Directions for Clinical Supervisor: Complete this evaluation during the student teacher's final week in your classroom. In a conference setting, review the evaluation with your student teacher. This final evaluation is to serve as a summary indication of the student teachers performance. Keep a copy for your file, provide a copy to the student teacher and mail the original to the University Supervisor for completion of the portion of the form that is designated for that individual. The University Supervisor will review that aspect of the evaluation in conference with the student teacher. The University Supervisor must receive the form not later than the last day of Final Exams.
