Foreword

Purpose of this document is providing applicants for the issue of an initial AOC with an overview
of the application process, guidelines on required documents/actions, information on
relevant/useful documents to be consulted and instructions on the use of the Centrik platform.

It is understood that the application for the AOC and Part-CAMO approval shall be submitted
only after TM CAD has determined the satisfaction of the preliminary eligibility criteria for the
organisation to be holder of an Air Operating Licence in terms of EC Regulation 1008/2008 and
the Principal Place of Business requirements.

It is imperative that the organisation identifies a specific aircraft MSN to ensure that the
documentation can be assessed for congruence with the specific aircraft.

This document is part of the pre-application package. All forms, checklists and notices recalled
in this document are available on TM CAD website and/or in Centrik.

Before proceeding with the submission of the applications, the organisation shall provide a draft
Schedule of Events (ref. paragraph 1.) and request access to Centrik (ref. paragraph 3.).

1 Schedule of events (SoE)

In order to complete the pre-application phase and be able to submit the applications, the
organisation has to return to TM CAD a completed Schedule of Event (form CAD 315). This is a key
document which lists items, activities, programs and aircraft and/or facility acquisitions that must
be accomplished or made ready for TM CAD inspection before certification. Through the Schedule
of events the organisation provides a draft project plan. This plan is not “written in stone” and is
subject to obvious adjustments in the course of the process; however, it still provides target dates
for the project milestones which both teams (TM CAD and the organisation) will aim to achieve.

The SoE is an excel file which consists of two sheets: the first one is intended for the “classic” AOC
with CAMO approval (with or without CAM subcontractor), while the second is intended for AOC
which contracts a CAMO within the same business group of AOCs (through an Appendix I
agreement). The organisation shall complete the sheet that is applicable to their case.

The following should be taken in consideration when defining the dates of each step:

 The formal application meeting is normally held within 2 weeks from the submission of AOC
and CAMO application (with organisation manuals, related checklists and NPs document – ref.
paragraph XX for information on the minimum attachment to start processing the application).

 CAD comments on the manuals are normally received within 1 month from the formal
acceptance of the AOC application (which happens within 1-2 days from the formal application
meeting and based on its results).

 TM CAD audits (section E.) can be performed after the manuals are at an acceptable stage and
after having received the pre-audit reports performed by the organisation (section D.)

 CofA application and AMP need to be submitted at least 1 month before the intended date of
issue of the certificate

 The dates of issue of the certificates (in chronological order: 1. CAMO approval, 2. CofR, 3.
CofA, 4. AOL, AOC & OPS Specs) would represent the target dates for the successful
completion of the process.

2 ICAO 3-Letter & RT call sign

Application for an ICAO 3 letter designator code and Radio call sign is done through an online
system whereby one has to propose maximum of three options for each in order of
preference. The operator is required to buy a pin from ICAO, and the process is explained in detail

Once ICAO confirms that application has been received, CAD has to validate that the operator is
known to same authority. We suggest that a request for the 3-letter code and Radio call sign is
done at the same time in order to avoid further processing fees. ICAO will later confirm whether
proposed 3 letter code and radio call sign have been accepted or not and advise accordingly.

3 Access to Centrik
All applications will be managed through Centrik. The organisation shall request Centrik log-in for all
required personnel (including their legal advisors, if applicable) by providing the following details:
name, position, telephone/mobile and email address.

Instructions on the Centrik Application module can be found in Appendix A.

4 AOC, AOL and Part-CAMO applications

Application forms and supporting documents shall be submitted in the respective tab (see figure
below), in particular:

- The AOL application shall be submitted in the “Initial Certification” tab.


- The AOC application shall be submitted in the “Flight Ops” tab.
- Part-CAMO, CAME, AMP, CofR and CofA (5 individual applications) shall be submitted in the
“Airworthiness Inspectorate” tab.

AOL, AOC, CAMO and CAME application shall be submitted at the same time.
AMP, CofR and CofA can also be submitted in a different moment, but anyway not later than 1
month before the intended certification date and complete with all supporting documents.

