The spotter was conducting work under the elevated portion of the forklift when the incident occurred. To prevent future incidents, the drilling contractor will develop new safety procedures, install equipment to improve lift safety, provide training on forklift operations and risk assessment, and ensure lessons learned are applied across all rigs.
