Check list for Eye Irrigation

Step by step procedures: B 1,2,3 M 4,5,6 E 7,8,9


1. Greet patient introduce self and
verify patient’s identity.
2. Obtain patient’s consent by
explaining the procedure, risks,
benefits and complications.
3. Prepare the equipment:
✓ Faceshield
✓ Irrigating Solution
✓ 20 or 30ml syringe
✓ IV set
✓ Kidney Basin
✓ Underpad
✓ Clean gloves
✓ gauze
4. Perform hand hygiene and don
PPE (Personal Protective
Equipment).
5. Have the patient lie with the head
tilted on the affected eye, put on
under pad under the patient’s head.

6. Clean lids and lashes with gauze


moistened with water or normal
saline. Wipe from the inner to
outer canthus and use different
corner of the gauze with each wipe.
7. Place kidney basin at cheek on the
side of the affected eye, if the
patient is able, ask him to hold the
kidney basin..
8. Place thumb near the margin of the
of the lower lid and two fingers
above eye. Apply pressure
downward to expose the lower
conjunctival sac as the lower lid is
pulled down. Hold upper lid open
with your fingers.
9. Fill the irrigation syringe with the
prescribed fluid. Hold irrigation
syringe about 2.5 cm (1in) from the
eye. Direct flow of solution from
inner to outer canthus along the
conjunctival sac.
10. Irrigate until the solution is clear or
all the solution has been used.
11. Pause the irrigation and have the
patient close his eyes periodically
during the procedure.
12. Dry the periorbital area after
irrigation with gauze sponge. Offer
a towel to the patient if needed.

13. Assist the patient to a comfortable


position.
14. Doff PPE (Personal Protective
Equipment).
15. Document the procedure.

Check list for Instillation of eye drops


Step by step procedures: B 1,2,3 M 4,5,6 E 7,8,9
1. Inform the patient of the need and
reason for instilling drops or
ointment.

2. Allow the patient to sit with head


tilted backward or to lie in a supine
position.
3. Verify the patient’s identification.
4. Check written prescription and
bottle, vial, or tube for correct
medication.
5. Check prescription, designating eye
requiring drops.
6. Wash hands before instilling
medication
7. Remove cap from container and place
on clean surface
8. Using forefinger, pull on skin below
lower lid

10. Drop medication amount prescribed
into center of lower lid (cul-de-sac)
11. If ointment is to be instilled,
squeeze out 8 -inch ribbon of
medication (size of grain of rice) from
the tube into the lower lid without
touching the eye with the end of the
12. Instruct patient to close eyes slowly
but not to squeeze or rub them. Open
eye.

13. Clear any excess medication with


gauze or tissues.

14. If additional eye drops are ordered,


wait 30 -60seconds between each drop
15. Place finger over lacrimal duct for 1
minute after instilling drops if systemic
absorption is to be avoided, prolonged
corneal contact is desired, or taste or
smell of medication is distressing.
16. Wash hands after instilling
medication.

17. Record time

REQUIREMENTS

A clean tray containing the following:

▪ Epilating /cilian forceps, Wet cotton wool swab

▪ A good source of light

▪ Magnifying loupe

▪ Antibiotic drops/ointment

▪ Gloves