The AOL application shall be submitted attaching a completed Form 0062; this form also details
all required documents which will have to be uploaded on the Assessment tab of the application
after acceptance (ref. Appendix A – Part 2 for instructions).

The AOC application shall be submitted attaching a completed Form 0068; this form also details
all required checklists and documents to be submitted. As discussed, the specific approvals that
may be part of the initial AOC are:

- NAT-HLA
- RVSM
- EFB
- DG

All documentation required in Form 068 should be submitted at the time of the application. When
this is not possible, the following minimum documents will have to be submitted at the time of
the application so that TM CAD can proceed with the next step (formal application meeting):

 Operations manuals (OMA, OMB, OMC, OMD, OME/CCOM, OMM/MSM/SMS/CMM – as


applicable) and related checklists

 Nominated Persons supporting documents, including:

o Board resolution appointing the Accountable Manager as such.


o Nomination letter signed by the Accountable Manager (note: there is no need to
submit a separate letter for each NP; all NPs can be appointed in their role in the
same letter)
o CVs
o Training Certificates and qualifications (relevant to the role)
o Employment/service contract (without financial details and specifying place of
work and hours)
o Competence assessment of CAMO nominated persons (NPCA, CMM and SM)
o Manpower plan (in case of proposed NP currently occupying the same post with
another AOC)
o Non-objection letter of the Competent Authority in case of a nominated person
currently occupying the same post within an AOC not issued by TM CAD (ref.
ORO.AOC.135).
Note. TM CAD policy does not allow the acceptance of an NPCA who is currently
holding the same post in the CAM subcontracted organisation, as this would
clearly generate a conflict of responsibilities.

The remaining documents can be submitted at a slightly later stage, but, in any case as soon as
possible, to enable starting the Document Review phase post acceptance of the AOC application.

The Part-CAMO application shall be submitted attaching, as a minimum, a completed EASA Form
2.

When completing the EASA Form 2 (Form TM/CAD/0041) the organisation shall take the following
in consideration:

 Section 3 – The organisation shall enter in this section the registered and operating
address (when different), as well as the address of any secondary office.
 Section 5 – The terms of approval and scope of work shall be stated as “managing the
continuing airworthiness of manufacturer/aircraft type/engine type and extending the
ARC of the aircraft managed. No Airworthiness Review privileges”. In case of CAM
subcontracted organisation, this will also be specified, e.g. “CAM tasks subcontracted to
subcontractor’s name/subcontractor’s address”.

When the supporting documents for the appointment of the CAMO NPs (NPCA, CMM, SMM) have
been uploaded in the AOC application, it will not be necessary re-uploading the documents in the
Part-CAMO application. However, a comment shall be entered to inform the assigned
airworthiness inspector.

The following checklists/documents shall then be uploaded as part of this application:

 Part-CAMO compliance checklist (AITP-A08 Appendix 09)
 Part-CAMO Management System Assessment Tool (AITP-A08 Appendix 10)
 Internal pre-audit report showing compliance with Part-CAMO and Part-M and including
subcontractor pre-contract audit report, as applicable.
 CAMO risk register/hazard log
 CAMO audit plan

The CAME application shall be submitted attaching the following documents:

 CAME and CAME checklist


 ATLS and instructions for completion
 CAMO staff manpower plan (including subcontractor, when applicable)
 Subcontractor agreement (when applicable)
 JPM (when applicable)

When CAME Part 2 refers to an integrated manual (e.g. OMM/MSM/SMSM, etc.), this shall be
uploaded too (or a comment shall be entered to inform the airworthiness inspector that the
manual has been uploaded in the AOC application).

The AMP application shall be submitted attaching an AMP developed in accordance with the
provisions and requirements of TM CAD IAN 03A and related supporting documents (e.g.
mods/repair status, ICAs related to STCs embodied, etc.). This application does not necessarily
need to be submitted at the same time as the AOL/AOC/CAMO/CAME; however, the organisation
should take in consideration that a timely review and approval of the AMP would allow to
produce a bridging check whose evidence of completion by a Part-145 approved organisation will
be required in support of the CofA application.

