Injury/ Cause of death : Nausea and became unconscious on 29.03.2021. Cardiac Arrest
on 08.04.2021 at 7:40 AM during treatment at Medanta Hospital,
Ranchi, Jharkhand.
What Happened:
On 29.03.2021, Deceased Person (DP) was in ‘B’ shift (2:00 PM to 10:00 PM) and was engaged for shifting
of rejects (by-product of clean coal) from Washery 2. During initial routine check-up, he found low diesel
level in a dumper. He tried to transfer diesel by using a plastic pipe from the tank of another dumper which
was under breakdown. During the process, he ingested some quantity of diesel. As a result, he felt nausea
and became unconscious. He was immediately taken to Tata Central Hospital, West Bokaro. After
preliminary examination, he was referred and admitted to Medanta Hospital at Ranchi, Jharkhand for further
treatment. On 08.04.2021, at about 7:40 AM, DP succumbed to a cardiac arrest at Critical Care Unit (CCU)
of Medanta Hospital.

Preliminary Findings:
• The correct Code of Practice for diesel filling through pump station was not followed by the DP.
• Half inch diameter pipe and oil measuring jar were found at the incident site.
• The pump house foreman was present at the site at the time of incident.
Immediate actions taken:
• All senior officials and Directorate General of Mines Safety (DGMS) were informed about the incident.
• Incident information was communicated to all concerned employees.
Family Details:
DP is survived by his spouse, one son and one daughter. All assistance, as per company norms,
is being extended to the family.
