According to the National Pharmacist Workforce Survey, in 2014, 60 %
of pharmacists provided medication therapy management and 53
percent performed immunizations. Ten years earlier, only 13 percent of
pharmacists provided medication management and just 15 percent
administered vaccinations.
This shows that pharmacists develop into an integral part of the health
care team. This allows them to perform more patient care activities,
including counseling, medication management, and preventive care
screenings.

Pharmacist Collaborative Practice Agreement
(CPA)
A formal agreement in which a licensed provider makes a diagnosis,
supervises patient care, and refers the patient to a pharmacist under a
protocol that allows the pharmacist to perform specific patient care
functions.
“A 2010 study found that patient health improves significantly when
pharmacists work with doctors and other providers to manage patient
care”.

Care Services
Medication Therapy Management (MTM): A distinct service or group
of services that optimizes therapeutic outcomes for individual
patients.
