NOTE: This form shall be properly filled out by the unit owner/resident or the hired contractor and shall be approved by the Village Manager before any
repair or renovation works in the unit. Please take note of the following guidelines:

1. Work should be done only from MONDAYS thru FRIDAYS (8:00 a.m. to 12:00 n.n./1:00 p.m. to 5:00 p.m. while on SATURDAYS, from 8:00 a.m. to 12:00 n.n. only.
2. Workers must be confined in the unit/area where they are working. Anyone caught loitering to other area of the premises will be reprimanded.
3. Workers must be properly dressed. Those wearing sando, short pants and slippers will be prohibited from entering the building.
4. WORKERS ARE PROHIBITED FROM EATING AND SMOKING INSIDE THE UNIT, ANYONE CAUGHT WILL ALSO BE PENALIZED.
5. THE CONTRACTORS ARE RESPONSIBLE TO ENSURE THAT GARBAGE AND DEBRIS FROM WORKING AREA ARE DISPOSED PROPERLY, WORKING
AREA INCLUDING THE PERIMETER SHOULD BE KEPT CLEAN AND IN ORDER AT ALL TIMES.ANYONE CAUGHT LEAVING THE AREA UNTIDY WILL BE
PENALIZED.
6. The unit owner/resident is responsible in ensuring that workers abide by the PPHLHAI Rules and Regulations.
