Chapter 1 : Introduction
1.1 & 1.2 Introduction and Project background
• PETRONAS thru its subsidiary of PRPC UF Sdn Bhd (Client PMT) has
appoint a competent Contractor to Install and Commission the Smart
Breather System. This is inclusive to supply the accessories of Smart LANDAS GEMILANG SDN BHD
Breather, necessary auxiliaries equipment such as power cables, network
cables, DBs, cable trays, analogue or digital inputs and other related
equipment and material in constructing or installing the system.
• Landas Gemilang Sdn Bhd as a contractor shall Install and
Commission the Smart Breather system. The contractor shall
supply and install One (1) unit of MESSKO Smart Breather
MTRAB DB100 and One (1) unit MESSKO Smart Breather
MTRAB DB200T to each transformer in the PRPC UF Sdn Bhd
plant with a total number of 14 Transformer.

2.1 Construction obstacle – corrugated pipes
• In the scope of work for this project, the contractor should install the cable into the existing corrugated pipes at
the transformer for the smart breather cabling system. During the construction, the contractor uses the cable
puller to make sure the corrugated pipes can be access from both sides before the cable can be installed into
the corrugated pipes. The total of corrugated pipes involve in this project is 28 numbers consisting of 4-inch
and 6-inch pipes.
Recommend action
As the contract holder and a responsible contractor, Landas Gemilang
Sdn Bhd decided to procure and install a new corrugated pipes.
Furthermore, contractor should build a new cable trench to install the
corrugated pipes at the transformer bays. The total length of
corrugated pipes and cable trench need to be build is 84 meter. The
cable trench need to build manually because it is construct near with
the live power cable in the transformer bay.

2.2 Insufficient of material – electrical cables
• Based on the scope of work, the Organisation of Landas Gemilang Sdn Bhd should supply the power cable,
control cable, and signal cable for the project. Based on the drawing given by the Client, the contractor should
supply a total of 2,800 meters of each type of cable resulting in 8,400 meters of cable. However, the cable is
insufficient in 500 meter.
Recommend action
• Based on the table above, the total cable needed to be used for the entire project is 8,900 meters but we only
procure a total of 8,400 meters of cable. We had a shortage of 500 meters of cable. With this calculation, we
should procure an additional 20 % of the total length to ensure the cable is sufficient for the whole project.
With this we procure an additional of 1,800 meters of cable consisting of :
1. Power Cable 7C x 2.5 mm CU/XLPE/PVC/SWA/PVC: 600 meter
2. Control Cable 7C x 2.5 mm CU/PVC/PVC/SWA/PVC : 600 meter
3. Signal Cable 5P x 1.0 mm CU/PVC/OSCR/PVC : 600 meter.

2.3 Malfunction of the power supply unit
• After several months of the MESSKO Smart Breather MTRAB was installed, the customer was informed that
MTRAB® 2.5s showed no functionality despite the correct power supply. With this condition, the organisation of
Landas Gemilang Sdn Bhd as a contract holder has arranged an investigation on the customer complaint.
Recommend action
• Root-cause for the defective power supply units seems to be a material
failure of a sub-supplier. There is a noticeable concentration of failures in
the first delivery batches of the affected power supply unit, which supports
this assumption. The fact that the failures occurred with a delay also shows
that there were no quality deviations during the assembly processes or the
End-of-Line test sequences of the MTRAB® 2.5.
• An adaption of our production and/or functional testing process is therefore
not necessary. Based on the technical analysis above, Organization will
replace the 21 units of the circuit board that is the root cause of the failure
of the power supply unit of the Smart Breather. The warranty also will be
extended for another 24 months to ensure the equipment is in safe care.

2.4 Recommended Action On Quality Plan For Issue In 2.1,2.2 And 2.3
For the issue 2.1 Construction obstacle – corrugated pipes
This issue is resulted in a lack of concentration and understanding from the project management team to ensure before every
construction starts, the preparation and existing construction need to be revisit and reconfirmed. The quality form and
inspection test plan (ITP) of site visit should be developed and will be approved by the person in charge at site and client.
For the issue 2.2 Insufficient of material – electrical cable
For this issue, the organization should hire a qualified quantity surveyor to developed the estimation quantity for the
construction material. With the qualified and experience quantity surveyor, the quantity of the material can be estimate
correctly which resulting in minimal risk of insufficient construction materials. This also can be verified by using Bill of
Material (BOM) documentation which is included in the existing quality management plan of the organisation.
For the issue 2.3 Malfunction of the power supply unit
This issue can be avoided by revisit and reenforce on the of sub supplier quality management plan. During factory
acceptance test (FAT), organization can ensure the mill certificate and the internal audit of the material is within the
standards. To avoid this repetitive issue, organization request the additional warranty for the particular the material to be
extended for another 12 month from the normal of 24 month warranty.

Chapter 3 : Quality improvement for the organisation
Total quality Management or TQM strategies will focus on long-term success by enlisting every member of an organization at all
levels to produce a product or service that creates value for the customer, leading to loyalty as well as satisfaction. In the context of
organization of Landas Gemilang Sdn Bhd, the customer satisfaction will be based on the Petronas Technical Standard or PTS.

Chapter 4 : Conclusion
• In conclusion, this assignment has aligned with the objective in the early chapter as to identified
the issue for the project, to recommend an action on quality plan to avoid the issue and also to
proposed the quality improvement plan for the organization of Landas Gemilang Sdn Bhd.
• With all the objective of this assignment is answered and the proposal of using Total Quality
Management strategy to improvement the quality management of the organization, hopefully the
management of Landas Gemilang Sdn Bhd can work together in their respective departments to
ensure the company is more quality oriented and thriving in the future.