The CofR application should be submitted attaching a completed CofR application form (AITP-R01
Appendix 1).
When submitting the application, the organisation shall enter a comment to inform TM CAD of
the required/desired date of issue of the certificate.

Minimum requirements:

 Evidence of aircraft ownership


 Aircraft Insurance
 Radio Station License*
 Installation of registration marks and fireproof owner’s plate
 Applicable lease agreements and due diligence documents of owner and sub-lessees
 Dereg/non-reg confirmation

* The Radio Station License has to be obtained from the Malta Communications Authority, Aircraft
Radio Station Licence (MCA/F/11-0575) (gov.mt) Payment of relevant fees is done online Online

The CofA application should be submitted attaching a completed CofA application form (AITP-C01
Appendix 1). Upon review of the application form and its Avionic Appendix, TM CAD will issue the
Mode S and ELT codes. The documentation to be submitted in support of the application slightly
varies depending on the aircraft being new, used and registered in a Member State or used and
registered in a third country, and is summarized in the following table.

*CAME and AMP are submitted with dedicated applications; however, their approval is required
before the CofA can be issued.

Please note that, subject to review and acceptance of all required documents, the CofA can only
be issued after the aircraft is on the 9H register (CofR issued).

In case of AOC which contracts a CAMO within the same business group, the Part-CAMO
application (EASA Form 2) and the CAME and AMP submissions are not required (note: the
approval of the AMP is delegated to the Authority responsible for the oversight of the CAMO).

The organisation shall submit AOL, AOC, CofR and CofA applications as previously explained and
upload the following documents as part of the AOC application:
 Nominated Person Continuing Airworthiness Contract supporting documents
 CAM agreement (Appendix I to Part-M) [see AITP-C01 Appendix 17 for a template:

 Interface procedures between the AOC and CAMO

5 Notes on organisation’s manuals and checklists


This paragraph has been developed to share those factors that, in TM CAD experience, are an
evident characteristic of low-quality submissions and uselessly slow down the process. By properly
implementing the notes below, the organisation will benefit of a faster and smoother certification
process, as well as a good reputation within TM CAD.

 All organisation manuals and exposition shall be uploaded as a pdf file with bookmarks.
 The organisation shall ensure that manuals and documents are reviewed by the CMM before
uploading to identify and correct non-compliances, errors and/or missing information. This
should ensure identification and correction of those items which, even if minor, have an impact
on the duration of the review and hence, the certifications process. Below some examples of
discrepancies which TM CAD expects to be identified and rectified by the organisation before
submission of the documentation:
o Manuals and procedures not customized to the organisation, their operations and their
aircraft
o Reference to obsolete or not applicable regulations
o Incorrect references to paragraphs within the same manual or other manuals
o Inconsistencies in procedures and responsibilities
o Structure/layout of manuals not aligned with ORO.MLR.100 and/or AMC1 CAMO.A.300,
as applicable
o Copy & paste of parts of the regulations without any description of how the
organisation intends to comply with the requirement(s). Procedure should be written
ensuring an answer to the questions: Who does what? When? How? Where? How is it
recorded?
o Missing signatures
o Incomplete or inadequate contents.
o List of Effective Pages not corresponding to the pages in the manual.

 OMs and CAME shall be accompanied by the respective checklists. The checklists provide, at the
same time, guidance for the operator on the information expected to be found in the
documents and to TM CAD inspectors when assessing the manuals. The references specified by
the organisation in the checklist shall be correct and accurate (a generic reference to a full

chapter is not acceptable when the requirement is covered in one – or more – specific sub-
paragraph). When the contents of the checklist are not found in the manual, this should be
amended to include the missing information before submission.

