COUNSELING
MUSLIMS
“Counseling Muslims is a valuable and outstanding contribution to the counseling
literature on a timely topic. For a variety of reasons, Muslims in the United States
and elsewhere have become an important ethnocultural and religious group that
counselors and psychotherapists need to understand. The editors have assembled an
outstanding group of scholars to provide a wonderful blend of scientific and clini-
cally relevant information in a handbook format. It is an amazing resource and should
become required reading in the field.”
–Frederick Leong, PhD, Director,
Consortium for Multicultural Psychology Research, Michigan State University
“During this post-9/11 period in which the Islamophobia and the fear of terrorism
pervade our society, there is a real need to understand Muslims, who represent 23%
of the world’s population. This book on counseling Muslims is an exceptional contri-
bution that helps to provide this understanding. Mental health students and profes-
sionals, as well as the general public, will find it amazingly insightful, informative,
engaging, and challenging.”
–Stanley Sue, PhD, Professor and Director,
Center for Excellence in Diversity, Palo Alto University, California
“This is an innovative collection of writings from authors who are well versed in the
field either as scholars or practitioners. Each chapter provides a depth of understand-
ing into clinically useful ways of understanding mental health practice with Muslim
patients; and the book provides most helpful guidance to practitioners, students, and
scholars alike. I find it user friendly, accessible, and practical.”
–John R. Graham, PhD, RSW, Murray Fraser Professor,
University of Calgary, Canada
“The need to address religion and spirituality in organizing and providing mental
health services is becoming increasingly evident in most western countries. But there
are few books that provide practical help for practitioners. This book provides mental
health practitioners with a comprehensive text written specifically to help improve
their services for Muslims whether through counseling and psychotherapy or more
medically orientated psychiatric treatment. It covers a wide range of topics abun-
dantly illustrated with case studies, and often includes direct practical advice. I think
this book should be essential reading for mental health practitioners who deal with
Muslim clients, but it could also interest educators and people involved in training
mental health workers.”
–Suman Fernando, Psychiatrist, Visiting Professor,
London Metropolitan University, UK
“This text teaches practitioners what words to use, techniques to implement, and how
to judge the success of interventions with Muslim clients. It is a practical, useful source
of information that represents an important contribution to the psychotherapy field.”
–Harold G. Koenig, MD, Professor, Psychiatry and Behavioral Sciences; Director,
Center for Theology and Health, Duke University Medical Center

Acknowledgments
The clinical depth and insights presented in this book would not have
been possible without our clients, who have shared their lives with us over
the years, and without our professional colleagues, who have influenced
our development as clinicians and researchers. Countless colleagues and
friends have contributed to this book by offering feedback, contacts, encour-
agement, and insights, and for that we are very grateful. We would like to
thank our publisher, George Zimmar, for his constant patience, guidance,
and support in the preparation of this book. We are moreover grateful
to the publication staff at Routledge who were supportive throughout the
process. We also appreciate the dedication and perseverance of the authors
of this handbook, who invested countless days capturing their wisdoms on
paper, even when there was very little in the published literature on which
to depend. Thank you for your openness to guidance and criticism.
Sameera is thankful to the activists, religious scholars, and leaders,
who have mentored, inspired, and exposed her to the complexities and
variations within the Muslim community. They have served as religious
resources, and provided her with the skills needed to cohesively inte-
grate her mental health, sociocultural, and religious knowledge. I am also
grateful to my colleagues, Syeda Mohammad, Hamada Hamid, Cynthia
Arfken, and Mona Amer, for the support and advice they have provided
over the years. I am indebted to my family and friends who have been
patient with my shortcomings and supported me in numerous ways; your
support has been humbling. To my children, who have sacrificed much
for this book and continue to brighten my life with their smiles, energy,
creativity, and good nature, thank you for reminding me to take breaks,
appreciate life, and for constantly pushing me to be a better human being.

Finally, to my soul mate and beloved husband—this book would not have
been possible without your continued support, encouragement, and per-
sonal sacrifices; your willingness to edit, review, and advise; as well as your
wisdom, insight, and vision.
