This assessment sheet is designed to evaluate a student’s ability to recognise
and identify the initial sounds of the letters S, A, T, P, I, and N.

Instructions:
1. Present each letter sound clearly and individually to the student.
2. Ask the student to identify the sound of each letter.
3. Mark “Sound Correct” if the student identifies the sound correctly.
4. Mark “Sound Incorrect” if the student identifies the sound incorrectly.
5. Use the “Notes” column to add any relevant observations
or comments regarding the student’s responses.
6. Use the Initial sound cards to identify the sounds using pictures.
7. Use the corresponding SATPIN letter to indicate the sound chosen.
