IV. Rationale:
Rationale:
Every school either public or private do its best effort for the advantage and well-being of its
learners. Hence, every learning institution aims towards improving the learning outcomes by recording
remarkable increments in the results of different assessments administered per quarter particularly in
reaching the standard MPS (mean percentile score of 75%) and above per learning area. Don Antonio
Bongolan Memorial High School (DABMHS) is currently under the recovery status when it comes to
curriculum and classroom instructions. Modifying the curriculum and classroom instruction will surely help
the school to unravel the learning gaps and losses. To fully recover from the learning gaps and learning
losses in curriculum and instruction, our school is set to organize, innovate, adjust, and implement
programs, projects, and activities (PPAs) to achieve 75% and above MPS in all learning areas. These
PPAs are normally different and diverse in content and strategies but perfect to support and achieve the
goal of the organization.
Hence, the school understands that in order to achieve the intended outcome, four priority areas
were identified with for specific school programs: Project BRAINS for literacy (English); Project BILANGIN
for numeracy (Math); Project ICARE for guidance and counselling and Project NOURISH for nutritional
gaps.
V. Objectives:
1. Improve the nutritional condition of thirty (30) learners with severely wasted status.
2. Provide supplemental feeding activities for learners with severely wasted status
until
it has improved.
3. Put up vegetable gardens in the school as a source of vegetables for the feeding
program.
VI. Beneficiaries: Don Antonio Bongolan Memorial High School learners, thirty (30) students.
VII. Resource Materials: SBFP Feeding Area, Utensils, Ingredients (Food and Water) Apron,
Gloves, Hairnet, Facemask
VIII. Budgetary Requirement: 25,000.00
IX. Source of Funds: MOOE, canteen funds and donations
X. Strategies:
1. Administer nutritional status to all learners to identify wasted and severely wasted.
2. Encourage stakeholders to support the school – based feeding program through financial
assistance.
3. Conduct regular monitoring of the nutritional status of the learners to check if they are
improving.
4. Assign parents volunteers to help the school – based feeding program through financial
assistance
5. Put up a vegetable gardens in the school as a source of vegetables for the feeding
program.
6. Conduct deworming activities for all learners to improve their immunity or to protect them
from chronic illness caused by worms.
XI. Expected Outputs
1. Administered nutritional status to all learners to identify wasted and severely wasted.
2. Encouraged stakeholders to support the school – based feeding program through
financial
attendance.
3. Assigned parents volunteers to help the school feeding coordinator plan, prepare, and
feed
the learners.
4. Conducted regular monitoring of the nutritional status of the learners to check if they are

them
from chronic illness caused by worms.