Mona is thankful to her mentors in multicultural psychology at the
University of Toledo and Yale University School of Medicine for inspir-
ing and guiding her growth, and for never wavering in their encourage-
ment to develop this book. I recall the intellectual loneliness of my first
few years developing cultural competence models for Muslims, so words
cannot describe the appreciation I feel for the thriving network of col-
leagues—including my coeditor Sameera—who have invigorated this work
with fresh insight. Most of all, my deepest appreciation goes to my family,
whom I’ve missed dearly during the process of working on this book. You
have been extraordinarily patient and have sacrificed immeasurably in
accommodating my absences and inadequacies . . . without your unwaver-
ing support none of this would have been possible.

Foreword
The timing of this publication is extremely important in mediating the
actual and potential conflict and misunderstandings between Muslims
and non-Muslims. This book provides the first comprehensive survey
of the issues confronting clinicians or researchers attempting to under-
stand the mental health issues that may impact Muslim clients. The book
is unique in that it discusses clinical issues and interventions that have
not been given previous attention in the literature, such as counseling
for sexual and substance abuse issues, and the use of humanistic, home-
based, and community interventions. The eighteen chapters were writ-
ten by highly qualified authorities to present and discuss Muslim beliefs
within a counseling framework while incorporating the latest literature
and research on the topic. The editors and chapter authors recognized the
absence of information needed to train counselors to be more religiously
and culturally sensitive when working with Muslims, and this book was
a response to that need. This book will increase the competence among
readers working with Muslim clients.
Each chapter follows a uniform format with a balanced emphasis on
both theory and practice. Each chapter acknowledges the diversity of views
within the Muslim community while at the same time acknowledging the
uniformity in basic beliefs. In addition, this book highlights the diversity
and multicultural identities of the Muslim community living in Western
nations, and leads the reader on a narrow path avoiding both the stereo-
types of absolute uniformity and the chaos of total diversity. The result is
a book that contextualizes the world views of Muslims across the topics
likely to come up in counseling. While there are great differences between
Islam and other religions, there are also similarities. Likewise, Muslims
share many similarities but among Muslims there are also many cultural
xv
xvi • Foreword
variations. The authors acknowledge the importance of these within-
group and between-group differences. All behaviors are learned and/or
displayed in a cultural context. Consequently accurate assessment, mean-
ingful understanding, and appropriate intervention require us to consider
the cultural context of a client’s behavior before we attempt to change the
client’s behavior.
There are several main ideas in this book which the reader might con-
sider. First, the book attacks the damaging negative stereotypes of Muslims
that may be particularly hurtful to clients. Second, the authors describe
spiritual perspectives of mental health, which distinguishes their approach
from other mental health literature and may be helpful in understand-
ing the clients’ worldview and conceptualization of their illness. Third, the
book includes a balanced emphasis on theory and practice. Fourth, almost
all chapters include case studies that help to illustrate the kinds of cases
encountered and are useful in the classroom. Fifth, both within-group and
between-group differences are included to give a broad representation of
the various Muslim cultures present in Western nations. These differences
may be ethnographic, demographic, or related to social status and affilia-
tions. Sixth, clinical, medical, and educational applications are included.
The authors of this book have contributed significantly to the literature for
counselors, researchers, and educators at a time when those contributions
are extremely important.
My own experiences have helped me see the value of this book in many
ways. From 1970 to 1980 I was a counselor in the International Students
Advisor’s Office at the University of Minnesota where I had consider-
able contact with Muslim students and faculty. I taught at Nommensen
University in Sumatra, Indonesia, the University of Malaya, Malaysia, and
at Taiwan universities for a total of more than seven years abroad, with
considerable contact with the Muslim community. In addition, I have
written or co-authored 45 books and several hundred articles on multi-
cultural counseling. Through contact with providers and consumers from
other cultures including Muslims, we will succeed in making multicultur-
alism a “fourth force” to supplement psycho-dynamic, humanistic, and
behavioral systems.
In conclusion, the reader of this book can expect to become better pre-
pared to deal with issues involving the Muslim community in specific
ways. Misinformation and prejudices towards Muslims have increased
over the years. This book is the foundation for counselors, researchers, and
educators seeking common ground across cultural differences.

Preface
Do you not see how God has given the example of a good word? It
is like a good tree, whose root is firmly fixed, and whose branches
reach the sky, ever yielding its fruit in every season with the leave of
its Lord. God gives examples for mankind that they may take heed.
