This Contract made and entered into by and between:

Cavite Amateur League Basketball Officials, Inc. represented by ____________________ with
official address at______________________________, hereinafter referred to as the “Second Party”.
WITNESSETH:
WHEREAS, the SECOND PARTY is willing to undertake the services called for in its agreement
with the FIRST PARTY.
NOW, therefore, for and in consideration of the foregoing premises and the covenant herein
below, the parties hereto agree as follows:
That the terms of this agreement shall be for the entire duration of the

That the per game in pesos for Basketball / Volleyball for Eliminations and Semis is _________

That the per game in pesos for Basketball / Volleyball for Eliminations and Quarter Finals is ____

Games for semis and finals shall be 3 – man officiating.
That all the scheduled games played or not are considered paid in full.
