Part I. Workplace Description
• ER Libunao Design have a On site office and Off site Office.
The on site office consist of a workplace adequate for conducting plan and work flow
chart monitoring and implementation, attached with a equipment storage and additional
barracks for construction employees. Typically build from recycle metal containers.
The onsite Office staff consist of 1 Project Manager and 1 main foreman plus workers
based on the construction stage.
The Off site office have the capacity to make architectural drawings, thru computers and
printers. The office staff consist of 2 apprentice , 2 architects
Part II. Identification of Occupational Hazards
With the Onsite Project, Majority of the Hazards come the construction related hazards
such as:
Slippery surfaces due to unfinished surfaces
Falling debris from upper floors
Hazardous chemical thru skin contact and inhalation
Protruding dowels on certain structural parts of the building
With the Off Site Office, The Hazards are at a minimal at most.
• Electrical power surges
• Fire false alarms
Part III. Current Safety Measures
In the workplace, several safety practices and policies are already in place to minimize
risks and promote compliance with RA 11058 and DO 198-18.
Onsite Project Safety Measures
Personal Protective Equipment (PPE): All workers are required to wear hard hats,
safety boots, gloves, and reflective vests. Respirators and goggles are provided for
chemical handling tasks.
Fall and Debris Protection: Safety nets, barricades, and warning signs are installed
to protect workers from falling objects and to restrict access to high-risk areas.
Housekeeping and Slippery Surface Control: Slippery and unfinished surfaces are
regularly cleaned and marked with signage. Non-slip footwear is mandated for all
site workers.
Chemical Safety Management: Hazardous chemicals are stored in labeled
containers with Material Safety Data Sheets (MSDS) accessible onsite. Workers
handling chemicals undergo safety orientation and are provided with spill kits and
first aid facilities.
Protruding Dowel Protection: Exposed rebars and dowels are fitted with safety
caps, and areas with protrusions are clearly marked with safety tape and barriers.
Off-Site Office Safety Measures
Electrical Safety: Surge protectors and circuit breakers are installed to prevent
electrical hazards. Regular maintenance checks are conducted to ensure wiring
safety.
Fire Safety and Evacuation: Fire alarms, extinguishers, and evacuation plans are
in place. Periodic fire drills are conducted to familiarize employees with
evacuation routes and procedures.
General Office Safety: Emergency exits are clearly marked and kept
unobstructed, and safety signage is posted in strategic locations.
These practices demonstrate the organization’s ongoing effort to implement preventive
measures, ensure worker safety, and comply with occupational safety standards.
Part IV. Compliance Audit Based on RA 11058 & DO 198-18
Provisions of RA 11058 Being Followed:
Mandatory use of Personal Protective Equipment (PPE) is enforced onsite (hard
hats, safety boots, gloves, goggles).
Fire safety equipment such as extinguishers, alarms, and evacuation procedures
are in place both onsite and in the office.
Regular safety orientations and trainings are conducted to ensure awareness of
hazards and preventive measures.
Hazardous chemicals are stored with proper labeling and Material Safety Data
Sheets (MSDS) are accessible.
Areas of Non-Compliance or Lapses:
Inconsistent enforcement of PPE use (some workers are observed not wearing full
gear at all times).
Housekeeping lapses, particularly slippery unfinished surfaces and protruding
dowels, pose risks when not consistently monitored.
Lack of sufficient safety signage in certain areas of the construction site.
Limited documentation and monitoring for electrical safety practices in the office.
Part V. Gaps and Non-Compliance Issues

Part VI. Recommendations for Improvement
1. Strengthen PPE Enforcement: Assign safety officers to monitor daily compliance
and impose penalties for non-compliance.
2. Enhance Safety Signage: Install visible, standardized safety signs in all high-risk
areas, including chemical storage, slippery zones, and protruding dowel areas.
3. Improve Housekeeping Practices: Implement scheduled clean-up drives and
assign dedicated housekeeping personnel to ensure safe walking surfaces.
4. Establish Stronger Monitoring Systems: Document fire drills, equipment
maintenance, and electrical inspections, ensuring records are accessible for
compliance checks.
5. Form or Strengthen the Safety Committee: Ensure active participation of workers
and supervisors in identifying hazards and proposing solutions.
