The attached Purchase Order and Notice of Award having been approved, notice is hereby
given to GODS LOVE OFFICE SUPPLIES AND EQUIPMENT TRADING that the delivery may
PROCUREMENT OF OFFICE SUPPLIES
commence/proceed for the project:
AND EQUIPMENT FOR CAMAGONG ES to Camagong Elementary School
within SEVEN (7) calendar days.

Upon receipt of this notice, you are responsible for delivering the goods awarded to your
company as a result of Canvass where you submitted the lowest calculated and responsive bid.
The said goods/services shall be under the terms and conditions stipulated in the specifications of
the project, and must be delivered within SEVEN (7) CALENDAR DAYS from the date after
receipt of this notice. After the deadline of delivery, the Camagong Elementary School will
deduct one- tenth (1/10) of 1% of the Contract Price from its Obligated Amount in each day
of failed delivery. Actual receipt of the goods delivered shall be performed by the Supply Officer
of the School.

Please acknowledge receipt and acceptance of this notice by signing both copies in the
space provided below. Keep one copy and return the other to the Camagong Elementary
School.
