What is an EHR?
• An Electronic Health Record (EHR) is an electronic version of a patient's
medical history, that is maintained by the provider over time. It may
include all the key administrative clinical data relevant to that patient's
care under a particular provider (EHR, n.d.).
• It includes:
o Vital Signs
o Past medical history
o Demographics
o Problems and progress notes
o Medication and immunizations
o Laboratory data and radiology reports (EHR, n.d.).

• EHR standards ensure the compatibility and integration of EHR systems

across healthcare settings. These standards allow the consistent and
secure sharing of patient information among providers, which enhances
the quality and continuity of care (Datavant, 2024).
• It automates access to information and streamlines the clinician's
workflow. Additionally, it supports other care-related activities through
various interfaces, such as evidence-based decision support, quality
management, and outcomes reporting (EHR, n.d.).

Negative Outcomes from the EHR
• Patient Interaction- expert panelist have concluded EHR would limit
patient encounters per day (Sines et al., 2017).

• Implementation costs- the transition from paper based to electronic

solutions have issues of cost, especially in a smaller practice with
limited resources (Sines et al., 2017).

• Learning curve- EHR necessitates the healthcare professional to

adapt to new systems and interfaces, this cause decrease in
productivity while learning the new system

• Patient Encounter would be limited due to the need to input data

into the EHR, and possibly lead to patient dissatisfaction (Sines et
al., 2017).
