A Cross-Over Study on Perceived Obtrusiveness of a Smartphone Selfie Recording App for
Fatigue Measurement: Comparing Frequency and Duration Parameters to User Preference

Please read the following, sign and return the form digitally or physically:

Taking part in the study


· I have read and understood the Participant Information Brochure.

· I have been able to ask questions about the study and my questions have been
answered to my satisfaction.

· I consent voluntarily to be a participant in this study and understand that I can refuse to
answer questions, and I can withdraw from the study at any time, without having to give a
reason.

· I understand that taking part in the study involves installing two app versions on my
Android smartphone for a total of 8 days, participating in two interviews, and sharing log files
(not videos) with the researchers.

· I confirm that I meet the eligibility requirements (age 18+, Android smartphone user,
daily phone usage, English communication ability).

· I agree to keep the video recordings on my device for 3 months after the study ends and
understand I can delete them after this period.

Use of the information in the study


· I understand that only log data will be shared with researchers, while the actual video
recordings will remain on my device.

· I understand that the information I provide will be used for a possible scientific
publication. Scientific publications will be supported by a pseudonymized dataset to protect
my anonymity.

· I agree that data collected through interviews and app logs can be used for a possible
scientific publication.

· I understand that my personal information (name, age, gender) will not be shared
beyond the study team. Data will be reported using group averages/totals of all participants.
Future use and reuse of the information by others
· I understand that the pseudonymized dataset can be used for future research and
learning by University of Twente researchers, which may be similar or different to this study.

· I understand that the anonymized dataset may be stored on University of Twente


servers or a recognized repository.

· I understand that I have the right to request the removal of my data within the 2-year
storage period by contacting the researcher.
