Pursuant to the provisions of section 25.3 of the Implementing Rules and regulations Part A of RA
9184 and in accordance with the provisions of Section 5 of department Order No. 13, series of
1998, of the Department of Labor and Employment (DOLE), we hereby submit to you, as part of the
Technical Proposal of our bi for the above-mentioned contract, this Construction Health and Safety
Program which, once approved by the Department of Health, shall be implemented by us for the
said contract if it is awarded to us.
GENERAL POLICY
It is the general policy of MARKBUILT ENTERPRISES to perform work in the safest manner possible
consistent with good construction practices. To fulfill the requirements of this policy, an organized
and effective safety and Health Standards must be carried out on every project.
1. Construction Safety and Health Committee:
1.1 Project Safety and Health Program
The construction site shall have a suitable Safety and Health Program, which must
be
In accordance with rules and other orders issued by the Department of Labor and
Employment (DOLE). The project manager or his absence, the Project Engineer as
authorized by the company, shall be responsible for the compliance with this
section.
2.1 The safety plan involves the following:
a) Organization
The project safety and health committee is organized and headed by the Project
Manager. Members include the safety officer and safety representatives from
different sections of the project organization.
b) Fencing
The perimeter of the project field office staff house workers bunkhouse and motor
pool will be completely fenced with one entry/exit point for workers and another for
vehicles. Both points are controlled by security guards.
c) Security
All employees entering the project site undergo safety induction or safety
orientation. Since projects sites are considered PPE zones, workers, and employees
are provided with safety helmet, safety rubber shoes, security pass and safety
spectacles. Entrance of vehicles PPE zone is restricted only to those carrying
materials and equipment for the project. All vehicles leaving the site are strictly
checked to ensure that materials/supplies and equipment leaving the plant are
authorized.
A gate pass must be secured with the head office in coordination with the DPWH – Lanao Del
Norte 2nd District Engineering Office

A formal meeting will be conducted every month to address major safety issues.
Highlights of meeting are documented for monitoring purposes.
b) Basic Personnel Protective Equipment
It is mandatory for every person in the PPE zone to wear safety helmet, safety
rubber shoes (steel toe/steel sole) and safety spectacles workers
c. PPE Store
A store for PPE is provided next to the warehouse. The warehouse will manage the
Issuance routine inspection and replacement of damage PPE.
d) First Aid Treatment Area
A first aid treatment area is located at the field office, it will be provided with
relevant medical supplies necessary for treatment of first aid cases. Competent
persons trained in treating medical cases are in attendance. All treatments are
recorded.
e) Site Housekeeping
The basic cleaning requirement is for workers/employees to clean up the work are
at the end of the day. Project personnel are also designated to do spot cleaning to
ensure the continuity cleaning process.
f) Security Contingency Plan
The project Manager will require the security in charge to provide his
recommendation for a security contingency plan. This plan shall be evaluated and
reviewed by project manager prior to the approval and implementation.
g) Security Reports
The security Officer-in-charge shall prepare the following reports to be submitted to
project manager:
- Incident/Accident
- Weekly Visitor
- Weekly Deliveries
- Security Posting
- Security violations of employees/workers
- Other related
h) Safety Organization
The safety Manager guides a team of reasonably experienced safety crew. The
member of safety officers is based on the number of workers.
2. SPECIFIC SAFETY POLICIES
a) Tool box Meeting
Supervisors will conduct a daily toolbox meeting with employees. Safety and
cleanliness in their work area will be assessing along with work assignment.
b) Construction Plant/Machinery
All construction machinery including power tools are inspected and approved by
the Safety team before allowed into the site. Machinery that requires certification is
supported with relevant licenses and approvals. Inspection is repeated as monthly
intervals.
c) Safety Team for Various Trades
The safety officer that will help and maintain the objective of this program/plan
shall organize the safety team. They will be composed of personnel coming from
the project like mechanics, laborer, electricians, carpenters, and supervisors. They
will undergo orientation and training to be conducted by the Safety Officer.
d) Work Permit
The Safety Manager controls the issuances of work permit for hot work excavation
tout/lock out and confined space work.
e) Focused Activity
This activity is safety pre-planning by assessing the risks of every trade and work
activity. This Process involves the participation of the Project managers, safety
crew members and the supervisors of the particular work group making the
presentation.

The safety team will use statistics to track and measure safety performance. The
maintenance the following records is integral part of the safety management
system.
3. PENALTIES AND SANCTIONS
a) Penalty System
A penalty system is in place for violation of safety rules and regulations. All workers
area made aware of the system during orientation.
b) Incentive Plan
A reward system is in place for good safety performances.
4. TRAINING
In house training is arranged for employees and workers. Topics include safety and health
Policy, rules and regulation, use and care for personal protective equipment, scaffold
erection competency, managing injuries in a project site, fall protection, fire prevention
and safe operation of Machinery/equipment, excavation and trench management.
A training room will be provided complete with training aids like overhead projector, white
board, television and video.
5. WASTE DISPOSAL
Good housekeeping must be observed at all time. This includes arranging the necessary
items in good order for use, cleaning and sanitizing the workplace at the end of the day
and taking out unnecessary items.
It has been proven from experience of local construction firms that accidents can
effectively be prevented in construction sites. This can be attained through the collective
commitment of DPWH – Lanao Del Norte 2 nd District Engineering Office,
construction management firms (Work Engineer) and MARKBUILT ENTERPRISES
implementing effective safety and health program at the site.
