30) minutes
to ensure their well-being.

16.8 Vehicle Operation


16.8.1 Standard
Only vehicles in safe operating condition shall be allowed on the site

16.8.2 Requirement
All personnel operating vehicles and plant shall observe the established Road Code of Qatar as
well as observe all site Health & Safety requirements.
All vehicle drivers shall possess a current Qatar driving license. All plant operators shall
possess the appropriate certificate of competency.
Pedestrians shall always have the right of way except on sign posted Haul Roads.
Vehicles shall be properly lighted during dark hours and mounted with revolving lights.
Overhanging loads shall be properly flagged, or a flag person provided.
Haul vehicles on sign posted Haul Roads have right away at all times.
Roadways and access to fire protection equipment shall not be blocked.
Personnel shall not be permitted to ride on moving equipment i.e. cranes, loads being lifted,
fork lift trucks, front end loaders, trailers, trucks or the back of utilities.
Workers are prohibited from riding on loads, fenders, running boards, and tailgates.
Workers must sit on the provided seats only.
Seat belts shall be used where available in all vehicles or equipment.
All personnel shall rigidly obey speed limits and traffic signs.
Vehicles shall be maintained in accordance with the manufacturers’ recommendations and the
maintenance records kept.

16.9 Signs
16.9.1 Standard
Hydroserv are to comply with the sign standards that have been set for this site.

16.9.2 Requirement
Signs shall be in be conformance with the Hydroserv standards, which have been established
for signage. There are statutory and site regulations regarding the use, layout, color, posting,
wording and size of all signs and tags. Compliance should be established through the
Hydroserv Safety.

The Hydroserv Project Manager & Safety Officer are responsible for authorizing the general
posting of signs on the project relative to warnings, precautions, notices, and other information
pertinent to workers, and for all road access, and closures.

Hydroserv are responsible for posting or displaying signs relative to their scope of work. For
example: No smoking signs, Electrical Hazards, required at a PSC Cabin area.

Signs.

17 CONFINED SPACE
17.1.1 Standard
No one is to enter or work in any space until it has been assessed, tested and determined to be
safe and a permit is issued by the main contractor in compliance with site Procedure –Confined
Space.

17.1.2 Requirement
A confined space is an enclosed or partially enclosed space that -
Is not designed primarily as a place of work. and
has a restricted means of entry and exit and is, or is likely to be, entered by a person to work.
And is, or is intended to be at atmospheric pressure while anyone is in the space and
contains, or is likely to contain, any of the following –

an atmosphere which contains potentially harmful levels of contaminant.


an atmosphere that does not have a safe oxygen level.
anything that could cause engulfment.

No one is to enter or work in any space until assessed and if determined to be a Confined
Space, a permit is issued by main contractor per the “Confined Space” Procedure. Main
contractor/Hydroserv will develop a procedure for the particular case and provide adequate
training for those workers involved. The Confined Space Procedure is to be rigidly adhered to.

18 AUDITING AND MEASUREMENT


18.1 Regulatory Compliance
18.1.1 Standard
All dewatering activities associated with the contract will be audited by Hydroserv to
determine the degree of regulatory compliance. These audits will be conducted within three (3)
months of the commencement of the contract, or at other agreed periods appropriate to the
contract.
18.1.2 Requirement
Hydroserv will be required to participate in regulatory compliance audits.

18.2 Safety Management System Audits


18.2.1 Standard
Safety Management System Audits by Hydroserv shall be undertaken to assess the pro-active
implementation of their own Health and Safety Management Plans.
18.2.2 Requirement
Hydroserv shall undertake an audit of the Health and Safety Management Plan at least once.

Hydroserv sub -contractors may use their own systems and formats however Hydroserv will
assess the suitability of these systems prior to their use.

All such audit results are to be forwarded to the Hydroserv Safety Manager. Hydroserv will
pass onto main contractor an overview report.

19. HSE PLAN INTERESTED PARTIES


Due to their effect or potential effect on the Hydroserv’s ability to consistently provide products

Hydroserv has envisioned the need for integrated management system to protect its own interests
and the interests of following stakeholders and interested parties:
a) Customers and/or Subscribers – Who need confidence in the services offered by Hydroserv.
b) Legal and Regulatory Authorities – Who impose legislation and/or directives, in order to ensure
availability of services, fair competition and privacy protection.
c) Hydroserv Employees – Who strive to meet their obligations to the customers and the public.
d) Hydroserv Management – Who ensure that the business objectives are fulfilled, the overall
posture of Hydroserv highlights quality, health & safety as a culture, the vendors and customers
feel a goodwill and comfort with the services provided and that Hydroserv complies with all the
regulation and laws.
e) Business Partners – Who want theirs and Hydroserv’s business objectives to be fulfilled.
f) Vendors, Suppliers and Contractors – Who demand that appropriate level of standards are
enforced to ensure that confidentiality of their service levels, terms & conditions, prices, and
other information is maintained.
The interested parties with any specific requirements are listed down in the same document
mentioned in List of Interested Parties “HYDRO/MGT/FR-35” & Organisational Risk &
Opportunity Register “HYDRO/MGT/FR-31”.
The specific requirements of these interested parties are monitored and reviewed through MRM.
