Detailed Account of Accident / Incident:
On April 18, 2025 around 0744 hrs., a fire incident occurred at SSRMCC Electrical Room. Where the capacitor bank installed
caught into fire, causing property damage on the capacitor bank system and affecting the insulator of the electrical room.
Around 0744hrs roving Security Guard of HDJV S/G Junrex Dimpaso observed SSRMCC electrical room emitting black smoke
which he immediately raises the alarm. SSRMCC personnel (watchman) immediately responded in conducting firefighting. HDJV
Safety Officers and Mr. Muyong Lee HDJV Safety Manager also responded in firefighting. Fire extinguishers were available at vicinity
which help in extinguished the fire. The fire out was declared around 0755 hrs. fortunately no personnel injured and only the
capacitor bank was damaged due to the incident.
As per investigation, the fire started at capacitor bank system which caused by overheating, damaging the whole capacitor
bank system and roofing insulator. Last April 13, 2025 (Sunday) the capacitor bank was installed in the SSRMCC electrical room as
compliance to the suggestion by HDJV to provide/installed Capacitor Bank to improve power factor, which reduces energy losses,
and to provide voltage support, also to maintains stable voltage levels. The requirements were for the overall improvement and
efficiency of the power system and reduce electrical cost. The supplier installed the capacitor bank already noticed heat
accumulation at electrical room, which could affect the performance of the system. Instructions were given for additional air
conditioning unit to accommodate the proper temperature in the electrical room.
Upon the instructions or recommendations by supplier SSRMCC already requested for the purchase of air conditioning unit to
improve the room temperature. For the meantime electrical room remain open to had ventilation and avoid accumulating heat on
the room. Also, instructions came from Mr. Sam Boholst SSRMCC Maintenance Supervisor last April 15, 2025, advise for the
shutdown of the capacitor bank due to high temp. Unfortunately, the instructions were not executed, and provision of another air-
conditioning unit was not immediately provided causing the fire incident. Investigation conducted with capacitor bank supplier that
the system had no provision of breaker for shutting down the equipment it was design only for the electrical load tripping or
malfunction. Breaker for the heat detection was auxiliary for the equipment.
After the incident on the following day April 19, 2025 TENEBRAS CORPORATION conduct a 3rd party inspection for the Turn Ratio
Test, Winding Resistance Test, Insulation Resistance test and Supervision until energization of the transformer witnessed by Sir
Melvin of HDJV Electrical Supervisor. The 3rd party required for provision of spare transformer, proper room for capacitor bank
system, alarm system for heat accumulation and air-conditioning unit to ensure room ventilated. The batching plant operation
resume using the previous settings of electrical system after the 3rd party inspection.

2. Instructions were not followed by the assigned personnel instead. As per Mr. Sam Boholst,
instructions came from HDJV to continue the capacitor bank to observe its functions, which
later he didn’t shutdown the unit.
1. Immediate firefighting of the fire incident by HDJV Security, SSRMCC Watchman and HDJV
Safety Officers and Manger.
2. Conduct housekeeping on electrical room due to fire incident.
3. Replenish of fire extinguisher used during firefighting.
4. Request for additional CO2 fire extinguisher for electrical room and equipment.
5. Re-design of electrical room to separate the transformers, panel boards and capacitor bank

to avoid congestion, acquire proper ventilation and mobility on the electrical room.
6. Replacement of fire-retardant materials for electrical room, especially on the roofings and
provision of inlet for air ventilation.
7. Provision of alarm and breaker system for heat detection.
