IVF Calculation

The document outlines the principles and methods of intravenous (IV) fluid therapy, including types of IV infusions and nursing responsibilities. It details the calculation methods for determining IV fluid rates and provides examples for practice. Key components include understanding IV sets, flow rates, and the administration of various medications through IV routes.

CALCULATION OF

IV FLUIDS

Belen G. De los Trinos, RN, RM, USRN, MAN

Intravenous fluid therapy is used to
administer fluids that contain water,
dextrose, electrolytes, vitamins, and
drugs
 There are an increasing number of
drugs administered by the IV route for
direct absorption and fast action
 Some drugs are given by IV push
(bolus), some drugs are given diluted
because they irritate the veins, and
some are given in a large volume over a
period of time ( 4-8 hrs)
2 METHODS OF ADMINISTERING IV FLUIDS
AND DRUGS

• Continuous IV infusion
- Replaces fluid loss
- Maintains fluid balance
- Serves as a vehicle for drug
administration
• Intermittent IV infusion
- Primarily used to give IV drugs
NURSING FUNCTIONS AND RESPONSIBILITIES

 Knowing IV sets and their drop factors

 Calculating IV flow rates
 Mixing and diluting drugs in IV fluids
 Gathering equipment
 Knowing the drugs and the expected
and untoward reactions
 Assessment of the client for
effectiveness and untoward effects of
the therapy
 Assessment of the IV site
 IV sets are used for the controlled
infusion of medications over long
periods of time.
 IV sets are used to connect the
medication to the needle inserted into
the patient
 IV extension sets are also used to
extend IV lines without risk of
contamination
INTRAVENOUS SETS

 Macrodrip – a set that

delivers large drops
per mL (10 – 20
gtt/mL)

 Microdrip (minidrip)
– small drops / mL (60
gtt/mL)
microset

macroset
 IV fluids are sometimes given at a slow
rate to keep vein open (KVO)
- Suspected or potential emergency
situation for rapid administration of fluids
and drugs
- Need an open line to give IV drugs
at specified hours
- KVO is regulated to deliver 10
mL/hour
CALCULATING IV FLUID RATES
(METHODS)

Method I: Three Step

a) amount of solution = milliliters/hour(mL/hr)

hours to administer

b) milliliters/hour = milliliters/minute (mL/min)

60 minutes

c) milliliters/minute x drops/mL of IV set = drops/minute

(gtt/min)
Method II: Two-Step

a) Amount of fluid__ = milliliters/hour (mL/hr)

hours to administer

b) milliliters/hour x drops/milliliter (IV set) = drops/min

60 minutes
Method III: One-Step

amount of fluid x drops/mL (IV set) = drops/min (gtts/min)

hrs to administer x minutes/hour (60)
USING ANY METHOD, CALCULATE THE FOLLOWING

1. D5LRs 1,000 mL to infuse in 8 hours. The drop factor is

14. The IV is to infuse at _____
2. D5NSS 1,000 mL to be infused over a period of 12 hours.
Drop factor is 20. How many milliliters are infused each
hour? How many drops/min?
3. D5W 1,000 mL to infuse in 12 hrs. The drop factor is 15.
The IV is to infuse at _____ gtt/min.
4. A 5-year old child is to receive 400 mL of IV fluid over 24
hours. How many milliliters are infused each hour? How
many drops/min?
5. The physician orders a continuous IV infusion to run at
90mL/hr. How many milliliters will be infused in 24 hours?
