How fill out the medical form

1. Click on ‘Medical record’ (My file tab on the left). Please indicate your child’s health
insurance number, expiration date and please read the school’s health policies.

2. In case of emergency when neither parents can be reached, please add one or two
additional people to contact and save.
3. Click on the grouping (according to your child’s medical condition) and add as needed.
Please use ‘Other’ for any condition not listed above. If your child does not have a
medical condition, please continue to step 5.

4. Insert the detailed information according to the medical condition and save.
5. *Entering your covid vaccination status* In the same menu under grouping, click
COVID Vaccination. Under ‘Health Statuses’, Select DOSE 1 - COVID VACCINATION
and enter the date in the space provided and click save. Repeat this process with the
option DOSE 2 - COVID VACCINATION, and click save. If you are not vaccinated,
please leave this section blank.
6. Click on the Doctors tab and add (mandatory).
7. Insert the doctor’s information and save.

8. Click on the ‘Food’ tab and add (not mandatory if your child does not have any
restrictions).
9. Insert information such as Vegetarian, vegan, halal, etc. and save.

10. If your child takes medication, please click on the medication tab and add.

11. Indicate all the medication your child is currently taking and save.

12. Click on the checkmark ‘I accept the school’s first aid policies’ and to finalize the
procedure click ‘Submit the medical record’.
