Preface
The objective of this book is to teach both the theory and practice of psychological assessment and testing in men-
tal health. To facilitate the latter component, this book provides a generous quantity of modifiable electronic word
processing files of all the practice forms, report forms, and test data tables presented throughout Parts I and II. The
intended audience includes those who wish to learn and practice psychological assessment and testing in the mental
health field, most especially those majoring in psychology at the advanced undergraduate and graduate levels of
training, along with those who are already fully credentialed. It is also intended for mental health therapists who do
not engage in testing themselves, but who desire such knowledge in order to be well informed when referring their
clients for assessment and testing services. Specifically, the materials in this book instruct readers how to conduct
assessments such that a proper targeted test battery is ordered, how to ask for and obtain insurance preauthorization
for the appropriate quantity of testing units (i.e., time), how to schedule appointments, and finally, how to effec-
tively score, report, interpret, integrate, and summarize the test results in addressing the diagnostic questions and
issues raised. Useful prior courses include basic statistics and research methods, and perhaps an additional course in
psychological measurement.
The book begins with an introduction chapter that provides an overview of the organization and structure of
the ensuing material. Thereafter, the book is apportioned into two large parts. Part I consists of Chapters 2 through
4, which together cover the psychological assessment and testing process from start to finish, including a visual
flowchart outlining five specific steps that constitute the entire sequence, along with practice and report forms
needed to move the case along to termination. New to this edition are four initial assessment forms designed for
early childhood, middle and late childhood, adolescence, and adulthood, rather than the dichotomous childhood
and adulthood forms presented in the first edition. These four initial assessment report forms are reviewed in detail
throughout Part I, along with several supplemental report forms that assist in transforming the original into a final
psychological assessment report with incorporated test data forms presented throughout Part II.
Part II contains the core subject matter, which is comprised of Chapters 5 through 13. Together, these chapters
present all of the tests that comprise this book’s inventory, including clear definitions of the constructs measured,
historical background (where applicable), reasons for inclusion within a test battery, development of the instrument,
test organization and structure, norm reference samples, administration, scoring, reporting of the data, and interpre-
tation of the data yield. Each instrument comes with detailed test data tables designed to be incorporated into the
final psychological assessment report, including pre-drafted interpretive excerpts following each of the data tables to
facilitate and guide the effective integration and summary of the results. In addition, interpretive flowcharts for either
single tests, or a series of similar tests, are available for download on Routledge’s e-resources website. They are each
reviewed in the book so that test reporting, interpretation, integration, and summarization can proceed efficiently
and precisely, and ultimately contribute to accurate psychodiagnostic conclusions and treatment recommendations.
Also new to this edition in Part II are chapters covering screening tests, developmental (i.e., infant) tests, pro-
jective tests that are empirically scored, and malingering tests. The latest editions of each test and/or their scoring
systems are also included, at least as of the date this book manuscript was submitted for publication on or about
November 1, 2021. This includes the MMPI–3 for ages 18 years through adulthood, which published in the fall of
2020. The reader will note that occasionally when a particular test has very recently been published into its new-
est edition, both it and the immediate predecessor is also included in this book. This is based upon the assumption
that the majority of clinicians may continue to use, or be trained upon, the prior test, this being due to such fac-
tors as greater familiarity with the previous test, the current relatively sparse independent supporting data for the
newest edition, and/or because a financial investment has been made in the prior edition and the user would prefer
to obtain more use of the test before moving to the next edition. For example, the MMPI–2–RF is also included
in the book as a companion to the newest MMPI–3. It is also instructive to observe how tests are modified into
updated editions.

Finally, a number of enhancements are available on Routledge’s e-resources website to supplement the book.
They consist of (a) test interpretive flowcharts, (b) case examples, and (c) electronic forms for actual testing practice.
The interpretive flowcharts provide a visual guide that presents a clear, logical, and efficient strategy for reporting test
scores and evincing meaning from the data yield for each major test or test series. Regarding case studies, there are
four in total, one selected for each major developmental period. For each case, a complete initial and final psycho-
logical assessment report is provided such that readers have concrete examples of how the system presented in this
book is intended to work. Finally, the electronic forms are apportioned into three classes as follows: (a) practice forms
(n = 10), (b) report forms (n = 9), and (c) test data tables (n = 91). The practice forms assist in moving a case from its
initial referral to termination (e.g., Referral for Psychological Testing; Consent for Psychological Testing Services).
