The Costa Concordia cruise ship capsized off the coast of Italy in 2012, killing 33 people. The 114,147 ton ship was sailing close to shore as a "salute" when it hit rocks and capsized. The captain evacuated the sinking ship in a lifeboat while many passengers were still aboard. Over 1,000 crew members and nearly 3,800 passengers were on board at the time of the accident. The last body was not recovered until two years later in 2014. The main cause of the accident was determined to be the captain ordering the ship too close to shore.

Causes
◦Main Cause: The captain ordered the vessel to be steered
close to the island of Giglio as a "salute“
◦Secondary Cause: The captain left the sinking vessel in
one of the lifeboats under the excuse “I slipped off the
deck and fell in one of the lifeboats.”
Fatalities
◦ 33 people died as a result from the accident, including 32
passengers and crew and one salvage member
◦ the last body was not recovered until November 2014
