4. Incase of authorization to collect the Certificate. attach PHOTOCOPY of ldentity
Cards (WITH PHOTO &SIGNATURE) Of the Candidate and the Authorized Person.
5. The fee willbe charged Rs. 100/- Per Degree/Certificate.
6. In case of more than 10 years old the fce will be charged 500/- per Degree/Certiticate.
ISSUE OF DUPLICATE DEGREEDIPLOMAJCERTIFICATE:
Duplicate degree/diploma/certificate can also be obtained on submitting an application
alongwith the following:
1. An affidavit signed and certified by the FIRST CLASS MAGISTRATE.
2. Cuting from the LEADING NEWSPAPER showing that the original has been lost or
destroyed, or submit defaced/remainíng portion of degree/diploma/certificate.
3. Prescribed fee: Rs. 500/
Time required for preparation/ssue of the certificate provided all other
required documents are attached.

Note : Old cases of more than 5years will require more time.