The report forms provide an initial report outline for each major developmental period, along with supplemental
forms that help transform the initial into a final report (e.g., Initial Psychological Assessment-Report Adulthood
Form; Assessment and Test Battery List-Adolescence). Lastly, the test data tables are modifiable electronic versions of
the forms presented throughout Part II, which are designed to efficiently report test scores within a final psychologi-
cal assessment report, including excerpts that guide interpretation of the data (e.g., NEPSY–II Full Battery for Ages
5–16; MCMI–IV for Ages 18–85). Referral sources very much appreciate psychological testing reports that effi-
ciently incorporate the actual empirical data that contributed to the psychologist’s conclusions. These test data tables
accomplish this objective. The author is confident that this book, along with its generous quantity of forms, will
contribute to the effective learning and practice of psychological assessment and testing in the mental health field.
About the Author
John M. Spores is a professor of psychology at Purdue University Northwest (PNW) located in northwest Indiana.
He is also a practicing clinical psychologist (Indiana Psychologist License #20040638A) and a licensed attorney
(Indiana Bar #21404–64, Inactive Status, In Good Standing). In 1990, he received a Ph.D. in clinical psychology,
including a minor in developmental psychology, from Purdue University at West Lafayette, Indiana. He subse-
quently earned a J.D. degree from Valparaiso University School of Law, also in Indiana, which was conferred in
1998. He teaches a wide range of courses, including introductory and advanced statistics for psychology, research
methods in psychology, personality psychology, forensic psychology, abnormal psychology, and psychotherapy.
In practice, he specializes in psychological assessment and testing. Professor Spores lives with his loving family,
including his wife, Billie Ladas Spores, M.A., V.R.C., his son, Andrew John Spores, who is matriculating at Butler
University in Indianapolis, majoring in computer science, his daughter, Demetra Spores, who is matriculating
at Loyola University in Chicago, majoring in forensic science, and his dogs, Max (who passed), Moe, and Macy
Spores. He and his family members are active in the Greek Eastern Orthodox Christian religion.

1 Introduction
Overview
This chapter has two goals. First, it introduces the principal objective of this book and its related tasks. Second, it
provides an overview of the book’s general organization, which is specifically designed to meet the aforementioned
objective.
Principal Objective
At its most fundamental level, this book is written to instruct readers how to effectively practice psychological assess-
ment and psychological testing in the contemporary field of mental health. In order to accomplish this goal, a number
of tasks are explicated throughout the book such that they are learned and eventually mastered. These tasks are as
follows. First, a preliminary listing of the diagnostic questions and issues of a case that has been referred for testing
must be obtained. Second, an initial clinical interview must be conducted in order to further refine the diagnostic
questions, identify whether further testing is needed or required, and if the answer is in the affirmative, to select a
targeted test battery that will efficiently, yet completely, address the enumerated issues. This may require the utiliza-
tion of screening tests in order to address some or all of the presenting assessment issues, or alternatively, to bolster
the request for follow-up testing. In instances where the initial clinical assessment is sufficient, a report needs to be
proximately generated with diagnoses and recommendations so that intervention can be initiated in an efficient and
timely manner.
On the other hand, if further assessment is deemed necessary, a targeted test battery must be judiciously selected,
along with the proper psychological services billing codes and number of units needed (i.e., time). Third, preau-
thorization (PA) for the requested services and units must be obtained from insurance carriers, with a preparedness
and strategy for appeal should some or all testing units be denied. Fourth, test administration appointments must
be scheduled and completed within a coherent order and a reasonable time frame. Fifth, the garnered test scores
must be incorporated into a formal report such that the results are presented within an organized, clear, and logical
sequence of data integration, and use of an interpretive strategy in search of converging evidence, which culminates
in a final diagnostic summary that serves as an impetus for treatment planning. Finally, effective communication of
the results must be accomplished, with the ability to realize when supplementary testing is needed in order to address
any enduring unresolved issues.
