MOORESTOWN HIGH SCHOOL JUNIOR/SENIOR PROM PROMISE

Dear Parent/Guardian & Student, In order to attend the Junior/Senior Prom, we are requiring parents/guardians and students to read, discuss and sign the guidelines below. Students attending this function with students from other school districts or people out of school are required to print their guests name on the assigned signature line. Also, the guest must provide a School I.D. card or other form of identification if asked. The guest is expected to follow and adhere to the guidelines as written. I understand that 1. Any student and/or guest attending this MHS function may be subject to all school policy and procedures as outlined by MHS Board Policy. 2. 3. Students suspected of being under the influence of alcohol or drugs while attending this function will be required to take a blood, urine test as outlined in our Drug and Alcohol policy. Students who consume alcohol or drugs before this function will not be admitted and will be required to submit to a test. If the test is positive, the student will be disciplined in accordance with school policy. Furthermore, placement on the restricted 45 day exclusion from activities will be initiated. Students must arrive at the Junior/Senior Prom before 8:00 pm and remain until at least 11:00 pm. Students arriving after 8:00 pm will not be admitted. In addition, students will NOT be permitted to leave the prom and return. This form MUST be returned prior to or at the time of ticket purchase. No tickets will be sold
