PREFACE
Dear Nursing Student:
Nursing is an exciting and dynamic profession! It is a timeless profession that embraces the holistic nature of
caring. To effectively address the health care needs of patients today, the professional nurse needs a strong
knowledge base, practical clinical experience, critical thinking and clinical reasoning skills, and a commitment
to continuous learning. The health care environment is changing, and new technological advances are shaping
the work setting. Technology is not only a part of nursing practice with the incorporation of electronic medical
records, but also a resource for obtaining continuous information. Used judiciously, technology can enhance
the knowledge base and facilitate research activities that support evidence-based practice.
As a student, you will become familiar with the work done by the Quality and Safety Education for Nurses
(QSEN) project in promoting the nursing core competencies of safety, evidence-based practice, teamwork and
collaboration, patient-centered care, quality improvement, and informatics to improve the delivery of care. The
clinical situations presented here will assist you to identify how one or more of these competencies are applied
in clinical practice.
This manual is divided into seven sections. Section One focuses on building your knowledge base and applying
this knowledge to short clinically based patient care situations. The clinical situations in Section One have
three purposes: (1) to build your knowledge base and reinforce fundamental concepts and principles, (2) to
demonstrate how learned knowledge is applied to clinical situations, and (3) to use critical thinking and clinical
reasoning skills as you apply the Critical Thinking Model to the clinical situations. Remember that knowledge
is fundamental to effectively using critical thinking skills and in promoting clinical reasoning. So, get actively
involved in the knowledge building learning activities.
Section Two presents common clinical situations that will help you prioritize selected nursing interventions
and make clinical decisions based on sound clinical reasoning. This section presents progressive clinical situa-
tions or unfolding clinical situations that allow you to see how nurses need to constantly assess and reevaluate
patient care situations. Actively discussing the clinical situation with your peers and with your instructor pro-
vides an opportunity to hear other points of view, establish sound rationales, and reflect on your clinical deci-
sions. The nursing process is included in this section to assist you to apply the components of assessment,
nursing diagnosis, planning, and implementation.
Section Three presents clinical situations using the current handoff report format. You are asked to analyze
and interpret the data based on the presenting clinical situation. Flow charts in the form of the patient’s
Nursing Care Rand/Kardex, Medication Record, Intake and Output Record, Nursing Notes, and History and
Physical are provided to assist you in gathering further data. This activity helps you to (1) focus on gathering
the data, both from the report and the flow charts; and (2) make relevant connections and synthesize the data.
As in Section Two, unfolding clinical situations provide you with the opportunity to experience how patient
situations change and thus require the nurse to apply critical thinking and clinical reasoning skills that promote
the nursing competencies of safety and patient-centered care.
Section Four focuses on the development of management and leadership skills. As you begin to work with
peers and to delegate to staff, work-related issues and personnel situations will arise. As with the patient care
clinical situations, it is important to apply critical thinking skills as you work through the various clinical situa-
tions. Take time to discuss the clinical situation with your peers and focus on your management and leadership
skills. Do you have a preferred style of management or leadership?
Section Five provides additional test questions for practice in selecting the most appropriate answer and
making nursing decisions based on the clinical situations. You will draw from your knowledge base and your
clinical experience as a nursing student to answer the test questions. Review and discuss your rationales, because
this will continue to reinforce your knowledge base.
Section Six presents situations that assist you in beginning to apply leadership and delegation skills. The
clinical situations offer you the opportunity to discuss the scope of practice of the registered nurse, the licensed

vocational nurse, and unlicensed personnel as it relates to the situation. In addition, you will experience solving
realistic work-related issues encountered in clinical practice.
Section Seven provides you the opportunity to use reference books and the Internet to research drugs while
learning their side effects or adverse effects. Based on your research findings, it is important for you to compare
and contrast drug information and further relate this information to patient teaching and follow-up care.
In addition, the following icon can be found on selected clinical situations. This icon indicates that addi-
tional information can be accessed at the Evolve website at http://evolve.elsevier.com/castillo/thinking. For exam-
ple, in Section One, under the Respiratory clinical situation, after accessing the web link you will hear audible
breath sounds, and under the Pain Assesment clinical situation, you will be able to access various pain assess-
ment scales in order to complete the clinical situation. The Delegation/Supervision icon highlights selected
test questions and clinical situations that require you to consider the functions and duties of licensed and
unlicensed personnel and make a decision that promotes patient safety. The Integrated Learning Activity icon
fosters the use of critical thinking and clinical reasoning skills. The Integrated Learning Activities promote
discussion and consideration of patient status in selected unfolding clinical situations that require you to make
clinical decisions.
Finally, as a beginning nursing student, you are encouraged to use the Critical Thinking Model to develop a
process for working through patient care situations. The Critical Thinking Model will help you to apply critical
thinking and clinical reasoning skills to make sound decisions. I hope that your passion for learning and for the
nursing profession is enhanced by using these clinical situations!
