The patient was experiencing an ineffective breathing pattern due to decreased energy and respiratory muscle fatigue, as evidenced by restless behavior, nasal flaring, weak appearance, use of accessory muscles, flushed skin, warm temperature, crackles in both lung fields, and a complaint of difficulty breathing. The nursing care plan aimed to establish a normal breathing pattern within 1-2 hours through encouraging slower, deeper respirations and administering analgesics as needed to promote coughing and deeper breathing. Evaluation after 1-2 hours found the patient had established a normal, effective respiratory pattern.