 Many organisations opt for an integrated Management System between the AOC and CAMO; as
a result, Part 2 of the CAME refers to other organisation manuals (e.g. SMS, MSM, OMM, etc.).
This is acceptable; however, the organisation shall ensure that the integrated manual actually
includes the Part-CAMO requirements and CAMO staff and that all points listed in the CAME
checklist in relation to the Management System are properly addressed.

Once approved, the consolidated documents shall be uploaded in Centrik, under the Documents
Repository (ref. Appendix B).

6 Security Manual
The Security Manual is dealt with by AVSEC (separate entity to TM) and the approval of this manual
is required before the issue of the AOC. You may contact [email protected] or

7 Cosmic Radiations
The applicable Maltese legislation in terms of cosmic radiations is CAP 585 - Nuclear Safety and
Radiation Protection Act https://legislation.mt/eli/cap/585/eng. Subsidiary Legislation 585.01 -
Basic Safety Standards for Ionising Radiation Regulations refers to aircrew
https://legislation.mt/eli/sl/585.1/eng.

Matters related to cosmic radiations are under the responsibility of the Radiation Protection
Commission (Tel.: +356 2799 8676). Operators are requested to send (typically by the end of April)
a report of calculated yearly exposure of each crew member.

10 Occurrence reporting
Mandatory and voluntary occurrence report can be submitted via TM CAD website. The system is linked to
Centrik (no log-in credentials are required to submit a report). Guidance on mandatory and voluntary

Appendix A - Centrik Applications module

All applications submitted to TM CAD are processed through the Centrik platform. This includes, for
example:
- Initial organisation approval (AOC, Part-CAMO, Part-145, Part-147, ATO)
- Changes to initial approval
- Aircraft certificates (CofR, CofA)
- Organisation manuals and documents approval (initial issue and subsequent amendments)
- Permits to fly
- Submission of quarterly mandatory reports, FDP Exceedances, audit plans

The Centrik platform constitutes the interface between TM CAD and the organisation and replaces the
traditional communication via email. The use of this platforms ensures transparency and traceability of
all stages of the process and related communication exchange.

Only applications submitted through Centrik shall be accepted by TM CAD.

After logging in Centrik, the Applications module can be accessed by clicking on the relevant icon
located at the bottom of the screen:

Select the tab that related to the application to be submitted (Flight Ops, in the example below) and
click on “+ Start New Application”.

A drop-down box will appear.

Select the application form, as applicable, and press Continue.

From the window that appears, select the “Application type being submitted” from the drop-down
menu and enter the “Key Identifier”.

The “Key identifier” should provide a quick description of what the application refers to and some
examples can be found in the table below:

Click on “Declaration”

Enter the contact details. The contact details should be for the person who is the focal point for the
application. All correspondence related to the application will go through this person.
Note: automatic notification emails from Centrik will go to the person submitting the application. i.e.
The email will be sent to the username of the person logged in.

Click on the “Attachments” line, add all the supporting documentation (e.g. application form, MOC –
in case of changes –) and then click on “Save”.

Once you select “Save (Draft)”, the relevant Inspectorate will receive a prompt which will allow the
acceptance and processing of the application.

Click on the “Finish and submit” line and press the button “Submit”.

The application will now be in the “Submitted” status and there will be a green tick next to the
Application tab.

REVIEW TAB

The Review tab is the tab through which all communications exchanged between the CAD and the
organisation are recorded. The comments added to this page (by pressing the button “+ Add
Comment”) replace the traditional function of emails. Attachments can be uploaded by the CAD
inspectors and/or the operator by pressing the button “Add Attachment”, which is within the
“Regulator Attachments” block.

Anytime a comment is added to this tab by the organisation or TM CAD, the other party will receive an
automatic notification email. The notification is sent to the email associated to the account that
submitted the application.

Note1: The addition of attachments does not generate notification emails; therefore, whenever new
attachments are added to the application, it is important to also add a comment, so that the Inspector
will be notified by Centrik.

Note2: The Centrik notification email is sent to the person who submitted the application. Currently
Centrik does not give the option to add multiple recipients. However, this can be bypassed by creating
an Outlook rule to automatically forward emails received from Centrik to other persons. The rule
should be set for each individual application.

