Exercise B8- Absence
Seizures
A 9 year old was bought with history of not doing well in class. on questioning the parents,
say they noticed she frequently stares into space and seems to be out of touch with her
surroundings for brief spells. The EEG was done and showed 3 cycles per second spike and
wave activity. A diagnosis of absence seizures was made. Write a suitable prescription for
this patient.
Absence seizures
Absence seizures, also known as Petit Mal seizures, are a type of Other Symptoms

neurotransmitter GABA is involved in the

Treatment
Antiepileptics are the drugs given for absence seizures. They do not cure the disorder but helps to
control and prevent all seizure activity at an acceptable level of side effects. Treatment is to be
initiated early because each seizure episode increases prospensity to further attacks.

Choosing appropriate drug group
Why Not Ethosuximide?

Has dose related side effects such as gastro-intestinal tolerance, tiredeness, mood changes, agitation,
headache, inability to concentrate. Hypersensitivity reactions like rashes, DLE and blood dyscarias
could also occurl.
Why Not Lamotrigmine?

Valproate levels are lowered by lamotrigmine. The half life of lamotrigmine is reduced to 16 hours
in patients receiving phenytoin, carbamazepine or phenobarbitone.
Why Not Benzodiazepines?

Clonazepam has adverse effects of sedation and dullness, some tolerance develops with chronic
therapy. Lack of concentration, irritability and behavioural disorders are seen in children.
It is the drug of choice for absence seizures.
Due to it's tolerability, it can also be given for myoclonic and

DRUG OF partial seizures.


It is widely used in pediatric epilepsy because of its multiple

CHOICE - mechanisms of action and acceptable safety profile.


Valproic acid is rapidly absorbed from the gastrointestinal tract,

Sodium valproate peak concentrations being attained 1 to 2 hours.


The bioavailability of valproic acid is complete and
independent of the preparation used.
Valproic acid is quickly absorbed when taken by mouth.

- SODIUM VALPROATE 200mg/5 ml syrup x 1 bottle of 60ml
Give the child ONE teaspoon full syrup 5ml twice a day
