PRIVACY STATEMENT
Central Mindanao University is committed to maintaining the accuracy, confidentiality, integrity and security of your
personal information. As part of this commitment, our privacy policy governs our actions as they relate to collection, use, disclosure,
retention and disposal of Personal Information as implemented by the Data Privacy Act of 2012 or otherwise known as RA 10173.
Personal Information will generally be collected directly from you through the use of any of our standard forms whether in
manual or electronic in format. Furthermore, we will not disclose your personal information which you provide without your permission.
We shall implement reasonable and appropriate organizational, physical and technical security measures for the protection of personal
data which we collected.
Personal Information which we collected from you will be accessed by authorized personnel only. All information will be
stored in a database (for email & softcopy) and at the Office of the University Registrar (OUR) Records Room (for printed
copy/hardcopy) after the purpose is acted upon.
Personal data will be retained only as long as necessary to fulfill the declared, specified, and legitimate purposes stated
herein, or as required by law. Once retention is no longer necessary, records will be securely disposed of through appropriate physical
or digital means based on the Policy and Guidelines in the Disposal of Personal Information and the CMU Records Disposition

PRIVACY CONSENT
I, ___________________________, ________ years old, and a __________________ (course & year) student of Central
Mindanao University hereby agree and consent that this academic institution may collect, use, disclose and process my personal
information set out in this form and/ or provided by me in any of the following purposes:
(A) Processing my application for enrolment;
(B) tracking my academic performance;
(B1) by my Parents (Signature over printed name) ____________________________or
(B2) By my Guardian (Signature over printed name) ___________________________;
(C) keeping/maintaining a copy of my academic records;
(D) generate my Academic Records and documents as per my request;
(E) disclosure of my personal information to agencies verifying my records;
(F) submission of reports to CHED, AACCUP Accreditation, PASUC, QS Stars Accreditation, and ISO Evaluation;
(G) research studies and extension of faculty and students;
(H) processing of insurance of the Office of Student Affair Services;
(I) job fair of the Office of Admission, Scholarship and Placement;
(J) tracer study of the Office of the Alumni Relations and Linkages and respective colleges and departments;
(K) record keeping, monitoring, budget allocation planning, blazer and election purposes of USSCO and SSC;
For item (G), research studies and extension activities shall follow ethical standards and applicable data protection
principles. Personal data used for research will be anonymized or aggregated whenever appropriate to protect the identity of the data
subject.
For item (K), the use of personal data for student organization activities shall be limited to the minimum necessary and
subject to institutional policies and applicable privacy safeguards.
I acknowledge that my personal information may/will be disclosed by Central Mindanao University to its third-party service
providers and to other government agencies as required by law. I am likewise aware and fully understand my rights to privacy which
are embodied in the Data Privacy Act of 2012 (Chapter IV, Section 16) and other relevant privacy policies and procedures.
These rights include the right to be informed, to object, to access, to correct, to data portability, to file a complaint, and to
damages, among others, as provided under Section 16 of RA 10173.
Furthermore, I hereby attest that by signing below I represent that I am the Data Subject and that I have read and understood
all of the above statement.
If I am below eighteen (18) years old, this form shall be signed and consented to by my parent or legal guardian in
accordance with the Data Privacy Act and applicable laws.
By affixing my signature, I give my freely given, informed, and explicit consent for the University to collect and process my
personal data for the purposes declared above.