There are several major themes that run through the entire book as does a motif within an artistic or literary
composition. First, the concept of efficiency is emphasized at each stage of the assessment and testing process. Sec-
ond, clarity is another idea that is applied to all of the aforesaid tasks. This is especially applicable to the many psy-
chological constructs that are measured by the tests incorporated into this book’s inventory. Briefly, a construct is an
abstract idea used to organize, interpret, and explain observed facts (e.g., intelligence, personality). Third, although
psychological assessment and testing are both thoroughly defined and described, this is foremost a pragmatic and
practice-oriented book. Therefore, the subject matter is teeming with practice forms that inform precisely how to
negotiate each step in the assessment process, along with data tables designed for each test that can be inserted into
formal psychological reports. The latter are designed to present each test’s data yield within a readily comprehensible
and organized framework, with diligently defined constructs, and narrative excerpts that assist in guiding the inter-
pretation and report generation process.
After readers have scrutinized and mastered this book’s subject matter and supporting materials, and assuming
proper supervised training, they will be fully capable and confident in practicing psychological assessment and testing
in the mental health field. Furthermore, a wealth of supplementary materials is available on the Routledge e-resource
website (see subsequently). Of note, this book incorporates a prudently selected inventory of tests that together have

proven capable of resolving innumerable assessment dilemmas most frequently encountered in contemporary mental
health. However, the knowledge gained by reading this book and utilizing it in practice will readily generalize to
other psychological tests and assessment cases. Next, the general organization of the book is presented, which is
intended to inform readers as to how the prior stated objective and its related tasks shall be accomplished.
Organization of the Book
The book begins with this introduction, followed by two broad parts and their more narrowly defined conceptually
related topics in the form of chapters (see Figure 1.1).
The latter represent the core subject matter of the book. The final grouping of items is comprised of supple-
mentary materials that have been uploaded to the e-resource portion located on the Routledge website, which are
designed to facilitate actual clinical practice, hence being consistent with the book’s pragmatic emphasis. Each is
described in more detail subsequently.
Part I: The Psychological Assessment and Testing Process: Practice Forms and Report Forms
Part I delineates a five-stage sequence of psychological assessment and testing, which begins at the point of accept-
ing a testing referral and proceeding through to case termination. To help explain the sequence, an analogy is made
to the process of scientific investigation, which begins with an important question that is in need of an answer,
and ends with publication of the results. In Figure 1.1, Chapter 2 begins the part with a succinct overview of the
process, which consists of five interrelated stages. Here, the objective is to provide a scheme or mental map of how
these stages are integrated into a coherent whole. Two complementary flowcharts afford a visual framework of the
sequence. The first highlights the specific tasks to be accomplished, while the second enumerates the associated
practice forms to be employed at each stage. These forms have been diligently designed and continually modified
over the course of several thousand cases, hence helping to ensure that they indeed facilitate actual clinical practice.
Of course, they may be readily modified to suit a practitioner’s varying needs and types of cases. Lastly, Chapter 2
serves as a prelude to Chapters 3 and 4, which together comprise Part I and describe this five-stage sequence in
significantly greater detail.
Chapter 3 proceeds from the initial referral through drafting an “Initial Psychological Assessment Report” by
means of a semi-structured clinical interview, along with selecting a targeted test battery, the proper type of testing ser-
vices and units of time, submitting any rating forms to examinees and collaterals, and scheduling the psychological
test administration appointments; that is, proceeding through the first three stages (see Figure 1.1). At this juncture,
there is a natural pause in the assessment and testing process. Therefore, it is logical to conclude Chapter 3 at this
point and subsequently transition quite readily into Chapter 4, which continues the sequence with test administra-
tion and scoring services, along with the efficient and efficacious strategy of simultaneous test interpretation and
report generation (i.e., test evaluation services).
The discussion then focuses on how the initial report evolves into a “Final Psychological Assessment Report.”
Although this concerns the manner in which some of the previous initial report sections are modified, the major