(Quran 14:24–25).
Much of the work of mental health practice involves conveying meaning
through words. When a practitioner’s words are rooted in cultural aware-
ness, knowledge, and sensitivity, they have the potential to stimulate posi-
tive growth like the tree described in this verse. Words can have a powerful
and far-reaching impact, even beyond the lives of the immediate clients.
Unfortunately, when working with Muslim clients, practitioners are
often unsure of what words to use, what techniques to implement, or how
to judge the success of interventions. They are mystified when trying to
understand why well-intentioned efforts do not produce the intended
results. Searching for answers or guidance can be difficult. Postgraduate
training textbooks do not include information on Muslims, and thus even
Muslim practitioners may be at a loss as to which theories or interventions
are better suited to Muslim clients. The popular media is tainted with mis-
conceptions and prejudice, and the scholarly literature is only now begin-
ning to emerge, with numerous gaping holes to fill.
The availability of culturally competent practitioners has not kept pace
with the growing number of Muslims in the West who are seeking for-
mal mental health services. This increase in service utilization can be
largely attributed to struggles coping with acculturation challenges, stress-
ors ensuing from the sociopolitical climate of the post-9/11 world, and
increasing mental health awareness. The fact that Muslims are bypassing
xvii
xviii • Preface
significant cultural stigma and shame to access mental health services
indicates not only a need for such services, but also a greater trust in men-
tal health professionals. Unfortunately, there is a lack of practical, useful
sources of information for this population.
This handbook aims to be a resource for mental health professionals in
guiding them to provide religio-culturally competent care, thereby fulfill-
ing their clients’ trust. It also compiles the latest research as a resource for
scholars. It goes beyond previous publications on this topic (which focus
mostly on introducing the reader to the Islamic faith and Muslims’ cul-
tures) by providing specific recommendations and examples for effective
mental health service provision. Authors have painstakingly synthesized
the latest research literature with their own clinical wisdoms as well as the
feedback of their clients and communities. This has produced unique and
powerful insights on the conceptualization and implementation of mental
health interventions for Muslim clients.
This book is distinct in its scope, content, and format. Previous mental
health literature conflated Muslim and Arab communities, or emphasized
heavily the experiences of immigrants and refugees. Instead, this book
offers relevance to the realities of the heterogeneous and multicultural
Muslim community, including persons born in the West, converts to Islam,
and those from smaller ethnic minorities. Moreover, unlike the previous
discourse, which has been largely limited to individual psychotherapy,
chapters in this book reflect interventions at various levels of ecological
analysis, ranging from the individual to the wider community.
Issues and interventions discussed in this book are diverse and multi-
faceted. The book introduces topics that have been virtually ignored in the
literature such as assessment interviewing, psychological testing, humanis-
tic-experiential therapies, sex therapy, substance abuse counseling, univer-
sity counseling, and community-based prevention.
The book begins by contextualizing the faith and worldviews of Muslims
by exploring Islamic beliefs and practices, religious and cultural mental
health concepts, and traditional coping and help-seeking behaviors. The
second section details various interventions and approaches that may be
effective with Muslim populations. This section reviews case formula-
tion and psychological assessment, in addition to individual, family, and
community-level models. The third section addresses the diverse needs of
Muslims in different service settings, specifically inpatient, home-based,
and college counseling centers. Fourth is a section focused on segments
of the Muslim community that are often misunderstood, namely converts,
youth, and refugees. Finally, the fifth section highlights sensitive issues
needing attention, specifically the topics of domestic violence, sexual dys-
function, and substance abuse.
Preface • xix
Readers of this book, whether they are practitioners, researchers, ser-
vice administrators, or students, will find the material straightforward and
easy to follow. Chapters integrate tables, lists, and suggested phrasing for
practitioners; these are all aimed at presenting the concepts in a useful
manner. Most distinct to this book, however, are the embedded case stud-
ies that are used by the authors to help illustrate concepts and potential
interventions. These cases challenge the reader to think more deeply about
the material as well as how to reframe psychological concepts and tech-
niques in a religiously meaningful manner.
It is the sincere hope of the editors and the authors that this book enables
mental health professionals to use the immense power of words to stimu-
late positive growth and development in the tree of humanity.
