II. Proponents
This project is headed by Group 3, Section D, of 3rd year students in the Bachelor
of Elementary Education program at Data Center College of the Philippines.
III. Rationale
Malnutrition is a significant issue affecting children’s health and development, almost
Filipino are suffering from poverty, which has an impact on children’s health. This program
titled “Empower Plates, Empower Lives: Enhancing Children’s Potential with Healthy
Meals” aims to provide nutritious meals to the children’s from Brgy. #23 Sarrat, I.N ensuring
they receive the necessary nutrients for proper growth and cognitive development. Also,
assisting them in achieving a better future and most especially beyond serving nutritious
meals to children, this feeding program will benefit them. As it encourages them to eat
healthy and nutritious will be monitored regularly to see if there is any improvement as well
as development. By nourishing their bodies, empowering their minds, we hope to create a
brighter and healthier future for all children in the community.
The community service, a feeding program titled “Empower Plates, Empower Lives:
Enhancing Children’s Potential with Healthy Meals”, is well-implemented, explain the
importance of the said activity and discuss the benefits of preventing underweight. Generally,
the expected outcomes include improved nutrition, health, and well-being of the children by
providing them vitamins to prevent malnutrition. After the program, we will conduct a
follow-up weighing schedule to determine progress among the children in Barangay #23,
Sarrat, Ilocos Norte. We ensure that the activity we undertake is effective and efficient for all
the children in Barangay #23, Sarrat, Ilocos Norte, from the start date of implementation, July
1 to 31, 2024.

VI. Venue/Beneficiaries
The venue of the said project is in Brgy. #23, Sarrat, Ilocos Norte. The
beneficiaries are children who are underweight children.

XI. Monitoring
With the help of the Brgy. Officials and/or volunteers from Brgy. 23 Sarrat, I.N. we
will conduct regular assessment, health check-up and supervision to ensure its effectiveness.
This includes tracking of attendance, collecting feedback from participants, as well as
adjusting the program as needed to address any challenges or improve outcomes.
XII. Assessment
The implementation of the feeding program has brought about significant changes in
the health and overall well-being of the beneficiaries. The program has successfully
addressed malnutrition and hunger, improving the physical health of the children by
continuously giving them vitamins to improve and make their body healthier. The program
has effectively addressed issues of malnutrition, leading to enhanced physical growth and
cognitive development, particularly in Barangay #23 Sarrat, Ilocos Norte.
XIII. Evaluation
The feeding program in Brgy. 23, Sarrat, Ilocos Norte, revealed positive outcomes. It
aimed to improve the health of children in the community, specifically focusing on their
height and weight. The organized management of the program led the children to have
progress in their health. The feeding program titled “Empower Plates, Empower Lives:
Enhancing Children’s Potential with Healthy Meals,” it was implemented from July 1 to July
31, 2024, in Brgy. 23, Sarrat, Ilocos Norte, and proved successful and working well.
