Good day!

I, Kristine Joy G. Valenzuela, a senior high school faculty member at Asia Source iCollege,
would like to request your approval for the proposed performance task examination for Grade 12
students in their Personal Development subject.

This performance task involves the creation of a Career Plan based on a provided template,
followed by the design of a corresponding Poster. Through this activity, students will integrate
the concepts of personal and social relationships, as well as family structures and legacies, into
their final output. This task will serve as their 2nd Quarter Examination in Personal
Development, providing an opportunity for students to demonstrate self-awareness, creativity,
values integration, and critical thinking.

This addresses the learning competencies from the Curriculum Guide for Weeks 7–9 of the 2nd
Quarter, specifically:
1. Discuss external factors influencing career choices that may help in career decision
making (EsP-PD11/12CP-IIg-13.1)
2. Identify pros and cons of various career options with the guidance of parent, teachers, or
counselor (EsP-PD11/12CP-IIh-13.2)
3. Prepare a career plan based on his/her personal goal and external factors influencing
career choices (EsP-PD11/12CP-IIh-i-13.3)

GUIDELINES FOR CAREER PLAN AND POSTER
1. Each student will accomplish the Career Plan Template that will be provided in
printed form by the teacher.
o The template will serve as their written guide in reflecting on their chosen career,
identifying their strengths, values, and external influences that affect their career
decisions.
2. After completing the career plan, students will create a poster that visually represents
their chosen career path.
o This poster will serve as a creative expression of their personal goals and
understanding of career planning concepts.
3. The poster must include the following elements:
o The chosen career and clearly stated short-term and long-term goals
o The key skills, values, and interests that align with the chosen career
o The external factors (family, peers, mentors, socio-economic background, etc.)
that influenced the career choice
o An inspirational quote or message that reflects the student’s career vision and
motivation
4. The poster must be:
o Creative, organized, and meaningful, showing a clear understanding and
application of the concepts learned from the module on Career Pathways
o Made using 1/8 illustration board as the base
o Created using any art medium such as colored pencils, crayons, oil pastels,
poster colors, or paints — students are encouraged to express their individuality
and creativity in their design.
5. Both the Career Plan Template and the Poster must be submitted on or before
October 15, 2025.
o Late submissions will only be accepted under valid and approved circumstances.

• The plan is well-structured and ideas are logically presented.

• Demonstrates awareness of personal goals, family influence, and
external factors affecting career choices.
4. Grammar and Presentation 4 points
• The career plan is neatly written, clear, and free from major
grammatical errors.
TOTAL POINTS: 20 points

B. POSTER (30 points)

• The poster effectively integrates concepts of personal and social
relationships, family structure, and legacies.

• The design shows originality, creativity, and a strong visual

• Information is logically arranged and easy to understand.
4. Values Integration and Message 5 pts
• The poster conveys a clear message reflecting the student’s goals,
inspirations, and personal values.
5. Overall Impact 2 pts
• The output leaves a strong impression and motivates others.
