NOTE: 16-17 y/o must present a letter of consent from
parents or legal guardian
Must weigh 50 kilos or 110 pounds
Must be in good health
Must be free of cough, colds, fever and wounds
Must pass the series of donor screening

2.. In case patient is indigent, get a letter of endorsement from the hospitals social
services (SWA) or PCSO to avail 25% or 100% discount on blood handling fee.
