I hereby request that you cancel the internet service for the account number listed above. The account was fraudulently obtained and I am the victim of identity theft. I was contacted by a collection agency regarding this account, which I did not open. I am requesting that you close the account that was fraudulently opened in my name and cancel any charges that have been made. If any new accounts are opened in my name, please contact me. In addition, I am requesting that a fraud alert be placed on my name or account so that incidents of identity theft may be prevented in the future. Please provide written confirmation that you have taken the requested actions. You may contact me at the above address if you have any questions or need additional information.
