Description
On June 15th,2023 at NYAMYUMBA Site BV30 there is hazard A at the
entrance. there are many cracks. The set support is very needed
Recommendation
• Workers must not enter in this tunnel unless it is supported.
• We are requesting subcontractor to install a set support.
They are working on it but still in progress

Description
On June 18th,2023 at NYAMYUMBA Site BV31, there is hazard B in the ore pass connecting BV31 and
BV32. where there is soft ground so the hanging wall need to be supported.
Recommendation
• To install steel support

Description
On 19th June Gasambya site BV 17, there is C hazard. we have a problem of latrines
with no water for flashing( now there are in use, it might spread infectious diseases)
Recommendation
• We request subcontractor to find the buckets for storing water to be used in
flashing.

DESCRIPTION
At Karambo site Around 11:00 a.m. in M3 Main drive a mine assistant called MVUNABANDI
Gabriel got injured on his hand when he was doing barring.
Immediate action taken
• He has given first Aid

protection administrator were conducted Health and safety induction about:

TRAINING
PRACTICAL BARRING DOWN PROCEDURE TRAININGS

Description
On 14th June,2023 at Sanzari BV30 there were practical trainings of barring
down procedures in underground offered about:
Steps of barring down
Responsible person (every one is responsible)
How to identify the rock cracks by knocking the rock with pinch bar
Adequate materials used (sharp pinch bar, hammer) in barring down
Importance of barring down

Date: 14th June,2023
Description
On 14th June,2023 at Headquarter held to leaders including mine,
store and engineering supervisors, team leaders, safety officers,
security attended the third intake VFL(visible Felt Leadership)
training which aims to achieve zero accident in the road of no harm.

On 13th June,2023 Rutongo OHS committee meeting held in board room, it started by reviewing the last
meeting where there is great achievement on last action plans:
Implementation of safe declaration books
• Hazard identification
• Stop & Fix
Improvement of information sharing.
What must be undertaken:
Mine Audit, must be done with scoring underground activities by safety and mining team.
Arrangement of PPEs store must be well organized at end of this June.
To retrain and give the refreshment trainings to all workers in all aspects.
The main focus was to look together how to get safety reps in each crew and follow up to be made to
ensure that it has been implemented.
Next meeting will be held on 11th July,2023.
WEEKLY INFORMATION AND POINT TO DISCUSS
The conclusion of OHS committee meeting is to have safety rep in each crew especially for
subcontractors and cooperative side for promoting pre-entry examination.
If there is fire extinguishers been used, the supervisor must report it to SHEC department.
