1 Is This Neuropathic Pain?

A 66-year-old man with a 3-year history of type II
diabetes treated by metformin visits you because of
pain involving the lower limbs. His lower limb pain
appeared 18 months ago and is rated as severe on
a 0–10 numerical rating scale (7/10). He states that
his pain is aggravated by stress and contact with
sheets at night, and it causes insomnia. Pain involves
the plantar surface and dorsum of the feet, extends
to the ankles, and is bilateral and symmetric. It is
mainly described as burning, associated with electric
shocks, tingling, pins and needles, and numbness.
Neurological examination shows decreased sensation
to pinprick and pain induced by brushing of the feet,
no motor impairment, no abnormal reflexes and
normal vibration sense. Blood tests are within normal
ranges except for hyperglycemia and a high low-
density lipoprotein cholesterol level. Hemoglobin A1C
level is 10%.

T his patient with uncontrolled type II diabetes reports chronic pain at the
feet. In the context of diabetes, pain at the lower limbs may correspond
to diabetic painful neuropathy (the most common type of neuropathy in the
Western world). However, other types of pain, such as nociceptive/inflammatory
pains (e.g., ulcers, osteoarthritis, and arterial disease), are frequent in patients
with diabetes and may also involve the lower limbs (Figure 1.1; Table 1.1).

damage to non-neural tissue and is due to the activation of nociceptors. This term is designed to
contrast with neuropathic pain. The three arrows in the figure refer to increased nociceptor input

Not directly related to diabetes but frequent because of common risk factors (e.g., overweight).
To differentiate neuropathic pain in this diabetic patient from other types
of pain, the characteristics of pain are essential. The patient describes his
lower limb pain as “burning, electric shocks,” worse by “contact with
sheets,” and associated with “tingling, pins and needles and numbness.”
These are typical neuropathic symptoms. Pain aggravated by contact with
sheets suggests allodynia (pain induced by normally non-noxious stimuli),
while tingling, pins and needles, and numbness, which are generally
non-painful, correspond to paresthesia or dysesthesia (the latter refer to un-
pleasant sensations).
To assist in the diagnostic workup of neuropathic pain, you may employ
an easy-to-use screening questionnaire. These questionnaires, which have
been validated in multiple languages for routine use and are recommended by
international scientific societies, all use common sensory terms and have up
to 90% sensitivity and specificity to discriminate between neuropathic and
non-neuropathic pains. They include the DN4 (“Douleur Neuropathique
en 4 questions” also commonly called “Douleur Neuropathique 4 question-
naire”), the LANSS (“Leeds Assessment of Neuropathic Symptoms and
Signs”), and PainDETECT. DN4 and LANSS include very simple bedside
examination (using a brush and a pinprick for the area of pain), but self-
reported forms (without examination) are also validated (DN4 interview
and S-LANSS).
This patient score on his lower limb pain is 7/10 on the DN4 question-
naire (see Figure 1.2). This is higher than the cutoff score for detection of
neuropathic pain (≥ 4/10) and suggests neuropathic pain (see Chapters 4
and 6 for examples of use of PainDETECT and LANSS).

To confirm the diagnosis, you may now (1) check for a link between
pain onset and diabetes, (2) assess the exact pain area, and (3) complete the
clinical examination.
1. The patient’s pain appeared 18 months ago after his diabetes was
diagnosed. Hence, there is a temporal link between the pain and
diabetes.
2. Pain involves the plantar surface and dorsum of the feet, extends
to the ankles, and is bilateral and symmetric. This corresponds to a

BOX 1.1 Principles of Simple Bedside Sensory Examination in Patients
Suspected of Neuropathic Pain
1. Sensory examination should be conducted in the area of pain by
comparison to the closest area of normal sensitivity. In patients
with suspected sensory neuropathy, you may start from the most
distal part of feet up to the knees to detect a change in sensitivity.
2. Thermoalgesic sensitivity (conveyed by small nociceptive fibers Aδ
and C) may be tested by:
• A pinprick (or disposable needle or 100-g monofilament) to
detect decreased (or increased) pinprick sensation
• Warm and cold tubes (or 20°C and 40°C thermorollers) to detect
thermal deficits
3. Tactile/proprioceptive sensitivity (conveyed by large myelined Aβ
fibers) may be tested by:
• A brush to detect gross tactile deficit (crude touch) or abnormal
pain sensation (allodynia)
• A 10-g monofilament to detect fine tactile deficit
• A 128-Hz tuning fork to detect vibration deficit
• The search for impaired position sense of a body part to
detect proprioceptive deficit (this may be completed by gait
examination in search for proprioceptive ataxia)
“stocking” topography and is typical of the neuroanatomical area of
sensory polyneuropathy.
3. The patient has decreased sensation to pinprick and pain induced
by brushing (allodynia) in a stocking distribution in the painful
area. This is compatible with a sensory neuropathy, but other
sensory modalities may be assessed (Box 1.1 for principles of
sensory examination). The lack of impairment of vibration
sensation, motor deficit, or reflex abnormalities does not rule out
the diagnosis of sensory neuropathy.
Of note, the fact that pain is severe and associated with insomnia has no
diagnostic value.
Pain characteristics, medical history and context (diabetes), neuroana-
tomical area of pain, and findings of a sensory deficit in the painful area
orient toward the diagnostic of probable painful diabetic neuropathy (ac-
cording to an international diagnostic algorithm).
