By virtue of the powers vested in me by R.A. No. 7160 (Local Government Code of 1991) thereof
appoint you as the Barangay Tanod (CVO) of Bayambang, Infanta, Pangasinan to take effect immediately
upon approval by a majority of all members of the Sangguniang Barangay, this Barangay, and after
taking oath of office before any authorized administering officer.

You shall thereof exercise the powers and perform all the duties and functions of your position
pursuant to the aforecited Code but not beyond the expiration of my term of office unless sooner duly
removed or suspended for cause.
