PTERYGIUM

This document discusses pterygium, pseudopterygium, and pinguecula. Pterygium is a degenerative condition where fibrovascular tissue grows over the cornea from the conjunctiva. Pseudopterygium is an inflammatory condition caused by corneal damage that results in conjunctival adhesions to the cornea but is non-progressive. Pinguecula is a yellowish patch on the conjunctiva near the limbus that resembles fat and is a precursor to pterygium. The document provides details on the differences between pterygium and pseudopterygium in terms of etiology, location, stages, probe test results, appearance on AS-OCT, and histology.

DIFFERENTIAL DIAGNOSES

Pseudopter
Pinguecula

2
PSEUDOPTERYGIUM
• Fold of bulbar conjunctiva attached to cornea
• Adhesions of chemosed conjunctiva to marginal corneal ulcer and the adjacent lid
• Induced by cicatrizing conjunctival inflammatory overgrowth produced by
trauma, thermochemical burn or chronic conjunctivitis
• Non-progressive
• Location at any quadrant of limbus

Pseudopterygium in slit-lamp (arrow); (C) pterygium in OCT (no plane of cleavage between the lesion and corneal epithelium); (D)
Pseudopterygium in AS-OCT in a horizontal scan. Notice (arrow) an overgrowing membrane that is not attached to the underlying cornea and
there is a real plane of cleavage between it and the corneal epithelium. AS = Anterior segment.

PINGUECULA
• Yellowish triangular patch on bulbar conjunctiva near limbus, resembles fat
• Precursor of pterygium
• Due to hyaline infiltration and elastotic degeneration of the submucous tissue
• Apex is away from the cornea, affects nasal side first
• Avascular
• Stationary
• No treatment required
