I. Name/Title of Activity/Program:

“From Addiction to Hope: The Role of CBDRP in Strengthening Lives within the Community”

II. Rationale

The proposal “From Addiction to Hope: The Role of CBRP in Strengthening Lives within
the Community” is based on research showing that community programs are effective in helping
people with addiction. In Ternate, Cavite, where substance use is a growing issue, this program
aims to provide support through counseling, peer mentorship, and job training. By focusing on
local needs, the CBDRP addresses not just the addiction but also the social and economic
challenges that people face, creating a safe environment for recovery.

This proposal aligns well with the goals of the Municipal Social Welfare and
Development office, which aims to improve the well-being of individuals and families in the
community. By implementing the CBDRP, the MSWD can effectively meet the urgent needs of
those struggling with addiction. The program focuses on inclusivity, reaching out to marginalized
groups to break down stigma and promote belonging. With strong local partnerships, the
CBDRP not only supports individual recovery but also helps strengthen the community, leading
to healthier and more resilient Ternate.

III. Purpose and Objective/s of Group Work Activity/Program.

The purpose of conducting the group work in the “Form Addiction to Hope” program is
to provide a supportive environment where individuals struggling with addiction can connect
with others facing similar challenges. This group approach helps promotes a sense of
belonging, reduces feelings of loneliness, and encourages participants to share their
experiences and coping strategies. Through facilitated discussions and activities, the program
aims to empower individuals to engage actively in their recovery journey and develop a
strong support network within the community.

The objectives of the group work include helping participants understand addiction
better, improving their communication and social skills, and promoting a positive group
environment. The goal is for participants to set personal recovery goals, learn practical skills
for everyday life, and build their self-esteem. At last, the goal is to equip individuals with the
tools and confidence necessary for achieving lasting recovery and successfully reintegrate
into their community.

IV. Description of the Group Work Activity/Program.

The “Addiction to Hope” program will have a small group of five members, all of whom
are working on their recovery from addiction. This small size allows for a safe space where
everyone can share their experiences and support each other effectively. The focus will be on
helping each other and building skills for recovery.
There will be seven sessions, each lasting about 60 minutes, scheduled once a week. In
these sessions, activities will include guided discussions about addiction and recovery, sharing
personal stories, exercises to build trust and communication, and workshops on coping skills
and daily life skills. These activities aim to create a sense of belonging, encourage personal
growth, and help participants on their path to lasting recovery.

V. Target Members

The target members for the “From Addiction to Hope” program will include five people
aged 30 to 52 years old, with one woman and four men. All participants must be diagnosed with
substance use disorders and be looking for support in their recovery. This group aims to provide
a safe and welcoming space for sharing experiences and connecting with each other. Members
need to commit to attending all five sessions and participating actively in discussions and
activities to create a supportive environment for everyone.

VI. Resources Needed

Human Resources:
 Facilitators/Trainers – Facilitators with experience in addiction recovery and
group therapy will be needed to guide discussions and activites. If
facilitators/trainers are not available within the agency, we can reach out to local
mental health professionals or organizations specializing in addiction recovery.
 Support Group Leaders – are individuals who can facilitate discussions, manage
group dynamics, and offer emotional support to the CBDRP involved in the
group.

 Speaker (available in the MSWD Office)
 Microphone (available in the MSWD Office)
 Laptop
 Television for Powerpoint presentation if needed
 Cartolina, Manila Paper & Pens for visual aid id televesion of laptop is not
available (available in the agency)

Financial Resources:
 Funding for the Program- Financial assistance may be required to cover
expenses such food, materials, and other necessities. This support can be
obtained through government grants, NGO funding, or community fundraising
initiatives.
 Incentives for Participation- Offering small rewards, like snacks or food packs,
may be important to motivate participants in the CBDRP to attend the sessions
consistently.
