SUBJECT: LETTER FOR INTERNSHIP


Dear Sir/Madam,
It is stated that The Department of Human Nutrition and Dietetics, The
Islamia University of Bahawalpur is conducting Bachelor (16 years) degree in the field of
Human Nutrition and Dietetics. As internship in any hospital is a mandatory requirement for
the acquisition of degree, it is requested that kindly provide following student a chance to
work in your esteemed organization for Two Months.

The name and registration number of the student is given under:

This internship will be helpful in enhancing the professional expertise of the student.
A positive reply is anticipated in this regard.
As per the internship policy, the following rules are to be observed during the internship
period:
1. The student must maintain at least 90% attendance throughout internship duration.

2. The student is expected to maintain professional ethics, including patient


confidentiality, respectful communication, responsibility, and integrity at all times.

3. The student must submit a comprehensive internship report, verified and signed by
the hospital supervisor, upon completion.

4. Any misconduct or violation of rules will be reported to the university and may affect
internship approval.
