Topic: Loss Time Injury (Fracture) while working on Live Equipment

Description of Accident / Incident / Facts:


Technician was at site to attend the DG issue (Air locking) in the diesel tank. He engaged for purge out the air
from the DG tank and for the same, he started the DG. While working on the DG, he felt a jerk and accidently
his left hand came in contact with the fan of the radiator which was hardly at a distance of 150mm and
evenly not protected with machinery guard.

He sustained fracture in two of his fingers of left hand. He himself plied his 4-wheeler to the nearest hospital
and then further accompanied by the FSE and his relatives for further medical attention.

• Strict adherence of Indus 6 LSR(use of Work related PPEs)
• Enforcement on validation of equipment PM (last PM done in Jan 2020 indicating the absence of ma-
chinery guard in the equipment)