In addition to viewing all communication exchanges, this tab also allows the visualisation of the
application status, which can be one of the following:

Submitted

Assessing

Signed Off

An application which has been accepted and is under evaluation will be in the status “Assessing”.

DECISION TAB

Once the assigned inspector has completed the assessment, they will indicate this via the “Decision”
page. Once the application has been signed off, an automatic notification email will be sent to the
operator. The inspector will also upload the final approval documents to this tab.

Following the approval of operators manuals, expositions or documents, the consolidated signed
approved versions shall be uploaded by the operator in the Documents Repository (ref. Appendix B).

Part 2 – Additional considerations on the AOL application

The AOL application in Centrik works in a slightly different way from the others. After acceptance of the
application by the Director General, in fact, the organisation can also access the “Assessment” tab (not
visible in the other applications).

The “Assessment” tab contains the workflow with all the steps which have to be successfully completed
for the Licence to be issued.
The organisation shall upload all documents as required in each line and under the individual step (this
can be done by dragging a file or through the use of the button “Add attachment”).
When the line requests to provide an information rather than a document (e.g. “State the type of
Aircraft, MTOW and registration marks”) this shall be entered by adding a comment in the Review tab
and specifying the number of the item (e.g. “1.2 – B737-800, MTOW 79,015 kg, 9H-XXX”).

Also in this case, it is recommended to enter a comment whenever new attachments are uploaded as
this will trigger the Centrik notification email.

Appendix B - Centrik Documents Repository

TM-CAD use Centrik as a document repository for organisation manuals and approved documents.
The use of this tool ensures the achievement of the following objectives:
1. Central Document Repository of all organisation approved manuals (OM’s, CAME, MOE, etc.) and
controlled/approved documents (AMP, contracts, etc.).
2. Expeditious transfer of documentation and consequently less traffic on emails; and, more importantly,
3. Traceability of all reviews.
Through their Centrik log-in, organisations may upload approved manuals directly onto their assigned
operator folder.

To access the document repository, the following steps need to be followed:

1. Click on the DOCUMENTS TAB

2. Select the OPERATOR DOCUMENTS tab. A folder with the operator’s name will be displayed.

3. When accessing the operator folder, the user will have the facility to create a document: The
document type is to be selected from drop-down list (eg. OMA, OMB, MEL, AMP, ATL, etc). The title
should include name of document, revision / issue number as well as date of amendment (eg: OMA
Rev 10 Oct 2021).

For each manual, the operator will be required to create a new document, i.e. separate documents
should be created for each of the document types. Furthermore, it is recommended that files
uploaded are in pdf bookmarked formats.

4. Once an operator document has been created, the user will have the facility to edit or create a new
version of the document, by clicking on the EDIT button on the right-hand side of page.

A new page will be displayed. Here the operator may use the “Edit” and “New Version” buttons in the
following scenarios:

- “EDIT” is used to change Document Type and Title in case of any amendments required;
- “NEW VERSION” is to be used whenever a new approved manual revision is to be uploaded. Once a new
version of the document is uploaded, the previous version is automatically archived.

IMPORTANT NOTE: The “DELETE” button is not to be used to replace a manual which has been
superseded by a newer revision. The “New Version” button is to be used instead. This will ensure that
older manual revisions are archived and may still be accessible if so required.

Any supplemental documents to the approved manual (e.g. training forms in case of the OM or CAM
subcontract agreements in case of the CAME) shall be attached to the document under SUPPLEMENTAL
DOCUMENTS.

Initially, access to the Operator Documents Repository will be granted to the following postholders:

- NPFO, NPCT, NPCA, CMM and SM

Other Centrik users may be granted access if so authorized by the above.

It is also important to note that the Documents Repository is to be used for approved manuals only.

Refer to OAN 12/20 for details related to the submission of applications for draft manuals with changes
requiring / not requiring prior approval after initial approval.
