1. INTRODUCTION
Insert Your Company was contracted to develop the insert project at insert address.
Work undertaken by Insert Your Company included:

2. SUMMARY
The work undertaken by Insert Your Company on the project proved to be a valued
experience and opportunity for all involved. As Insert Your Company reflects on what
was done well and what could be improved upon, the standard set and expected
will pave the way for how processes and procedures will be implemented on the next
Insert Your Company project.
Appreciation is extended to all who assisted Insert Your Company in meeting or
exceeding safety standards.

3. PROJECT SAFETY PERFORMANCE


Insert Your Company had a responsibility and was accountable with providing safe
processes, practices, structures, equipment, education and training, to enable
employees and contractors to undertake their work free from the risk of workplace
injury and illness.
Insert Your Company management implemented a compliant safety management
system with the cooperation and consultation of its employees and contractors,
which enabled a safe and diverse range of services to be provided to the insert
project.
Insert Your Company worked consistently within the safety management system.
The Corporate support by Insert Your Company management has been central to the
success of safety performance and has included the following:
• Site visits by Corporate Representatives to monitor and guide the site
management team.
• The continued enhancement of policies and procedures to reflect the current,
and evolving work processes at the insert project.
• Closely monitoring all safety reports to identify improvement opportunities.
Regular meetings were held to inform insert your client of safety performance and to
classify statistical data; this assisted with identifying incident trends and allowing
resources to be channelled into problem areas.

4. PROJECT HEALTH AND SAFETY


Insert Your Company undertook a firm commitment upon commencement of the
contract to ensure compliance with the safety standards and expectations.

Compliance was achieved by implementing a range of processes, such as weekly
loss prevention inspections, take 5’s and hazard observations, to assist with
compliance and continually improve workplace safety performance. These
processes and other safety lead and lag performance indicators were recorded
throughout the project.

4.1. Compliance with Project Key Performance Indicators


Key performance indicators were recorded to ensure high levels of compliance were
met on an ongoing basis. This strategy also provided an effective means of gaining
information from the workforce on the hazards and risks they may be exposed to when
carrying out their day to day activities.
The key performance indicators monitored and reported on throughout the project
were:
• Safety rules.
• Behaviour observations.
• Loss prevention inspections.
• Incident reporting.
• LTIFR’s.
• MTIFR’s.
• Add or delete the above, as applicable.

4.2. Safety Rules


The safety rules adhered to through Insert Your Company’s safety procedures and job
hazard analyses were:
• Electrical isolation.
• Working at heights.
• Lifting operations.
• Driving Safely.
• Add or delete the above, as applicable.
Insert Your Company took steps to educate and train workers on the safety rule
obligations; initially upon commencement of employment at the safety induction and
then ongoing at toolbox talks and safety training sessions.

5. BEHAVIOUR BASED SAFETY PROGRAM


Insert your Company’s behavioural based safety observations proved to be effective
and practical ways to reduce incidents and injuries and improve communication and
awareness around health and safety.
Insert Your Company’s behavioural based safety program was an observation and
education program supporting risk control measures in the workplace through on the
job behavioural interactions with workers.
The behaviour-based safety program also ensured visible leadership through direct
management and supervisory interactions with workers, based on safety.
