AFFIDAVIT OF HEIRSHIP

WITH SPECIAL POWER OF ATTORNEY

WE, JOSEPH R. LOYLOY AND MARLAN R. LOYLOY, all of legal ages,


Filipinos, with residence and postal address at Banilad, Dumaguete City, Negros
Oriental, Philippines, after being duly sworn to in accordance with law, depose and
state:

That we are all legitimate children and compulsory heirs of PURIFICACION


REBUYAS LOYLOY, who died intestate in Banilad, Dumaguete City on October 15,
2021;

That our deceased mother was a member of the Office of the Senior Citizen’s
Affair (OSCA) Dumaguete Chapter and was eligible for the Unconditional Cash Transfer
(UCT) from the said organization;

That pursuant to Section 1 of Rule 74 of the Revised Rules of Court, WE,


JOSEPH R. LOYLOY AND MARLAN R. LOYLOY, hereby adjudicate unto ourselves
the said UCT from OSCA;

That due to immobility, WE, JOSEPH R. LOYLOY AND MARLAN R. LOYLOY,


do hereby name, constitute and appoint, JOMAR M. LOYLOY to be our true, lawful and
sufficient Attorney-In-Fact, for us and in our name, place and stead, to do and perform
the following things, to wit:

1. To process, sign, claim the Unconditional Cash Transfer (UCT) of our late mother,
PURIFICACION REBUYAS LOYLOY from OSCA-Dumaguete;

2. To claim and change the initial PIN of DSWD Cash Card;

3. To do such any other act or thing that maybe required necessary or incidents to carry
out effectively any and the purpose for which this authority is hereby given.

HEREBYGIVING AND GRANTING unto said Attorney-In-Fact FULL powers and


authority to do and perform all and every act requisite or necessary to carry into effect
the foregoing authority, as fully intents and purposes as we might or could lawfully do, if
personally present with full power of substitution and revocation, and hereby ratifying
and confirming all that our said Attorney-In-Fact or our substitute shall lawfully do or
cause to be done by virtue hereof.
